Attachment A, Cost Information Form, is hereby deleted and replaced with Attachment A-1, revised Cost Information Form - LOC Number 41539, which is attached to this Clarifications Memorandum.
No vendor questions were received for this LOC prior to the deadline. LOC responses are due Friday, May 29, 2015, at 3:00 p.m. (Central Time).
