1. AIM

This method statement describes the methods proposed to be used for Cutting and Bending of rebar at AGRU/SRU area on the Plateau Maintenance Project (PMP) in Ras Laffan Industrial City (RLIC). Kindly note that this arrangement is temporary awaiting approval of permit in the TSF for cutting and bending Rebar.

5. RESPONSIBILITIES

Site Manager: Shall be responsible for the proper execution of this procedure and to ensure that works are undertaken in a safe manner as described herein, to the requisite quality in accordance with relevant quality procedures and to the agreed timescale. He shall liaise and co-ordinate with TCJV, QG and other Subcontractors as applicable.

Safety Manager: Shall be responsible for the implementation of safety procedures on site and to ensure that the Job Safety Analysis within this method statement is followed. He shall assign a competent Safety Officer to enforce these procedures and to provide guidance to the operations where applicable.

Field QA / QC Manager: Shall be responsible for the proper implementation of these procedures and ensure that inspections and tests are carried out according to the relevant

approved Inspection Test Plan. He shall assign a competent Quality Control Inspector to perform inspections.

Construction Manager: Shall be responsible for the site implementation of these procedures and to provide all the resources from competent and trained manpower, tools, equipment and approved materials. He shall coordinate with the Quality Control Inspector (QCI) for inspections and issuance of Request for Inspections (RFI).

Safety Officer: Shall be responsible for the implementation of all safety requirements and to ensure that the works are undertaken in a safe and proper manner, all in accordance with the approved Construction Phase HSE Plan, the approved method statement and the approved Job Safety Analysis.

Quality Control Inspector: Shall be responsible for inspection and verification that the procedures are properly implemented and that inspections are undertaken in accordance with the approved Inspection Test Plan in a timely manner.

Rebar Foreman: shall be responsible for the proper delivery of rebar to the work place, correct cutting, bending and stocking of rebar in accordance with the approved procedures. He shall be responsible for the safe use of rebar equipment and plant and the safety of his workforce.

6. PROCEDURE

6.1 Permit to Work (PTW)TCJV & QG Work Permit Procedures shall be applied as required by this activity

All interfacing activities shall be fully coordinated with TCJV, QG and RLIC as applicable, to ensure the works proceed without incident.

For emergency response procedures please refer to Job Safety Analysis (Appendix 2)

6.2 Working AreasAs defined on the construction issue drawings.

6.3 Rebar

6.3.1 Rebar will be cut and bent within the AGRU/SRU area as per the attached drawing (see appendix 1) to approved bar bending schedules prepared by KCQ, which were developed from TCJV approved construction issued drawings. A small shade will be erected to be used as working area (see attached drawing).

6.3.2 Rebar will be supplied from Qatar steel (QASCO) or its authorised dealers in Qatar, which will either be manufactured directly by QASCO or be imported by them and shall comply with A615 G60 as per QG specification and approved material submittal.

6.3.3 Rebar will be stored on sleepers and clear off the ground and will be covered by polyethylene sheets at all times.

6.3.4 Rebar will be detailed, cut and bent in accordance with BS 4466.

6.3.5 Care shall be taken to ensure full compliance with minimum radius bending during fabrication in accordance of the project specs and procedures.

6.4 Procurement of material Site procured materials to be used for works shall be submitted by using material approval

request as per QG procedure.

7.0 RESOURCES

The below detailed resources are indicative as to requirements and will be subject to change to ensure that sufficient resources are applied to comply with the approved timelines required.

7.1 Plant & Equipment

8.0 QUALITY CONTROL

8.1 Compliance

8.1.1 Site Manager, Section Engineer, Supervisor and Quality Control Inspector will closely supervise and monitor all phases of the work to ensure full compliance with the approved Project Quality Plan (PQP) and project specification.

8.2 Inspections and Tests

8.2.1 Inspections and tests will be in full compliance with the approved Inspection Test Plan (ITP) and in accordance with the RFI procedure 199/00/00/QA/SW/NA/000831. Notification to TCJV QC team by KCQ of a formal inspection will be made a minimum of 24 hours of working day notice (48 hours for weekend and holidays), RFI will only be submitted when witness or hold points are specified in the approved ITP’S.

9.0 HEALTH, SAFETY & ENVIRONMENT (HSE)

9.1 Compliance

9.1.1 Subcontractor and any persons working for the subcontractor shall comply with this method statement including appendices and follow the rules and guidelines as set out in the approved Construction Phase HSE Plan and to any instructions given by TCJV and or QG.

9.1.2 Subcontractor to keep TCJV fully informed at all times of any issues regarding HSE.

9.2 Safety Instructions

9.2.1 All personnel involved in the activity shall undergo TCJV and QG safety induction courses as required. If the area is a Brown field area they shall undergo additional orientation training, to be performed by TCJV and or QG as required.

9.2.2 All personnel involved will use necessary PPE as required for the task.

9.2.3 The workers will be made fully aware of safety requirements related to the activity through Daily Task Instruction prior to works commencing. Attendance shall be recorded, maintained and attached to PTW site copy.

9.2.4 All required PTW’s and supporting documents shall be acquired and kept available at respective work locations.

9.2.5 No homemade tools are allowed on site

9.3 Job Safety Analysis

Job Safety Analysis shall be conducted for rebar works (cutting and bending) and be fully complied with, after discussed at the tool box talk.

See Appendix 2.

9.4 Personal Protective Equipment

The following Personal Protective Equipment (PPE) shall be used during the execution of the work:

Safety Helmet Coverall (Nomex type ) Safety Shoes Hand Gloves / leather gloves for task Dust Mask (dusty conditions) Safety Glasses (dark in sunlight, clear in night working) Escape Respirators Toxic Clips (Brown Field Area 1 per person / Green Field Area 1 per 5 persons and 1 per

person for confined space)

Further PPE as required by the task will be used.

9.5 Protective Measures

9.5.1 All work areas shall be hard barricaded and posted with safety warning signs before any works commence. Barricades shall be formed with scaffolding pipes and shall be subject to TCJV and QG approval before being put in use.

9.5.2 Where applicable continuous monitoring of the works will be undertaken for H2S, to ensure a safe environment exists at all times for the workforce

9.5.3 All vertically exposed rebar will be capped for safety.

9.5.4 Where required safe walkways shall be placed on rebar for safe movement of personnel.

9.5.5 Rest shading and drinking station for workers shall be provided

9.5.6 During night works (if any), sufficient task and access lighting will be provided around the working areas.

9.5.7 During summer season heat stress program as initiated by TCJV and QG should be implemented and emphasized.

9.6 Access to Site

9.6.1 Subcontractor and any personnel under his authority shall strictly follow the approved procedures for site access for plant, equipment and personnel.

9.7 Plant & Equipment Control

9.7.1 All drivers and operators shall be competent persons and have a Qatari Driver’s License and approved 3rd Party Certification where required.

9.7.2 Defence driving training and 3rd Party Certification will be required for drivers entering Qatargas OPCO areas.

9.7.3 All plant and equipment operating within Qatargas OPCO areas to possess QG “Blue” inspection sticker and project inspection sticker.

9.7.4 All plant and equipment will be approved 3rd Party Certification.

9.7.5 All plant and equipment shall be inspected and recorded on a daily basis.

9.7.6 If a vehicle meets an emergency call QG emergency number 4473-7777 or RLIC emergency number 4474 8888.

9.7.7 In case of Plant alarm, pull vehicle/plant/equipment to a safe location, render the vehicle/equipment safe, switch off ignition and proceed to nearest muster point on foot.

9.7.8 All the traffic rules set by RLIC, QG and TCJV shall be obeyed strictly.

9.7.9 Banksmen, fire watch and Riggers must be QG approved and certified.

9.7.10 Riggers must be third party certified as per QG requirements.

9.7.11 All equipments must be equipped with fire extinguisher individually.

10.0 APPENDICES
