    Ellipsis Math Class Sponsored by Audubon PTSA

    While most students will benefit from this program, students who like math and commit to positive thinking and participation will benefit the most. Each afterschool class is fun and enriching yet it requires hard work and commitment. It will cover a range of basic math topics from basic integers operations to fractions and decimal number concepts. It will also introduce students to other math topics appeared in the past competition tests, which include algebraic reasoning and patterning, logical thinking, geometry and probabilities. This class will strengthen students basic math skills as well as problem solving skills as they are preparing to compete at various competition in spring 2014. Students will be exposed to multi-steps problem solving strategies as well as thinking outside the box. Many topics discussed will be above their current grade level. Students must be self-motivated and ready to learn and participate. There will be weekly assignments, and students are responsible to complete them on time. Assignment may take about one hour per week. All competitions are optional, but encouraged. Possible competitions: Fifth Grade; MRMI (Feb), Math Kangaroo (March 20), Math is Cool (March 7), Math Olympiad (May 3).

    Important notes: Interested students must take a placement test scheduled on September 24 (Tuesday), at 3:10 p.m. The test will be

    about 1 hour. Location is the school library. At the same time, we will have a short parent meeting right outside the

    library. This is a good time for parents to ask any question about the program.

    To be eligible for the spring session, student must either complete the fall session or pass our spring placement test.

    1. Checks should be made out to Audubon PTSA. Or you may pay by credit card through PayPal at http://www.audubonpta.org/Pages/ExtendedClasses.aspx.

    2. Please complete and return this form, along with the signed Chaperone Policy and Agreement to schoolATTN: Audubon PTSA Extended Classes.

    3. You must sign up to chaperone on at least one day. Dates will be assigned on a first-come-first-serve basis. If your preferred dates are already assigned, we will contact you with the available dates. NEW PLEASE INDICATE BELOW IF YOU ARE WILLING TO CHAPERONE ALL THE CLASSES IN THIS SESSION. IF SO, YOUR CHILD MAY BE ELIGIBLE FOR A FEE WAIVER OF $100!

    4. You may be required or volunteer to sign up to chaperone on more than one day. If any day remains without a parent chaperone, there will be no class that day. Hence we encourage you to sign up for more than one day if your schedule permits. Thank you!

    5. Registration will be on a first come, first serve basis, contingent on placement test result. Please be advised that an application

    alone does not guarantee you a spot in our class. A placement will be finalized when placement test result is available. You will

    be getting a placement confirmation via e-mail about one week before the session starts. Payment is due as soon as you secure

    a placement (payable to Audubon PTSA)

    6. Refund policy: No refund if cancelled after the second week of the session.

    Audubon Elementary Extended Classes Chaperone Policy and Agreement

    All extended classes are PTSA sponsored events and are covered by the PTSAs General Liability Insurance Policy. For

    the protection of our students and to limit the risk of a claim, one adult PTSA member is required to chaperone every

    extended class. Therefore, every child who is enrolled in an extended class will be required to provide an adult

    volunteer to chaperone one or more days of the extended class. A chaperone is defined as an adult LWSD-approved

    PTSA member. Days will be assigned prior to the start of each class.

    Requirements for chaperones:

    1. Chaperones must be approved by the Lake Washington school district. Approval generally takes 2 weeks so

    please ensure that you submit your application early. Application forms can be obtained from the front office or

    2. Chaperones must be adult PTSA members. A link to the PTSA membership form can be found at

    3. Parents, grandparents, aunts, uncles, nannies and neighbors are all acceptable chaperones provided that they

    meet the requirements listed in points 1 and 2 above.

    4. On their assigned day(s) chaperones are required to be physically present in the extended class for the entire

    duration of the class. The role of the chaperone is that of a witness so the chaperones attention needs to be on

    what is happening in the class. The presence of siblings who are not participants in the extended class will be

    allowed only at the discretion of the class instructor.

    If a chaperone is unable to make their assigned day, it is the responsibility of the chaperone to provide a replacement

    LWSD-approved, PTSA chaperone to cover for them. Any replacement chaperone will have to meet the requirements

    set forth above. There will be a sign in sheet at each class in order for the PTSA chaperone to document their presence.
