Nomination Form

Thank you for being a part of the JYSA Booster Club. As a member you are entitled to a say in how your program is run. It is time to nominate and elect the following positions for 2018:

Secretary, Treasurer and Booster Club President.

You will be able to make your nomination for the open positions in 2 ways:

1. Complete this form and mail to: PO BOX 2378; Jasper, GA 30143. Must be postmarked no later than December 7th.

2. Or email your completed form to board@jysadragons.org.

NOMINATION FORMS MUST BE RECEIVED BY DECEMBER 10th.

I would like to nominate for a board member position for JYSA for the 2018 year. (Nominations may be submitted on behalf of yourself or someone else.)

Elections will be held online and at the December Board Meeting on Thursday, December 21st. Voting information will be sent out on December 11th (if we receive more nominations than there are open positions). Votes will be counted and winners announced at the December 21st meeting. Positions to be held will be determined at the meeting once the board members have been elected (subject to clear background checks).

PLEASE NOTE – Nominations made by a 3rd party must be confirmed with and accepted by the candidate being nominated before ballots will be sent out.
