Introduction
This progress diary is to help you keep anongoing record of your condition and toallow your doctor and/or nurse to monitoryour health between appointments.
The diary enables you to record anychanges – positive or negative – as andwhen they occur. It is important thatyou fill in these pages on the actualday – it can be very difficult toremember exactly how you werefeeling a few days earlier. Space isalso provided for you to write down anyconcerns or questions to make sure you donot forget to mention them at your nextappointment.

Keeping a DiaryEvery time you see your doctor ornurse, he or she will ask how youhave been feeling since your lastappointment. This information isimportant to help guide yourtreatment over the coming weeksand months.The following table will help youkeep a record of your condition. Use it to write how you are feelingand what activities you can andcannot do.For example, you could record, ‘Pain in hands disturbed sleep’, or ‘Went swimming today, no pain.’
