Information to Parents/Guardians of Middle School and High School Students Regarding the Maryland Youth Risk Behavior Survey/Youth Tobacco Survey
This section of the form is to notify you about the Maryland Youth Risk Behavior Survey/Youth Tobacco Survey (YRBS/YTS) and procedures to follow if you DO NOT want your child to participate in the YRBS/YTS.
Your child’s school may be taking part in the YRBS/YTS, conducted by the Maryland Department of Health (MDH) in collaboration with the Maryland State Department of Education (MSDE) and the Centers for Disease Control and Prevention (CDC). The survey was designed by the CDC to identify risk behaviors that may include safety behaviors such as use of helmets and seat belts, depression and mental health; use of tobacco, alcohol, or other drugs; nutrition and physical activity; and sexual behavior.
The survey has been designed to protect your child’s privacy. The survey is confidential and students will not put their names on the survey. No school or student will ever be mentioned by name in a report of the results.
The survey is voluntary. If your child is not comfortable answering a question, your child may skip it. No action will be taken against the school, you, or your child, if your child does not take part. In addition, students may stop participating in the survey at any point without penalty.
If you have any questions about your child’s rights as a participant in this survey, or if you feel your child will be harmed in any way by taking part, please call toll-free 1-877-878-3935, leave a message including your name and phone number, and someone will call you back as soon as possible. FREQUENTLY ASKED QUESTIONSQ. Why is the Maryland Youth Risk Behavior Survey/Youth Tobacco Survey (YRBS/YTS) conducted?A. The MDH and the MSDE will use the results from the YRBS/YTS to (1) monitor how priority health risk behaviors among middle and high school students change
over time; (2) evaluate the impact of broad state and local efforts to prevent health risk behaviors; and (3) improve school health education policies and programs.
Q. Are sensitive questions asked?A. Some questions may be considered sensitive by some districts, schools, or parents/guardians. All such questions are presented in a straightforward and sensitive
manner and were designed by the CDC. Topic areas covered include use of helmets and seat belts; depression and mental health; use of tobacco, alcohol, other drugs, nutrition and physical activity; and sexual behavior.
Q. Will student names be used or linked to the surveys?A. No. The survey is designed to protect your child’s privacy. The survey is administered by specially trained field staff. Students do not put their name on the survey.
When students finish the survey, they place the completed survey in a large box or envelope.
Q. Are students tracked over time to see how their behavior changes?A. No. Students who participate cannot be tracked because no identifying information is collected.
Q. How are children picked to be in the survey?A. Statewide, approximately 360 schools and 85,000 students are picked to take part. First schools are randomly picked, and then classrooms in selected schools are
randomly picked. Every student in a selected class may participate.

STATEMENT OF COMMITMENT
As a volunteer working in Montgomery County Public Schools, I agree to—
Sign In/Out Sign in and out of the building using the Visitor Management System (VMS), or as otherwise directed, at the beginning and end of each volunteer activity and wear a volunteer identification name tag at all times. The VMS enables staff to scan a visitor’s driver’s license or identification card to produce a visitor badge and cross-reference the visitor’s information with state and local sex offender registries.
Commitment Honor the commitment to volunteer as scheduled and notify the school in advance if you cannot fulfill the commitment.
Confidentiality Volunteers may not access confidential student or personnel records, and should otherwise respect the confidentiality of students.
Student Contact Volunteers working with students must be within view of others (e.g., either through a window in a door, a door left open, or in a public space). In some programs, such as mentoring, volunteers may be required to spend a brief amount of time one-on-one with a student. Volunteers can manage these situations by avoiding physical contact, only going to authorized destinations, and reporting activities or whereabouts to the principal or supervisor.
Discipline Volunteers may not discipline students, but should report behavioral problems to the supervising staff member. Volunteers should address any concerns about student or school safety to the principal or other appropriate staff member.
Board Policies and MCPS Regulations
Volunteers must comply with all Montgomery County Board of Education policies and MCPS regulations and rules.
Training All volunteers who regularly support schools, and school sponsored activities, such as: student teachers and interns; volunteers who regularly help with school dismissal or recess; as well as those who attend field trips, and chaperone outdoor education, must complete the MCPS online Recognizing and Reporting Child Abuse and Neglect volunteer training. All other volunteers are encouraged, but not required, to engage in this important, informative training. For list of volunteers who are not required to complete the MCPS online volunteer training, see MCPS Regulation IRB-RA.
Online child abuse and neglect training for volunteers is provided on the MCPS website, in English and the languages most requested by MCPS students and parents/guardians.
Frequency of Training
Beginning with the 2017-2018 school year, volunteers will need to complete the child abuse and neglect training at least once every three years to continue volunteering.
Criminal Background Checks and Fingerprinting
The following categories of volunteers are required to complete criminal background checks, including fingerprinting—• Volunteer coaches;• Grade 6 Outdoor Environmental Education Programs (Outdoor Education) overnight chaperones;• Volunteers on overnight field trips;• Volunteers on other field trips that extend beyond 7:00 p.m. in the evening (e.g. out of area, out of state), and/or trips that require Office of School
Support and Improvement (OSSI) approval, as determined by OSSI;• Other volunteers as designated by MCPS.
Accidents Any accident or injury involving a volunteer must be reported consistent with MCPS Regulation EIB-RA, General Liability Insurance.

The Community Action Agency (CAA) of the Montgomery County Department of Health and Human Services is the grantee for the Montgomery County Head Start program. Montgomery County Public Schools (MCPS) operates the Head Start program in Montgomery County as a designee of the Department of Health and Human Services.
Head Start regulations require grantees follow certain rules that apply only to children riding buses while being transported to a Head Start program. Among other things, those rules require that children:
• be transported in an age appropriate child restraint;
• have an adult monitor on the bus and;
• not ride in vans.
The CAA does not operate or provide transportation for the Montgomery County Head Start program.
However, if you wish to have your child transported at no cost by a MCPS school bus, you may request this service by completing and signing the form below. Your child will be transported in an MCPS school bus that adheres to all federal and state safety regulations and requirements for transportation of all other students in Prekindergarten through Grade 12. Otherwise, you may wish to provide transportation yourself for your child.

La Agencia de Acción Comunitaria (Community Action Agency - CAA) del Departamento de Salud y Servicios Humanos del Condado de Montgomery es la entidad concesionaria del programa Head Start del Condado de Montgomery. Montgomery County Public Schools (MCPS) opera el programa Head Start en el Condado de Montgomery como designado por el Departamento de Salud y Servicios Humanos.

Pedestrian Safety: Tips for Parents/Guardians
1. Hold your child’s hand when you walk together along the street. 2. Walk on sidewalks. If there is no sidewalk, walk facing traffic. 3. Cross at intersections and in the crosswalk. 4. Stop at the curb and look left, right, and left again for traffic. (Children who don’t
know left and right can be taught to look “this way, that way, and this way.”) 5. Wear bright colors or reflective clothing if you are walking near traffic at night.
Carry a flashlight when walking in the dark. 6. Talk to your child about street safety. 7. Set a safe example. Show and explain to your child the safe way to cross streets.
Give plenty of time to practice. Praise your child for copying safe behavior. 8. Teach your child that seeing the driver in a car does not mean that they see them. 9. Teach your child: Never run into the street. If a toy or pet goes out into the
street, ask an adult for help getting it back. 10. Find safe places to play. Keep children away from traffic. Fenced yards, parks, or
playgrounds are good places for your child to play. 11. Listen to traffic helpers—patrols and crossing guards are there to help you.
10 Basic Bus Safety Rules Parents/Guardians should review with the rules with their children so they can learn how to stay safe when riding to and from school. Children should learn to always be aware of the street traffic around them. Drivers are required to follow certain rules of the road concerning school buses, however, not all do. Teach them to protect themselves and watch out. 1. Be on time (Never run to or from the bus). 2. Have a safe place to wait for your bus, away from traffic, and away from the
street. 3. Stand back from the curb until the bus has come to a complete stop. 4. Wait for the driver’s signal before crossing. 5. Always cross at least 10 feet in front of the bus. 6. Do not push or shove. 7. Stay in your seat. 8. Do not yell or shout. 9. Always obey the driver. 10. Never crawl under a school bus

MARYLAND SCHOOLS RECORD OF PHYSICAL EXAMINATION
To Parents or Guardians:
In order for your child to enter a Maryland public school for the first time, the following are required:
• A physical examination by an authorized health care provider must be completed within nine months prior to entering the public school system or within six months after entering the system. A physical examination form designated by the Maryland State Department of Education and the Maryland Department of Health must be used to meet this requirement.
• Evidence of complete primary immunizations against certain childhood communicable diseases is required for all students in preschool through the twelfth grade. A Maryland Immunization Certification form for newly enrolling students may be obtained from the local Department of Health and Human Services or from school personnel. The form and the required immunizations must be completed before a child may attend school. (Form MDH 896).
• Evidence of blood lead testing is required for all students who reside in a designated at risk area or who are enrolled in Medicaid when first entering Prekindergarten, Kindergarten, and Grade 1, and for ALL children born on or after January 1, 2015. The Maryland Department of Health and Mental Hygiene Blood Lead Testing Certificate (DHMH 4620) (or another written document signed by an authorized health care provider) shall be used to meet this requirement.
Exemptions from immunizations are permitted if they are contrary to a student’s or family's religious beliefs, and require parent/guardian signature on MDH Form 896. Students also may be exempted from immunization requirements if an authorized health care provider certifies that there is a medical reason not to receive a vaccine. Exemptions from blood lead testing is permitted if it is contrary to a family’s religious beliefs and practices. The Blood Lead Testing Certificate must be signed by an authorized health care provider stating a questionnaire was done.
The health information on this form will be available only to those health and education personnel who have a legitimate educational interest in your child.
In order to assist your child in gaining the most from their educational experience, please complete Part I of this Physical Examination form. Part II must be completed by an authorized health care provider, or attach a copy of your child’s physical examination to this form. If your child requires medication and or a treatment to be administered in school, you must have the authorized health care provider complete a medication and or treatment administration form for each medication and or treatment to be administered. These forms can be obtained from your child’s school or online from the Montgomery County Public Schools (MCPS) website at www.montgomeryschoolsmd.org: MCPS Form 525-12, Authorization to Provide Medically Prescribed Treatment, Release and Indemnification Agreement, MCPS Form 525-13, Authorization to Administer Prescribed Medication, Release and Indemnification Agreement, MCPS Form 525-14, Emergency Care for the Management of a Student with a Diagnosis of Anaphylaxis, Release and Indemnification Agreement for Epinephrine Auto Injector. In order to provide the best learning experience for your child, teachers need to be aware of any special health or developmental problems that your child may have. Documentation of a student’s health status is required as noted below.
PHYSICAL EXAMINATION

• Children who do not have health insurance may be eligible for the Maryland Children’s Health Program (MCHP) or the Montgomery County Care for Kids (CFK) Program. Both programs offer preventive health check-ups, sick care and other services. For assistance in applying for MCHP or CFK, please contact the Head Start Income Assistance Program Specialist, or visit the Office of Eligibility and Support Services (OESS) according to your zip code.
DENTAL EXAMINATION
• A dental examination is required for the Pre-K/Head Start Program
• The Dental Health Form should be completed by a dentist/dental hygienist.
TUBERCULOSIS
Freedom from tuberculosis (TB), documented by a negative skin test or chest x-ray, is recommended for students who enter MCPS from outside of the United States, including all children who have lived outside the U.S. for 12 months or more.
Skin tests may be obtained from private health care providers or for no cost at the Montgomery County DHHS, School Health Services (SHS) Immunization Center, 4910 Macon Road, Rockville, Maryland, 240 740-4430.
Individuals with positive skin tests must be cleared by chest x-rays before attending school. Chest x-rays are available through private physicians or Montgomery County DHHS, TB Control Program, 2000 Dennis Avenue, Silver Spring, 240-777-1800.
IMMUNIZATIONS
• Proof of the following immunizations must be certified by your authorized health care provider on the Maryland Immunization Certificate Form 896 (MDH Form 896):
• 4 DPT/DTAP (Diphtheria, Pertussis, and Tetanus)
• 3 Polio
• 1 MMR (Measles, Mumps, and Rubella) on or after 12 months of age (2nd required by fifth birthday)
• 1 Hib (Haemophilus Influenza type b) on or after 12 months of age
• 3 Hepatitis B
• 1 Varicella (Chicken Pox) on or after 12 months of age
• 1 Pneumococcal Conjugate (Prevnar)

Estimado Padre/Madre/Guardián:
A fin de proveer la mejor experiencia educativa para su hijo/a, los maestros necesitan estar al tanto de cualquier problema especial de salud o del desarrollo que su hijo/a pueda tener. Se requiere documentación del estado de salud del alumno según se indica a continuación.
EXAMEN FÍSICO
• Debe incluir una revisión de todos los sistemas, incluyendo:
• Prueba de plomo en la sangre
• Análisis de sangre para detectar anemia (hemoglobina y hematocrito)
• Pruebas de audición y visión
• Medición de altura y peso
• Medición de la presión arterial
• Evaluación diagnóstica del desarrollo
• Examen de sangre para detectar células falciformes (si fuese indicado)
• Los niños que no tengan cobertura de seguro de salud podrían ser elegibles para el programa de salud infantil de Maryland (Maryland Children’s Health Program–MCHP) o para el programa de atención médica para niños del Condado de Montgomery (Montgomery County Care for Kids–CFK). Ambos pro-gramas ofrecen chequeos preventivos de la salud, atención de enfermedades y otros servicios. Para obtener ayuda para solicitar MCHP o CFK, comuníque-se con el especialista del Programa de Asistencia sobre Ingresos de Head Start o visite la Oficina de Elegibilidad y Servicios de Apoyo (Office of Eligibility and Support Services–OESS) correspondiente a su código postal.
EXAMEN DENTAL
• Se requiere un examen dental para el Programa de Pre–K/Head Start
• El formulario de salud dental debe ser completado por un dentista/higienista dental.
TUBERCULOSIS
Se recomienda una prueba cutánea o una radiografía de tórax con resultado negativo que documenten la ausencia de tuberculosis (TB) para los alumnos que ingresan a MCPS desde fuera de los Estados Unidos, incluyendo todos los niños que hayan vivido fuera de los Estados Unidos durante 12 meses o más.
Las pruebas cutáneas se pueden obtener de proveedores de atención médica privados o sin costo alguno de DHHS del Condado de Montgomery, en el Centro de Inmunización de Servicios de Salud Escolar (School Health Services–SHS), 4910 Macon Road, Rockville, Maryland, 240 740-4430.

HOW TO USE THIS FORM
The documented tests should be the blood lead tests at 12 months and 24 months of age. Two test dates and results are required if the first test was done prior to 24 months of age. If the first test is done after 24 months of age, one test date with result is required. The child’s primary health care provider may record the test dates and results directly on this form and certify them by signing or stamping the signature section. A school health professional or designee may transcribe onto this form and certify test dates from any other record that has the authentication of a medical provider, health department, or school. All forms are kept on file with the child’s school health record.

CÓMO USAR ESTE FORMULARIO
Las pruebas documentadas deben ser las pruebas de plomo en sangre a los 12 meses y 24 meses de edad. Se requieren dosfechas de pruebas y resultados si la primera prueba se hizo antes de los 24 meses de edad. How To Use This Form
The medical provider that gave the vaccinations may record the dates (using month/day/year) directly on this form (check marks are not acceptable) and certify them by signing the signature section. Combination vaccines should be listed individually, by each component of the vaccine. A different medical provider, local health department official, school official, or child care provider may transcribe onto this form and certify vaccination dates from any other record which has the authentication of a medical provider, health department, school, or child care service.
Only a medical provider, local health department official, school official, or child care provider may sign ‘Record of Immunization’ section of this form. This form may not be altered, changed, or modified in any way.
Notes:
1. When immunization records have been lost or destroyed, vaccination dates may be reconstructed for all vaccines except varicella, measles, mumps, or rubella.
2. Reconstructed dates for all vaccines must be reviewed and approved by a medical provider or local health
department no later than 20 calendar days following the date the student was temporarily admitted or retained.
3. Blood test results are NOT acceptable evidence of immunity against diphtheria, tetanus, or pertussis (DTP/DTaP/Tdap/DT/Td).
4. Blood test verification of immunity is acceptable in lieu of polio, measles, mumps, rubella, hepatitis B, or varicella vaccination dates, but revaccination may be more expedient.

Immunization Requirements
The following excerpt from the MDH Code of Maryland Regulations (COMAR) 10.06.04.03 applies to schools: “A preschool or school principal or other person in charge of a preschool or school, public or private, may not knowingly admit a student to or retain a student in a: (1) Preschool program unless the student's parent or guardian has furnished evidence of age appropriate immunity
against Haemophilus influenzae, type b, and pneumococcal disease; (2) Preschool program or kindergarten through the second grade of school unless the student's parent or guardian has
furnished evidence of age-appropriate immunity against pertussis; and (3) Preschool program or kindergarten through the 12th grade unless the student's parent or guardian has furnished
evidence of age-appropriate immunity against: (a) Tetanus; (b) Diphtheria; (c) Poliomyelitis; (d) Measles (rubeola); (e) Mumps; (f) Rubella; (g) Hepatitis B; (h) Varicella; (i) Meningitis; and (j) Tetanus-diphtheria-acellular pertussis acquired through a Tetanus-diphtheria-acellular pertussis (Tdap) vaccine.”

Healthy vision and hearing is important for your child’s success in school. Difficulties in hearing and seeing that are not recognized and treated may impact your child’s ability to see and/or hear. Seeing and hearing are necessary to learn and develop social relationships. State of Maryland Education Law 7-404 requires school children to have their hearing and vision screened. In compliance with this law, students new to MCPS, including PreK and Head Start will be screened for possible hearing or vision problems. Trained hearing and vision screeners will provide the screenings at your child’s school.
You will receive a referral form, via US mail, to take to your child’s authorized health care provider if your child fails the screening. If your child is already under care for a hearing or vision problem, or if you do not wish your child to be screened at school because of conflicts with your religious beliefs, please notify the school nurse in writing.
If your child wears glasses or contacts they should wear them every day and bring them to the screening.
School based vision screenings may not detect every vision problem. CHILDREN at RISK for VISION PROBLEMS
Vision screening is not a substitute for and does not replace a comprehensive eye exam performed by an optometrist or ophthalmologist. At risk groups, including the following children, are encouraged by the National Center for Children’s Vision and Eye Health to have a comprehensive eye examination by an optometrist or ophthalmologist.
• Eyes that do not look normal
• Neurodevelopmental disorders such as hearing impairments, cerebral palsy, Down’s syndrome, autism spectrum disorders, speech delay
• Health conditions with known effects on the eye—diabetes and rheumatoid arthritis
• Parents or siblings with vision problems
• Prematurity of less than 32 weeks
• Parents/guardians who believe their child has a vision problem
VISION PROBLEMS
• Amblyopia, also called lazy eye, is blurry or reduced vision that is not corrected by glasses. Crossed eyes, eyes that don’t line up or one eye that focuses better than the other can cause amblyopia. Amblyopia can be treated.
• Astigmatism is blurry vision that occurs when the cornea (clear front cover of the eye) has an irregular shape. Children with astigmatism may need glasses/contacts.
• Farsightedness (hyperopia) occurs when the eyeball is too short for focusing. Images of near objects may appear blurred. Hyperopia may be corrected with glasses/contacts.
• Nearsightedness (myopia) occurs when the eyeball is too long for focusing. Images of distant objects appear blurred. Myopia may be corrected with glasses/contacts.
• Strabismus is crossed eyes or eyes that do not line up with each other. If untreated, strabismus can cause amblyopia.

Es importante que su hijo/a tenga una visión y una audición saludables para su éxito en la escuela. Las dificultades de audición y visión que no se reconocen y no se tratan pueden afectar la capacidad de su hijo/a para ver y/u oir. Ver y oír son necesarios para aprender y desarrollar relaciones sociales. La Ley de Educación 7-404 del Estado de Maryland exige que la visión y la audición de los niños en edad escolar sean evaluadas. Income Assistance Program Specialist Information
Children who do not have health insurance may be eligible for the Maryland Children’s Health Program (MCHP) or the Montgomery County Care for Kids (CFK) Program. Both programs offer preventive health check-ups, sick care and other services. For assistance in applying for MCHP or CFK, please contact the Head Start Income Assistance Program Specialist, or visit the Office of Eligibility and Support Services (OESS) according to your zip code.
What to Bring:
1. Proof of income. For example, pay stubs, tax forms, W-2, letter from employer, etc.
2. Proof of identity for all applicants. For example, driver’s license, employment ID, child’s birth certificate, passport, Green card, Arrival/Departure Record/I-94, etc.
3. Proof of Montgomery County residence (please bring one of the items below). For example, lease, utility bill (gas, electric, water, land line phone), school records, etc.
4. For MCHP only: Social Security Number/Card or immigration/citizenship documents
