The District Office has deter-mined that all students will now be required to provide 3 Proofs of Residence each year.

You will need to provide the fol-lowing:

1) Property tax bill/Lease/Real Estate Closing documents

2) 2 bills dated May 1, 2015 or later. These can be electric, water, sewer, cable, satel-lite, phone, cell phone, ve-hicle tax or voter registra-tion card.

When a student and his/her

parent/legal guardian move in

with a family in an existing

home/apartment, the

parent/legal guardian must pro-

vide proof of acceptable ad-

dress. The homeowner must

provide the 3 proofs of resi-

dence above and an affidavit

must be completed and nota-

rized. Affidavits must be re-

newed yearly.

Field Day is quickly coming upon us. We want this to be one of the best days of the year for our students. We still need volunteers, donated ice-pops & bottle water and to borrow tailgate tents & cornhole games. If you can help in any of those ways, please contact Mrs. Plemmons or complete the online volunteer form on Mrs. Plemmons’ webpage www.lexrich5.org/webpages/cplemmons/ .

• Bring a bagged lunch. If you choose to purchase a lunch at school it will be a bagged lunch with a choice of peanut butter/jelly or corndogs.

• Dress for the weather. Each grade level has an as-signed shirt color. K=Yellow, 1st-Green, 2nd-Blue, 3- Neon, 4th – Red,

5th- Blue • Put sunblock on at home. • Bring sunglasses, sun visor or hat to protect face

from the sun. • Water bottle with student’s and teacher’s name on

it. • Smiles and positive attitude

Room

Thank you for allowing me to help with meeting

your child's health needs this year

at school. The end of the year is

almost here and we are making preparations

for closing of the health room. If your child

has medication here at school, a parent or

guardian will need to pick up the medication

before the end of the school year. For safety

reasons, we do not send medication home with students

and medications cannot be stored over the summer. Any

medications remaining will have to be disposed of accord-

ing to DHEC regulations. Please stop by the health room to

pick up your child's medication before the last day of

school. If you need to make special arrangements, please

call Nurse Harrington at 476-4609.

Parents, if your student will need prescription medicine to

be given at school or if they have a chronic medical condi-

tion , an Individual Health Plan and/or medication form for

the 2015-2016 school year will be mailed home. These will

be mailed to you at the beginning of summer so that you

can have them reviewed and signed by your medical doctor

prior to school starting in August. This is extremely im-

portant! It helps us start the year off being able to fully

take care of your child's medical needs at school. This is

also helpful to your doctor as they get an overload of medi-

cation permission forms and IHP approval forms to sign at

the beginning of the school year!

LMES 4th Grade Students Competed in District

5's Farm to Five "Chopped" Competition - Farm to Five hosted the Healthy Lifestyles/Chopped

Challenge, based on Food Network’s “Chopped,” in

the Spring Hill High School cafeteria Tuesday, May

19. Farm to Five is Lexington-Richland 5’s adaptation

of the USDA’s farm to school program. Small gardens

are being planted at the schools and students are being

exposed to healthy eating choices. Our 4th grade team

of Katharyn J., Matthew B., Carola R-C., and Alex D.

rose to the occasion, placing 2nd in the challenge.
