    INTRODUCTION

    MENTAL HEALTH AND DISORDER

    Our societies have both constructive as well as destructive features. Destructive does not always

    mean to be demolished or destroyed. In this context it can be referred to burdens on the societywhich may hamper the development of the society in whole. From them mental disorder or

    impaired mental health is one of the burden that our society is facing. Even though medical

    science has done fast progress in the last century in this sector with the coming of genetics and

    biotechnology along with improvement in psychology still many interventions are not available

    to the common man due to many barriers. So to get rid of this problem many organizations who

    work in mental health sector were trying hard to come with such packages that it will help to

    meet the needs of the client as well as the society.

    STRUCTURE OF THE ESSAY

    This essay will show how new packages and services given by the mental health service is

    helpful in different way and how it can be applied to both the client and the society people. What

    all are included in those service packages and for which age groups as well as what more can be

    done in this sector so as to minimize the consequences.

    NEED FOR MENTAL HEALTH SERVICE PACKAGE

    Nothing happens in this world without a valid and logical reason behind it. Similarly we have to

    see why there is a need of packages and services from the department of mental health when

    many other disorders still haunts the common man.

    MENTAL HEALTH PROBLEMS -EFFECT ON GLOBAL SCALE

    The global sharing of the mental disorder stands at 12% in total disease section of the world,

    which is expected to increase to 15% by the end of this decade that is 2020. It first makes people

    disable later forcibly transfer them to illness category. Those people are discriminated all over

    the world. In developing countries till today they are not able to give minimum assistance to

    those who suffers from this problem.(Jacob et al, 2007)

    SOCIAL AND ECONOMIC EXPENDITURE FOR MENTAL HEALTH DISORDERS

    Problems need to be addressed when they have substantial effect on the society as well as theeconomy of a country. If we will take the case of USA the so called richest country in the world,

    the indirect cost behind the treatment of mental health disorder is 148 billion dollars, which

    outweighs direct investment behind other diseases by two to four times which will be very high

    in other developing nations where the direct cost is very low for different type of diseases. The

    social cost is also high as the burden of cost has to bear by the family members and sometimes

    one family bear for the others too as nobody is there to take care of the person. The groups who

    were most vulnerable to this problem are the young adults who form the most productive

    population of a country. So it adds different type of stress to the family as well as the patient who

    is suffering from thus making this problem both social and economic.(World health report,

    2001).

    FUNDINGS AVAILABLE FOR THIS SECTOR.

    If we will analyze the fundings as well as resources that these sectors gets from their respective

    governments then there is a huge question mark on this that is why this area is so neglected even

    though it takes a great toll on the cost? Statistics says the developed countries whose percentage

    in whole world is 37% they spend less than 1% of their total health budget after the mental

    health, while its still less in developing nations. If the fundings will be compared to the other

    disease sector such as AIDS and cancers it seems to be a handful of water for sea. So this needs

    to be addressed seriously.(WHO, 2007)

    MENTAL HEALTH POLICY AND PROGRAMME

    The first thing which is needed is the development of a mental health policy that will effectively

    implement other plans and programmes which will be helpful in this regard and will mitigate the

    problem of the mental health problems that society is currently facing. So we will see different

    service packages which are made available to the people of society by the health department of

    various governments as well as different public sectors in mental health service providers.

    METAL HEALTH AND PRIMARY HEALTH INTEGRATION

    The primary health care is an important sector in health care department doesnt matter if it is

    provided by the government or private sector. As it provides curative, preventive and generalhealth treatments to the common man so integrating the mental health services to the primary

    care will help in this regard as it will be available to the common man at ease as primary care is

    available to all. So this helps in making the treatments available to the common man and other

    patients as well as the clients too who can get insurance policies there also with ease.(WHO,

    2005)

    TRAINING FOR THE HEALTH PROFESSIONALS AS WELL AS THE NGOS

    This module in the package includes training of those health professionals who were in the

    mental health sector. The rationale behind this is most of the mentally challenged person hardly

    get a chance to see the doctors as well as other health professionals. Even if they get a chance to

    see the doctor but till the treatment received by them hardly is effective in nature because most

    of the time the treatments that are made available to them is not to the point due the untrained

    staffs in this sector. So the package comes with the training of the individual who were in the

    health care sector that too in mental health treatment.

    Training module is also available for the common man as well as the different persons involved

    in the different stages of the health care sector. For common man training is made available

    especially to deal with the patients as well as how to do better effective communication. This

    will be beneficial for both the societies and clients as it helps in the effective treatment of the

    patients.

    LOCAL HOSPITALS AND CLINICS

    The mental health department comes with another package that is making the availability of

    mental health treatment at local hospitals as well as the local clinics through well trained staffs

    who can do screening and diagnosing of the disease properly as well as the monitoring the

    medications which were mostly psychotropic. Those patients who cannot be treated with the

    facilities available to them are referred to the next up level hospitals and transport facilities were

    also available for the serious patients along with proper information why they are referred to the

    other hospitals.

    Many types of facilities were also made available to the inpatient under this scheme such as 4-6

    beds will be provided to the psychiatric patients in each ward in every hospital as well as

    providing enough space for the outpatient. Besides the ward consultants and doctors there will be

    nurses and social workers to assist the family whose person is suffering from mental disorder as

    well as patient. All types evaluation should be carried out so that if any false positive symptom

    can be rechecked. Discharge plans should be made available to the family assisting the patients

    along with the primary health care plan as it will help them to know in which way the treatment

    is proceeding and how effective it is? Help will be provided in different sectors such as health,

    food, work, security etc so that if anybody who is suffering and have no body doesnt go to road.

    The confidentiality will be maintained for the patients from outpatient and inpatient caretakers.

    EMERGENCY UNITS FOR SUBSTANCE ABUSE CASES

    The substance abuse cases are on rise in every sector and same with the mental health sector too.

    So the package has also arrangement for those who were also addicted to substances and

    emergency units are available to treat both withdrawl and starting symptoms of substance

    assume. Necessary counseling is also provided so that they will not return to that path as well as

    detoxification will be done for those who have passed the tertiary level in the substance abuse.

    CARE CORDINATOR

    The role of care coordinator is very crucial in any of the health services. So the mental health is

    also the same where the package includes the care coordinator for each group of patients

    especially one or two for each ward so that those patients who were coming for treatment doesnt

    get deprived off any facility that is available in the hospital.

    COMMENCEMENT OF THE COMMUNITY MENTAL HEALTH TEAM

    Community mental health teams are those who can assist people suffering from mental disorders

    or any type of mental problems in their homes and can give effective solutions. The team must

    have psychiatrists, psychiatric nurses, mental medicine specialists, occupational therapists etc so

    that they can treat the patients effectively, along with this they are distributed in different sectors

    so that this facility could be availed by all the persons rather in a single special hospital or clinic.

    MAKING AVAILABILITY OF DIAGNOSTIC TOOL

    For any disease there exists an evaluator mechanism or tool, but at times as the health care

    professionals does not follow those guidelines they end up in misdiagnosis and wrong

    prescription of medicines. So now days the mental health department gives a structural

    evaluation framework for the right diagnosis of the disease which will be provided to all

    hospitals and clinics and that will be explained to the family members as well as the sufferers by

    trained personnel. The system has been developed in such a way that all the evaluation made was

    made to feed to the database where it is analyzed by the mental health department and then

    feedback is given to that hospital or clinic. Standardized protocol for the evaluation of all typesof mental diseases is also provided to the different institutes for better and effective treatment.(

    MAKING MENTAL HEALTH STUDY IN SCHOOLS

    The children are very sensitive both in physical and mental sphere. They tend to be more prone

    to any of the mental disorder which may be congenital or else acquired after birth due to any

    traumatic experiences or any such accidents which has left them mentally challenged. So the

    mental health service package also try to incorporate the mental health study at school level so a

    child need can be known and can be analyzed during various case studies. Another rationale behind this approach is to remove the stigma behind the mental illness as in most cases the

    patients are discriminated in the society so making the younger generation aware of the true facts

    both scientifically as well as logically which will also help in developing a better thought process

    and they will be aware of the facts and reasons behind the mental illness.( Institute of Medicine,

    FUNDING OF THE PATIENTS AS WELL AS MEDICAL COST

    Medical cost is one of the important factors which have always played a major role in any

    disease. In developing countries mostly people failed to meet the cost of the treatment as a result

    the mortality in most of cases rises even though it is not in the case of the mental health but still

    most of the families cannot meet the cost that has to be incurred during the treatment of mental

    health disorder. So the package includes financing the families as well as the patients through

    suitable insurances that will help them in bearing the cost of the treatment as well as help the

    insurance clients to get there business. The package also includes providing jobs according to

    their capability to the patients or any members of the family so that they will not feel that they

    have been discriminated in the society as they are mentally challenged.

    So these are some of the services that are provided by the different government as well as private

    sector health departments or hospitals in mental health which is really beneficial to the client as

    well as the society.

    CONCLUSION

    Development of packages as well as services and its effective implementation is a complex

    process. There are many factors that have to be considered while developing a package and the

    share of the stakeholders or clients has to be kept in mind while developing. The package which

    the mental health department provides may vary from place to place or from country to country

    based on their economy. Even though the process is complex for a country, but it is

    recommended that the country must come with a suitable policy for mental health so that it will

    be easy for coordination while monitoring packages in different hospitals and whether it is

    breaching ethical guidelines or not. It is also recommended that the hospitals or the institutions

    who were providing those packages not only aim for providing effective treatments only but also

    providing them same environment that a common human being enjoys during his/her lifetimewhich will encourage them to think in a positive way that they also can do many things with

    these impairedness. The packages must be helpful in improving the functionality of the patient

    different areas such as in school, other educational institutions, family and social get together

    which will help in mitigating there mental illness and it is one of the effective way of treating

    other than the medications.

    So in overall the packages which were coming after the guidelines set by the WHO in 2001

    about the mental health will help the medical institutions in developing their own packages and

    services.
