Instructions: This questionnaire should be completed by the
property owner(s) or by a direct family member,preferably over the
age of 18 years. Please answer the following questions in the space
provided and feel free toattach an extra page if necessary. If you
are uncertain about the answers, it is okay to leave the answer
spaceblank. Please return the completed questionnaire no later than
September 18, 2009 to Joe Harwood. There will ba random draw for
LLPOA towels from all completed questionnaires received. Reponses
will be used by U-Linksstudents to prepare the report on Loon Lake
