• First Access allows a user to initiate and obtain managed access to a patient record to which they do not currently have access.

• Requires users to provide specific information in order to receive access to the patient record– Hard Stops: First/Last Name, DOB and Sex

– First Access to patient is currently set for 10 days

• Direct match only will permit linkage

• Does not permit “shopping for patient”

• Patient may not exist in UCSF system when an external provider is placing a Referral /eConsult or Radiology / Lab order to UCSF. Allows for end user to create patient and complete workflow w/o UCSF MD Link Team intervention.

• User will first search using “First Access” workflow to determine if patient already exist. If patient not found user will click “Create New”

• User completes the required data fields

• A list of possible duplicate patient records appears on the Patient Create Confirmation page.

• If no matching records are found, user clicks “Create New” to create a new patient record

• Message routed to MR New Pt. creation InBasket Pool

• Without First Access:– A referring physician, or covering PCP colleague cannot

access patient’s record

– A primary care physician is locked out if UCSF has not accurately entered the PCPs info (common)

– With the creation of BCP, and affiliations with MGH, SRMH and WHHS, a new cadre of affiliated physicians will come on line, initially relying on MD link. These physicians may not be PCPs. First Access allows timely access to the patient’s record

Issues:

• Without Patient Creation:

– Referring physicians cannot easily make use of the online referral process

– Ordering labs and imaging for non-UCSF patients will be impractical.

– E-consults are not feasible for non-UCSF patients
