MAKE YOUR MOVE:Motivate. Organize. Visualize. Empower. Motivate. Organize. Visualize. Empower are the tenets that this year’s program planning team and presenters have used to create a unique event designed to provide you with the opportunity to create an agenda for practice and professional development. You’ll be challenged to prepare yourself for practice change through a World Café kick-off session where you will explore the most significant opportunities for advancing practice and how to build influence with high-impact actions to do so. You’ll craft a plan for learning throughout the event, through interactive keynote sessions, clinical pearls, roundtables, and pharmacy management/practice and clinical topic programming in addition to the popular MTM Symposium.

Keynote Speakers

Keynote Session–Preparing Ourselves for Practice ChangeIn this engaging World Café session, you’ll explore the most significant opportunities to create practice change for Minnesota pharmacy. You will be challenged to think about the areas within your practice that are ripe for development and craft an action agenda to create change. You’ll identify the knowledge and skills you need in order to move your change agenda and positively influence your action agenda.

Kristin Janke, PhD, Professor, Department of Pharmaceutical Care & Health Systems, University of Minnesota College of PharmacyDr. Janke is an advocate for learning with teaching and scholarly interests in Continuing Professional Development (CPD). From 2009 to 2016, she taught Minnesota’s fourth year CPD Portfolio course, which was a required, year-long course completed concurrently with Advanced Pharmacy Practice Experiences (APPEs) and designed to develop life-long learning skills. Dr. Janke is a professor in the Department of Pharmaceutical Care & Health Systems at the University of Minnesota College of Pharmacy and currently serves as the Director of the Wulling Center for Innovation & Scholarship in Pharmacy Education.

Steve Simenson, BPharm, FAPhA, FACA, DPNAP, CEO/Managing Partner, Goodrich Pharmacy, Inc.Steven Simenson graduated from the University of Minnesota College of Pharmacy in 1977 with a B.S. degree in Pharmacy. He is a member of the Minnesota Pharmacists Association, National Community Pharmacists Association, American College of Apothecaries and is a past president of American Pharmacists Association. Steve is on the Board of the American Pharmacists Foundation and the Center for Pharmacy Practice Accreditation. Simenson’s practice includes Medication Therapy Management, Collaborative Practice and Compounding. He precepts community pharmacy residents, leadership students and APPE and IPPE student pharmacists. Simenson is CEO and Managing Partner of Goodrich Pharmacy Inc., with community pharmacies in Anoka, Andover, Blaine, Elk River and St. Francis, Minnesota. He a fellow of APhA and ACA, and a Distinguished Practitioner of the National Academy of Practice.

Pain—Addiction Medicine

Attendees will understand that chronic pain and substance use disorder are chronic brain diseases. Additonally what does the evidence say about best practics for withdrawal syndrome including signs of severe disease.

Breakout 2: Pharmacy Practice/Management Innovative Best Practice Across an Integrated Care System

In this engaging session, you will hear from pharmacists Kati and Joe Dvorak about innovative best practices that have been implemented at FirstLight Health System. First Light Health System is a rural health system consisting of a critical access hospital (+ emergency room), three family practice clinics, an eye clinic, a community pharmacy and urgency services. Pharmacy services have been integrated throughout these settings, allowing pharmacists to play an essential role in patient care. You’ll hear about the pharmacists role on the care team, strategies to improve care transitions, their 340B program, and innovations in their inpatient and outpatient pharmacies, including new ways they are engaging pharmacy technicians.

The session will identify potential implications of biosimilars for pharmacists, including issues of product substitution and interchangeability.

Nate Hunkins is the Manager of Business and Systems Development from Minnesota Community Measures, a private partnership of health plans, employer groups and health care providers. Regardless of your pharmacy practice site, knowledge of these measures can facilitate partnerships and collaborations. You will leave with an action plan.

Breakout 3: Keys to Effective Medication Management During Transitions of Care

Transitions of care provide pharmacists the opportunity to improve health outcomes as patients move between health care settings. This transitional period has the potential to cause gaps in patient care. Coordination between pharmacies and health care facilities can provide a continuity of safe and quality care during these transitions. This session describes the role of Thrifty White Pharmacy in Duluth during transitions of care as patients discharge from the hospital and move into a long term care facility. Improving communication between the local hospitals and Thrifty White has allowed potential problems to be solved sooner and medications to arrive at the facility earlier in the day. This demonstrates the need for a continuity of care across all health care transitions and these concepts can be applied in several types of pharmacy settings.

Dr. Affeldt joined Fairview Pharmacy Services in September 2013.

Fairview Pharmacy Services is an integral part of Fairview Health Services, one of the largest health care systems in Minnesota and a partner of the University of Minnesota Medical Center and University of Minnesota Children’s Hospital. Fairview serves patients from across the state of Minnesota, the upper Midwest and beyond.

Prior to joining Fairview, Dr. Affeldt spent 19 years at the Walgreen Co. He was a District Pharmacy Supervisor for most of the last 10 years at Walgreens — where he led the implementation of Specialty programs, MTM program, and Diabetes programs at Walgreens in Minnesota.

Dr. Affeldt is a graduate of the University of Minnesota College of Pharmacy. He is a former Pharmacy Alumni Society board member and remains active at the College of Pharmacyasamentortofirst-yearstudents, and through involvement in the Leading Change in Pharmacy program; he also holds an Adjunct Assistant Professorship at the University of Minnesota College of Pharmacy in the Department of Pharmaceutical Care and Health Systems. Aytay serves as co-chair for the MPhA Public Committee and as a member of the Pharmacy Practice Act Joint Task Force. She is on the MPhA Board of Directors and Executive Finance Committee. Amanda Brummel, PharmD, BCACP, Director, Clinical Ambulatory Pharmacy, Fairview Pharmacy Services

Dr. Brummel has been employed by Fairview Pharmacy Services since 1999 when she graduated from the University of Minnesota. While at Fairview, she has built and practiced Medication Therapy Management (MTM) in multiple clinic locations, was the clinical supervisor for the MTM department, the MTM Operations & Program Manager. Currently Dr. Brummel has responsibility for the MTM program, the clinical development and integration of pharmacy services in the Fairview Health Network including the transitions of care approach and retail clinical services. She works closely with the Fairview Medical Group and the Fairview Network in its population health approach and new payer product development. Dr. Brummel is also an Adjunct Associate Professor at the University of Minnesota. She has published multiple articles on MTM and pharmacy’s role in the care team. She has chaired and served on multiple committees and is a current member of the Minnesota Pharmacists Association, the American Society of Health-System Pharmacists, the American College of Clinical Pharmacy and Pharmacy Quality Alliance (PQA).

Dr. Bruzek earned his Bachelor of Science and Doctor of Pharmacy degrees from the University of Minnesota College of Pharmacy. His pharmacy experience includes pharmacy practice in the United States Public Health Service – Indian

Health, hospital clinical services and directing the Minnesota State Medicaid pharmacy program. Dr. Bruzek began his involvement in managed care pharmacy at BlueCross BlueShield of Minnesota and was one of the founders of Pharmacy Gold, Inc., the predecessor of Prime Therapeutics. At HealthPartners, Dr. Bruzek has overall responsibility for pharmacy programs whichincludespharmacybenefitsfora health plan of 850,000 members as well as operation of clinic pharmacies, centralized prescription fillingfacility,mailorderfacilityandclinical pharmacy services. He is also an Adjunct Assistant Professor in Pharmaceutical Care and Health at the University of Minnesota College of Pharmacy.

Heidi Burres, PharmD BCACP, MTM Pharmacist, Fairview Pharmacy Ser-vices

Heidi Burres graduated from the Uni-versity of Minnesota College of Phar-macy with her PharmD in 2010. After graduation, Dr. Burres completed a residency through the University of Minnesota Ambulatory Care Residen-cy Program with Fairview and earned hercertificationasaboardcertifiedambulatory care pharmacist. She has been practicing MTM with Fairview since the completion of her residency in 2011 and currently works at the Edina and Fridley clinics. Dr. Burres is also a preceptor for University of Minnesota pharmacy students and a residency preceptor for the University of Minnesota Ambulatory Care Resi-dency Program.

Amy Busker graduated from the University of Minnesota College of Pharmacy in 1999 with her Doctor of Pharmacy degree. She works with Fairview Partners and Geri-atric Services, where she provides Medication Therapy Management to patients in the community and Assist-ed Living setting. Joe Dvorak, PharmD, Community Pharmacy Manager, FirstLight Health System

Dr. Joseph C. Dvorak graduated with his Doctor of Pharmacy Degree from the University of Minnesota College of Pharmacy in 2008, after earning his Bachelor’s Degree in Biomedical En-gineering. Before joining the FirstLight team as manager of the Community Pharmacy in Pine City, he gained valuable experience in a variety of community settings. Dr. Kati J. Dvorak, PharmD, grad-uated with her Doctor of Pharmacy Degree from the University of Minne-sota College of Pharmacy-Duluth in 2008. She completed an Ambulatory Care Residency with the University of Minnesota at Goodrich Pharmacy in 2009, specializing in Medication Ther-apy Management (MTM). At FirstLight Kati plays an essential role in expand-ing the MTM practice, precepting residents and working as part of the multidisciplinary team to create indi-vidual patient care plans. She cares for patients both in the hospital and in the community providing MTM. She is passionateaboutrefiningthetransi-tion of care process and severs on committees to improve continuity of care for our patients. Molly Ekstrand, RPh, BCACP, AE-C, Medication Management Program Leader, Park Nicollet Health Services

Molly Ekstrand is a pharmacist and leader of the Medication Manage-ment program in the Park Nicollet Health System in the Minneapolis/St Paul Metro Area. She leads efforts to develop and implement the MTM program and infrastructure. In just 4 years, Park Nicollet has expanded to ten pharmacists (9.6 FTE) serving 16 primary care clinics reaching over 800 patients each month. Ekstrand practiced MTM in Family and Internal Medicine clinics for over 10 years; she has provided and documented

MTM services to thousands of differ-ent patients.

Krista Gens, PharmD, BCPS-AQID, Infectious Diseases Clinical Pharmacy Coordinator, Abbott Northwestern Hospital

Krista earned her Doctor of Pharma-cy degree from North Dakota State University in 2008 and completed her pharmacy practice (PGY1) and infectious diseases (PGY2) residen-cies at Abbott Northwestern Hospi-tal (ANW) in Minneapolis. Krista is board-certifiedinpharmacotherapywithaddedqualificationsininfectiousdiseases. As the Infectious Diseases Clinical Pharmacy Specialist at North Memorial Medical Center (NMMC) for 5 years, she led the development and implementation of the antimicrobial stewardship program. As an Infec-tious Diseases Clinical Coordinator at ANW, Krista helps maintain antimicro-bial stewardship programs at Abbott Northwestern and at several Allina regional hospitals. Krista is actively involved in several multidisciplinary committees including Pharmacy and Therapeutics (P&T) committee, Infection Prevention and Control, and the C diff Task Force. Krista serves as a preceptor for the PGY1 and PGY2 infectious diseases rotations and preceptor for the University of Minne-sota College of Pharmacy. Krista is an active member of the Minnesota Society of Health-System Pharmacists (MSHP), American College of Clinical Pharmacy (ACCP), Minnesota College of Clinical Pharmacy (MCCP), and So-ciety of Infectious Diseases Pharma-cists (SIDP). She also graduated from ASHP’s Pharmacy Leadership Acad-emy in June, 2015. Krista’s areas of interest include antimicrobial stew-ardship, antimicrobial resistance, and precepting residents and students. Nathan Hunkins, MPH, Manager, Business and Systems Development, Minnesota Community Measurement

Nathan leads initiatives that aim to produce long-term value for Minnesota Community Measurement. After receiving his Master’s degree in public health from the University of Minnesota Nathan has been workinginthefieldofhealthcarequality measurement for over 7 years. His work includes developing and maintaining strategic partnerships with local and national stakeholders, pursuing new funding opportunities, and developing ways to enhance existing MNCM services in the areas of health care measure development and testing, data collection, analytics and report writing. Other focus areas for Nathan include exploring ways to reduce the administrative burden related to data collection for medical groups who participate in various local and federal quality measurement programs. Overall, Nathan is a conceptual thinker who enjoys working on big ideas that seek to improve health and health care. Daniel Jude serves as the manager of specialty clinical services at Fairview Specialty Services Pharmacy, a large regional specialty pharmacy provider in Minneapolis. His team is multi-dis-ciplinary, including both registered nurses and pharmacists. He is a fac-ulty member of Midwest AIDS Training and Education Center and a member of Minnesota Program HH’s formulary committee. He is an accredited phar-macist with the American Academy ofHIVMedicineandalsoaCertifiedSpecialty Pharmacist. He received his PharmD from the University of Iowa’s College of Pharmacy in 2008. Dr. Jude has served on the board of directors for Minnesota AIDS Project for the past 3 years.

Matt Just, PharmD, CGP, Thrifty White Pharmacy

Matt Just graduated from the Univer-sity of Minnesota College of Pharmacy in May 2012 with his PharmD. He started as a part-time staff pharmacist and part-time consulting pharmacist with Thrifty White Pharmacy right after professional school and after 4 months of being a pharmacist, he took over as PIC at the LTC pharmacy in Duluth. Six months later, he transi-tioned into a full-time consulting role

for Thrifty White Pharmacy. He ob-tainedhisCertifiedGeriatricPharma-cist licensure in December 2014 and he is currently a Senior Care Phar-macist in the Duluth and Iron Range area. He also serves as a community member of the Stratis Health initiative on improving Transitions of Care.

Lara Kerwin, PharmD, Pharmaceutical Care Leadership Resident, University of Minnesota College of Pharmacy

Lara Kerwin received her PharmD degree from the University of Missouri - Kansas City (UMKC) School of Pharmacy in 2015. During pharmacy school, she developed her interest in professional advocacy, patient-centered care, and collaboration with faculty, students, and the community as part of APhA, Phi Lambda Sigma, and the Missouri Pharmacy Association.

Lara is currently in year two of the Pharmaceutical Care Leadership Residency at the University of Minnesota College of Pharmacy. Part of her second year involves building an ambulatory care pharmacy practice at a North Memorial primary care clinic as she has a passion for inter-professional collaboration and building long-term relationships with patients. At the College, she teaches in the Pharmaceutical Care Learning Center and enjoys working with students as a New Practitioner Mentor. After completing her residency, Lara envisions a career in academia in which she is able to teach and precept student pharmacists while innovating pharmacist-provided services in primary care. She plans to continue active involvement in local, state, and national organizations and pursue scholarly activities.

Outside of pharmacy, Lara enjoys listening to music and concert-going, playing trivia, exploring local sights, cheering on her Kansas City Royals, and spending time with friends and family.

Heidi Le, PharmD, Leadership Resident, University of Minnesota College of Pharmacy

Heidi Le earned her Doctor of Pharmacy degree at the University of Minnesota in May 2015. She received her Bachelor of Science in Chemistry and Bachelor of Arts in Biology from Augsburg College in Minneapolis in 2010. Upon graduating from Augsburg, she received the Fulbright Scholarship, where she spent a year teaching English Speaking in Vietnam. Dr. Le is currently in year two of the Pharmaceutical Care Leadership Residency, where she spends half her time providing ambulatory care services at HealthPartners Neurology Clinic and the other half teaching at the University of Minnesota College of Pharmacy. She also works part time at M Health in infusion pharmacy, where she worked as an intern during pharmacy school. She has a passion for working inter-professionally and caring for the underserved. In her spare time, Dr. Le enjoys traveling, running, exploring new restaurants, and spending time with family and friends.

Michelle Nemec, PharmD, Thrifty White Pharmacy

Michelle Nemec is a long-term care pharmacist at Thrifty White in Duluth. She graduated from the University of Minnesota Duluth in 2009 with a Bachelor’s degree in cell and mo-lecular biology. She received her doctoral degree in pharmacy in 2013, also from the University of Minneso-ta Duluth. Dr. Nemec has worked in Thrifty White long-term care and retail settings since 2008. She participates in Stratis Health, an organization that promotes collaboration between local health care facilities. She also men-tors pharmacy students.

Raquel Rodriguez, PhD, Director, Experiential Education, University of Minnesota College of Pharmacy

Dr. Raquel Rodriguez has been working with the University of MN, College of Pharmacy since 1995 as the Director of Experiential Education Program . She is also an Assistant Professor at the PCHS Department and teaches at the SAPh graduate program, earned her BS Pharmacy degree in Puerto Rico, and practiced for four years as a community pharmacist in a geriatric setting. She came to Minnesota to obtain her MS in Social & Administrative Pharmacy (SAPh) in 1984. She returned to the U of M in 1985 to successfully complete her PhD in SAPh in 1989. Dr. Rodriguez received various awards such as Faculty of the Year Award from students as well as Faculty of the Year from the Pharmacy Alumni Society (PAS).

In 2011 Dr. Rodriguez completed her training through the Center for SpiritualityandreceivedaCertificatein Integrative Therapies & Healing Practices: Health Coaching from the

University of Minnesota. This Center was established in 1997 and Dr. Rodriguez has an associate Faculty Appointment with them. The new skill set that this training provides will be put to good use with some changing responsibilities within the Professional Education Division. Now she has a greater role in working with students on professional and career advancement. She will remain connected with students and preceptors, but her role will expand to include career counseling to students as well as assisting students with their academic performance.

Stephen W. Schondelmeyer, PharmD Ph.D, Professor and Head, Department of Pharmaceutical Care and Health Systems, University of Minnesota

Stephen Schondelmeyer is a profes-sor of Pharmaceutical Economics in the College of Pharmacy at the Uni-versity of Minnesota, where he holds the Century Mortar Club Endowed Chair in Pharmaceutical Management and Economics. Dr. Schondelmeyer is the Director of the PRIME Institute, which focuses on economic and policy analysis related to the role of pharmaceuticals and pharmacists in society. A leading expert on pharma-ceutical economics and public policy, Dr. Schondelmeyer has spent his 40-year career working extensively on competition and pricing of drug products, affordability and access to drug therapy and pharmacists service, coverage and reimbursement policy under public and private third party programs, and pharmacy work-force trends and policy. His work has shaped both policy and actions relat-ed to the role of pharmaceuticals and pharmacists’ services in today’s so-ciety. He has served as an advisor to the U.S. Congress, to many state leg-

islatures, and to foreign governments. He is often called upon by the public media as an expert on the dynamic and economic aspects of the pharma-ceutical market. Dr. Schondelmeyer holds a BS in Pharmacy from the University of Missouri; a PharmD and clinical pharmacy residency from the University of Kentucky; and a Master’s in Public Administration and a PhD in Pharmaceutical Administration from Ohio State University.

Anita Sharma, PharmD, BCACP, started the pharmaceutical care practice at HealthEast in 2013 during her second year of residency. She’s passionate about patient care, educating future pharmacists, and advancing our profession. She’s a member of the ACCP Clinical Practice Affairs committee and served on the ASHP Ambulatory Section Advisory Group. She authored two articles in JAPhA last year on motivational interviewing and Model for Improvement. She is also an adjunct faculty member at the University of Minnesota and is the co-director of an elective course, entitled Pharmaceutical Care Experience, which gives students

Steven Simenson graduated from the University of Minnesota College of Pharmacy in 1977 with a B.S. degree in Pharmacy. He is a member of the Minnesota Pharmacists Association, National Community Pharmacists Association, American College of Apothecaries and is a past president of American Pharmacists Associ-ation. Steve is on the Board of the American Pharmacists Foundation and the Center for Pharmacy Practice Accreditation. Simenson’s practice includes Medication Therapy Man-agement, Collaborative Practice and Compounding. He precepts commu-nity pharmacy residents, leadership students and APPE and IPPE student pharmacists. Simenson is CEO and Managing Partner of Goodrich Phar-macy Inc., with community pharma-cies in Anoka, Andover, Blaine, Elk River and St. Francis, Minnesota. He is a an APHA, ACA fellow and a Dis-tinguished Practitioner of the National Academy of Practice.

Todd Sorenson, PharmD, Professor, University of Minnesota College of Pharmacy

Dr. Sorensen has been with the Uni-versity of Minnesota since 1998. Prior to joining the College, he held a joint appointment

with the Queen Elizabeth II Health Sciences Center and Dalhousie University in Halifax, Nova Scotia and also served as a Clinical Manager for PCS Health Systems. He is a native Minnesotan, hailing from the small rural community of Tyler, MN.

Dr. Sorensen currently serves as the Peters Endowed Chair in Pharmacy Practice Innovation. He serves as Associate Department Head in PCHS, focusing on supporting practice-based faculty and practice-related activities of the department.

Jill Strykowski is Director of Pharmacy at Mercy Hospital and Unity Hospital, part of Allina Health. She earned her BPharmDegree from the University of Wisconsin and her Master’s of Science in hospital pharmacy (exper-imental pharmacotherapy) from the University of Minnesota, where her thesis focused on Anticoagulation Clinic implementation and outcomes. She has lectured and is a published author on anti-thrombotic topics. Her recent interests include emergency department pharmacist practice, pharmacy leadership, global health, geriatrics and changing models of care for pharmacy practice. She has served on the MPhA Board of Di-rectors, and was MPhA President in

2013-2014. In addition, she is a mem-ber of the ASHP, MSHP and Midwest College of Clinical Pharmacy and is a clinical assistant professor at the Uni-versity of Minnesota. Ms. Strykowski serves as a Leadership Network Part-ner and is a mentor to staff, students and residents. Jill is very interested in pharmacy practice in the developing world and precepts students in Africa as well as volunteers as a preceptor locally at the Phillips Neighborhood Clinic.

Dr. Brent Thompson, PharmD, Phar-macy Director, FirstLight Health System

Brent Thompson, PharmD, has been Pharmacy Director at FirstLight Health System in Mora, Minnesota since 2002, starting in 2000 as the only Clinical Pharmacist. In his time with FirstLight, the practice has grown to 8pharmacistsand8certifiedtechni-cians practicing in acute, ambulatory, and community pharmacy, caring for patients in a small rural hospi-tal, 3 rural clinics and a community pharmacy. He has also operated an independent consulting practice for various long-term/intermediate care and assisted living facilities since 1999.

future. He introduces high school students to pharmacy through a Careers Exploration class, maintains a full schedule of interns, and a residency. As a community leader, Dr. Thompson is a director on the Pine County Fair Board, bringing thousands of kids to the Pine County Fair each year. His passion, second only to pharmacy, is deer hunting, and he serves as Habitat Director for the Jim Jordan Chapter of the Minnesota Deer Hunters Association. This organization works to educate area youth through hunter education camps in Northern Minnesota.

Most pertinent to today’s topic, Dr. Thompson has served on the Substance Abuse Coalition of Kanabec County, since 2012 and finishedhistermaschairin2015.The mission of the Substance Abuse Coalition of Kanabec County is to promote healthy choices in the community through prevention and collaborative partnerships for the reduction of alcohol, tobacco and other drug abuse among youth and residents.

Dr. Umbreit is a 2008 graduate from the University of MN College of Pharmacy . She completed a PGY1 residency with a focus in ambulatory care at Essentia Health in Duluth, MN andhasaboardcertificationinambu-latory care. In 2016 she received the academic appointment of Instructor at the Mayo Clinic School of Medicine. She continues to enjoy precepting student pharmacists, pharmacist resi-dents and medical residents.

Geoff Wall, PharmD, FCCP, BCR, COP, Professor of Pharmacy Practice, Drake University

Dr. Geoffrey C. Wall is a Professor, Department of Clinical Sciences, College of Pharmacy at Drake Univer-sity and Director of the Drake Drug Information Center. His clinical prac-tices include the Internal Medicine and Medical Intensive Care Teaching Services at Iowa Methodist Medical Center in Des Moines, IA. Dr. Wall received his Bachelor of Science in Pharmacy from the University of Utah in 1992 and his Doctor of Pharmacy from Idaho State University in 1998. He completed an ASHP-accredited Internal Medicine Specialty Residency at Scott and White Memorial Hospitals and Clinics in 1999. He is Board-Cer-tifiedinPharmacotherapyandisaCertifiedGeriatricPharmacist.Heis a Fellow of the American College of Clinical Pharmacy. Dr. Wall has written a number of peer-reviewed papers and textbook chapters on a variety of topics, and has designed or participated in several clinical trials. His research interests include drug treatment of gastrointestinal disorders, clinical evaluation of drug allergy, pulmonary pharmacotherapy, and rheumatologic disorders.

Jordan Wolf, PharmD, CGP, LTC Consultant Pharmacist, Thrifty White Pharmacy

Jordan Wolf is a Long Term Care Consultant Pharmacist for Thrifty White Pharmacy, where he has been employed since 2012. He graduated from the North Dakota State University College of Pharmacy, Nursing, and Allied Sciences in 2012, with a Bach-elor’s of Pharmaceutical Sciences de-gree, Doctor of Pharmacy degree and minors in Chemistry and Spanish. He successfullyobtainedhisCertificationin Geriatric Pharmacy in November 2014. From August 2013 through Sep-tember 2014, Dr. Wolf represented both Thrifty White and the Minnesota Pharmacists Association (MPhA) on the Minnesota Department of Health’s Immunization Pharmacy Advisory Group. He subsequently served on the Minnesota Department of Health’s Pandemic Preparedness Workgroup. He currently serves as Vice President of the Minnesota Chapter of the Amer-ican Society of Consultant Pharma-cists, and is an active member of the American Pharmacists Association, Minnesota Pharmacists Association, North Dakota Pharmacists Associ-ation, and the American Society of Consultant Pharmacists. Through his current role as a consultant pharma-cist, Dr. Wolf has extensive expe-rience in medication management in a variety of settings, including: Community, Skilled Nursing Facili-ties, Assisted Living Facilities, Group Homes, Correctional Facilities, etc. He isacertifiedtrainerfortheAmericanPharmacists Association’s MTM Cer-tificationCourse,andhastaughtthiscourse on behalf of MPhA on multiple occasions.
