MAMIE AGNES JONES ELEMENTARY Leadership Academy SCHOOL WIDE DISCIPLINE PLAN In an effort to establish specific rules for behavior and expectations for our learners at Mamie Agnes Jones Elementary School, the following School-wide Plan will be implemented. The plan will provide a visual reminder to learners for the rewards and consequences for appropriate and inappropriate behaviors. Mamie Agnes Jones School Rules As learners of Mamie Agnes Jones Elementary School, we will: 1. Respect the rights and property of others 2. Keep our hands and feet to ourselves. 3. Obey all teachers and staff. 4. Raise our hands for permission to speak or to get out of our seats. 5. Practice safety at all times. Guidelines for Success To help students develop the attitudes, traits, and behaviors necessary for success in school and in life, the following Guidelines for Success will be implemented. As successful students at Mamie Agnes Jones: 1. We are prepared to succeed with all we need. (prepared) 2. We use kind words, voices and actions. (respectful) 3. We do the right thing even when no one is looking. (integrity) 4. We are in the right place, at the right time, doing the right thing. (dependable) 5. We always do our best. (Excellence) School-Wide Attention Signal When students hear, “Give me five!” and see a hand raised, they will do the following: 1. Eyes on the speaker 2. Quiet 3. Be still 4. Hands free (Put things down.) 5. Listen closely

    Behavior Chart A behavior chart will be posted in each classroom. Each learner will have a pocket or clip on the

    chart. Each color on the chart indicates to the student how he/she is doing and whether or not an

    award or consequence will be necessary as a result of choices the student has made. This system will

    be implemented in all classrooms, including resource classes and lunch. A note will be placed on the

    classroom clipboard to let the classroom teacher know if there are students who should receive

    rewards or consequences for their choices during special classes.

    *May result in a referral to the Principal or Principal’s Designee in the main office.

    Incentives Periodically, we will celebrate students who are at the pink level on the Behavior Chart. For

    example, we may announce that “Any student on “pink” will receive a prize.” When the

    surprise announcement is made designated students will come to the office to receive a treat.

    Learners who earn an ‘A’ in citizenship on their Report Card will receive a ribbon at the end of

    each quarter. Learners that receive an “A” each quarter will receive recognition at the end

    of the year awards ceremony.

    LEADER OF THE MONTH.... Each month, a student of the month from each class will be

    recognized. These students exhibit the qualities of successful learners in keeping with the 7

    Habits and/or Guidelines for Success. These special students will be recognized on the morning

    announcements and their pictures will be placed on the bulletin board in the hallway near the

    cafeteria.

    Consequences/Interventions Interventions/disciplinary actions will concur with the Mamie Agnes Jones Elementary School

    Discipline Plan and the Duval County Student Code of Conduct.

    PROJECT S.T.A.R. (Scholarly, Thoughtful, Avid, Reader) Students at MAJ will seize every opportunity to read and learn. In the morning while waiting for school to start,

    students will participate in Project STAR. Sit along the wall with your legs crossed.

    1. Remain quiet.

    2. Read, Read, Read!

    3. Obey all patrols & adults.

    Cafeteria Standards: 1. Enter and leave QUIETLY and RESPECTFULLY.

    2. Keep Hands, FEET, and FOOD to yourself. Always obey the adult in charge.

    3. Speak softly at all times.

    4. Remain seated and RAISE your hand for help.

    5. Clean up after yourself.

    Consequences/Interventions: Verbal Waning, change student seating at table, assign to a designated time-out area, and use

    clipboard. Learners committing a serious violation will be sent to the office.

    Restroom Standards: 1. GO

    2. FLUSH

    3. WASH

    4. LEAVE

    Travel (Hall Walking) Standards: 1. Listen or watch for the teacher’s attention signal.

    2. Wait for directions from the teacher.

    3. Stand directly behind the person in front of you.

    4. Walk Indian style (hands to self, mouths closed, stay in place, face forward).

    5. Enter the new destination calmly.

    Dismissal Standards: 1. Listen to announcements.

    2. Wait for the teacher’s directions.

    3. Leave the room calmly.

    4. Walk Leader style (hands to self, mouths closed, stay in place, face forward)

    5. Enter the destination calmly.

    6. Approach vehicles with care. (bus, car, van)

    Playground Standards:

    Walk to the playground.

    1. Keep your hands, feet and objects to yourself.

    2. Play only on the equipment here at the school.

    3. Contact sports cannot be played at school.

    4. All students are welcome to play.

    5. Line up QUIETLY when you are called the FIRST time.

    DISCIPLINE REFERRAL Teachers should teach students the School Wide Discipline Plan during the first weeks of school and

    revisit throughout the year as necessary. Components of the plan should be posted in a visible

    location. Learners and parents should be notified in writing of classroom expectations. Teachers

    should execute the discipline plan in a consistent and fair manner. When students know the

    expectations, they will comply and excel!

    Procedure for Discipline Referrals:

    1. Use the Discipline Referral Form to refer learners to an administrator. You must send the

    completed form with the student.

    2. After action is taken, a copy of the Discipline Referral Form with the action taken will be

    placed in your box. An additional copy will be sent home with the student.

    Please remember that a referral to an administrator’s office should be the last resort utilized. The

    Duval County Code of Conduct is an excellent reference to use when deciding which offenses

    should be referred to an administrator. For example, failure to bring materials and failure to complete

    assignments are not causes for disciplinary referrals (Class I offenses). All teachers are to familiarize

    themselves and their learners with the Duval County Code of Student Conduct. Conduct Assemblies

    will be held at least twice per year to support the discipline plan.

    can hear)

    Level 2 = Conversation between 2 or 3 friends

    Level 3 = Outside Voice
