PAYMENT & PAYMENT SCHEDULING OPTIONSYou may pay in full or make a $50 deposit (per camper, per session) whenyou register, and schedule the remaining payments. The $50 deposit isnon-refundable and non-transferable.

Scheduled payments must be paid in full by June 1 for sessions that beginon or before July 11; or by July 1 for sessions that begin after July 11.(June 1 is the last date personal checks will be accepted.) You may payyour balance before the due date; when paying on line or using theautomated touch-tone telephone system, your payment will be shown asa credit to your account. On the due date, the system will automaticallyapply that credit to the balance.

WAYS TO REGISTER...To register via Internet or telephone, have your SMARTlink barcodes andfamily PIN, course number, and MasterCard/VISA credit/debit card number.

Register on the web – visit www.pgparks.com and click on theSMARTlink logo before midnight June 12.

Register via SMARTlink automated, touch-tone telephone – call 301-583-BOOK (2665) before midnight June 12.

Register in person at any M-NCPPC staffed facility in PrinceGeorge’s County. All walk-in registrations during the week prior to

camp start date must be processed at the offering facility, and are subjectto supervisor approval.

EXTENDED CARE INFORMATION• You must first be registered in the corresponding day camp before registering for extended care. (Not all camps offer extended care.)

• Please complete extended care registration at least one week prior to camp start date.

• You must pay in full at the time you register for extended care. • Extended care registration is NOT available via SMARTlink telephone.

CANCELLATION POLICYRegistrants are entitled to a 100% refund (or SMARTlink credit) in theevent that M-NCPPC cancels a camp.

CHECK POLICYChecks returned for insufficient funds will be assessed a $20 fee by M-NCPPC, in addition to your bank charges. NO starter checks accepted.

FEE ASSISTANCE PROGRAMFee assistance is available (subject to approval) for Prince George’s Countyresidents only. For complete information and application forms, stop byyour local community center or obtain the forms on line atwww.pgparks.com>About Parks and Recreation>Fee Assistance. Applica-tions must be processed no later than 3 weeks prior to start of program.

LATE PAYMENT PENALTIESIf you’ve made a deposit and full payment is not received by the due date,your space may be forfeited and you will lose your $50 deposit. If thecamp accepts your late payment, you will be assessed an additional $20late payment fee.

WAIT LISTSIf a camp session is full, you may put your name on a wait list and registerfor another camp during that session. If a spot opens at your first-choicecamp, upon your request your registration will be transferred to the first-choice camp without penalty.

WITHDRAWAL & REFUND POLICYWithdrawal and refund requests for fully paid or partially paid registrations are subject to a $50 non-transferable, non-refundablewithdrawal fee per child/per session. Withdrawal and refund requestsfrom extended care (with continued enrollment in associated camp) willbe assessed a 20% fee. NO EXCEPTIONS. Withdrawals from sessions must be processed by the facility offering the program.

WHAT (R), (NR) AND (IA) MEAN(R) means Residents of Prince George’s and Montgomery Counties; (NR) means Non-Residents of all other jurisdictions; and (IA) meansresidents living in the Impact Area of the Prince George’s Sports & Learning Complex. EXTENDED CARE OPTIONS• Supervised, non-structured playtime before and after camp session hours are available for many camp sessions. Extended care sessions are listed with each camp session in this brochure. Not all camps offer extended care. Please check activity descriptions.

• All disability accommodation requests (inclusion services) for extended care must be indicated on the Participant Profile Form (p. 57) and requested at least two weeks prior to the start date of the camp.

LATE PICKUP FEE POLICYA late fee of $5 per camper for every 15 minutes late (or portion thereof)will be assessed for campers who are not picked up by the program’sscheduled closing time. Payment is due when you pick up your camper.We recognize that emergencies arise and request that parents call thecampsite if they are delayed for any reason; however, the late fee still willbe assessed.

REQUIRED FORMSA complete list and explanations of forms is available at ww.pgparks.com,and in the Parent Handbooks (available in English & Spanish), or obtainedfrom staff. All forms and handbooks can be downloaded fromwww.pgparks.com. NOTE: Maryland state law requires that each childhave a complete set of health forms at each program. Completed healthforms must be turned in prior to the first day of the program session. It isthe parent/guardian’s responsibility to deliver all necessary, completedhealth forms to each program location. Children may not attend anysession without completed health forms on site. NO EXCEPTIONS. • Participants must provide date of last tetanus or DPT vaccination. • All participants must complete and return a Participant Profile Form (p. 57) and Health Form (p. 58).

• All participants who take prescription or over-the-counter medication or use medical devices (i.e. inhalers, EpiPens®) must complete and return a Day Camp Medication Form. (A physician’s signature may be required.)

• All participants must complete and return a Day Camp Authorization Form.• All forms are due by June 1.

SUMMER CAMPINFORMATION AND POLICIES

Swimming Trips Policy –SWIMMER DESIGNATIONSPlease read the following information carefully prior to selectingyour child’s swimming designation on the Health Form.

To help ensure the safety of your child while he or she is participat-ing in swimming pool trips this summer, you will be asked todesignate him or her as a “Non-Swimmer” or “Swimmer” on theHealth Form. Use the following descriptions as a guide for selectingyour child’s appropriate swimming designation.

NON-SWIMMER:Non-swimmers have limited or no previous experience withswimming pools or other aquatic environments. They are unable toindependently swim distances of 20 yards or more and/or cannotswim or play comfortably in water that is above their heads.

If you designate your child as a “Non-Swimmer” on the HealthForm, he or she will be restricted to chest-deep water on swim trips.

SWIMMER:Swimmers have previous experience with swimming pools or otheraquatic environments. They can swim a minimum of 20-yards andcan swim or play comfortably in water that is above their heads.

If you designate your child as a “Swimmer” on the Health Form, heor she will be required to pass a swim test each time he or she visitsa pool. The swim test requires participants to enter and to exit thepool unassisted, swim a minimum of 20 yards in water more thanfive feet deep and continuously tread water for a minimum of 20seconds.

Upon successful completion of the required swim test, children designated as “Swimmers” will have full access to both shallow anddeep-water areas of the pool during swimming trips. Designated“Swimmers” who do not pass the required swim test will be re-designated as “Non-Swimmers” on their Health Form andrestricted to chest-deep water.

If your child’s swimming abilitieschange between the time you register for a program and the start of the program, pleasecontact staff, you will need to fill out and sign a new Health Form.

POLÍTICA DE CANCELACIÓNEn caso de que M-NCPPC cancele un programa, el registrado recibiráun reembolso completo, es decir de 100% (o crédito SMARTlink)

INFORMACIÓN GENERAL• Registración empieza el jueves, 1 de abril a las 9 am. • Los patios de recreo y sitios de centros para jóvenes estarán cerrados el lunes, 4 de julio

• Cada participante debe proveer su propio transporte (M-NCPPC no provee transporte)

• Cada participante debe traer su propio almuerzo. En sitios sin refrigeración, deben traer alimentos no perecederos.

FORMULARIOS DE SALUDLa ley del Estado de Maryland requiere que cada participante tengaformularios de salud completos en cada sitio donde este registrado.Estos deben entregarse antes del primer día del programa. • El formulario titulado “Formulario de Dispensa de Aparatos Médicos Patios de Recreo/Centros para Jóvenes” debe estar completo en caso que el participante esté tomando medicina en casa (una firma del médico puede ser requerida). ¡OJO!: Los únicos medicamentos (dispositivos médicos) que se permiten en los patios de recreo y centros para jóvenes son los inhaladores y los lápices anafilácticos.

POLÍTICA DE CHEQUE SIN FONDOS El M-NCPPC cobrará $20 por cheque que rebote por falta de fondos,este cobro es por separado de lo que le cobre el banco. No se aceptancheques que no hayan sido imprimidos con su nombre y dirección.

POLÍTICA DE REEMBOLSO Y CANCELACIÓN• El pago de $40 por patios de recreo y centros para jóvenes durante el verano no es reembolsable ni transferible. NO HAY EXCEPCIONES.

INFORMACIÓN DE SMARTLINK Estableciendo una cuenta de SMARTlink

Para servirle mejor, M-NCPPC en el Condado de PrinceGeorge’s, utiliza un programa llamado SMARTlink. Paraestablecer su cuenta, preséntese en cualquier centrocomunitario del Departamento de Parques y Recreación

M-NCPPC con documentación que compruebe la residencia (porejemplo: licencia de conducir o tarjeta de identidad MVA, cuenta deagua, teléfono, luz; contrato de alquiler, certificado de propiedad), edade identidad (por ejemplo: licencia de conducir u tarjeta de identidadMVA, pasaporte, visa de residente, certificado de bautismo y certificadode nacimiento con sello autorizado, carta del colegio u otraidentificación emitida por el gobierno) de cada miembro de su familiaque quiera incluir en su cuenta SMARTlink. SUMMERXTREME TEENCENTERS

Summer Xtreme Teen Centers offer

Prince George’s County teens, ages

13-17, age-appropriate activities on a

drop-in basis. Centers operate

Mondays-Fridays from June 27 until

August 5. The fee is $40 per teen,

whether a teen attends six hours a

day for all six weeks, or select days

and times only. Optional field trips

and special events may be offered for

additional fees. Teens must be the

minimum age of 13 on or before

December 31, 2011. All sites are

closed July 4.
