JOE’S CARE PLAN – JANUARY 2012

My name is Joe Bloggs and this is my Care Plan.

My date of birth is: 1st January 2003

Legal Status: In Foster Care on a Care Order, this means that my social worker is responsible for working with everyone involved in caring for me to plan for my future and make decisions for me.

The overall care plan for me is to:

Live with Adam and Eve while my social worker works with the fostering team to look for a permanent foster placement for me.

The reason I am in care now is because:

On the 1st July 2010 my Mum dropped me at school and people thought my Mum had been drinking and so were worried about me. The social workers were called and went back to the flat after school with us. The social workers were very worried about the state of the flat, it was very dirty and cluttered, and that my Mum had been drinking. The social workers decided that my Mum was not able to look after me that evening and so they agreed with my Mum that I would go and stay with a foster carer, Dora. Then 3 days later we went to stay with Adam and Eve as the social workers decided that they would have to do some work with my Mum to see if she would be able to look after me. My Mum, Dad and I had lots of different assessments. Everyone went to court in June 2011 to talk to the Judge about what was the best plan for me. The Judge decided that neither my Mum nor my Dad would be able to look after me. After a lot of work and thinking what was best for me it was decided that I should stay in foster care until I am an adult.

I will be seeing people that are important to me:

Mum: Every other Thursday, supervised by Suzy, at the office or out and about

Aunty Karen, Uncle Richard and Peter: to be arranged in the school holidays

Friends: I will see them at school and my foster carers will arrange for me to see my best friend at other times when it’s the school holidays

My cousins: I am not seeing them yet but I would like to in the future

Dad: While my Dad is in prison he will write to me and I can write back to him if I want. He should be released in April or May next year. My social worker will then talk to me and my Dad and help me decide if I want to see him and if it is a good idea for this to happen.

The plans for my education include:To continue to attend Middle Street Primary School until all the plans about where I am going to live in the future are made. My Social Worker will talk with my teacher at a special meeting called a PEP to make sure I am doing the best I can at school.

My foster carers will make sure I go to the dentist and doctors and any other appointments as I need to and help me become better at looking after myself. My parents agree that this is a good idea too.

I will also be encouraged to carry on attending meetings with Polly, my therapist, who can help me with my feelings and my behaviour.

I will also carry on being helped to think about what are called my identity needs, which means the things that are important to me and are part of who I am.

This might include: carrying on going to church like I have with my Aunt and Uncle,

playing the guitar like I used to with my Dad,

or eating a vegetarian diet;

as well as seeing my family and friends and learning more about my life story and why I am in care now.

This Care plan will be talked about at my LAC REVIEWS. This meeting happens to make sure that I am getting all the help I need with my schooling, my health, my contact with my family and friends; my religious needs and any other interests I may have. The person who will be running this meeting will be my IRO whose name is Mickey Mouse.

If this plan has to change someone will talk to me about this and help me to understand why there are changes. They will also ask me what I think about the changes.

My Social Worker explained the detail of my Care Plan to me on 14th December these are my views/opinions about my Care Plan:

I would like to live with my Mum but if I can’t I want to carry on living with Adam and Eve. I want to see my cousin Jimmy as well as Archie. I do not want to see my Dad.
