Let us help you find the right Cigna provider for your needs.

Logon to www.healthscopebenefits.com.

Click on “Member”. Type in “OC” under Company Name, click “Enter”.

Click on “Provider Network Listings”.

Click on the Cigna Network.

Under “Find a ...”, choose either “Doctor”, “Dentist”,

or “Hospital, Pharmacy or Facility”.

Under “Select A Plan”, Click the “Pick” box.

A list of Medical Plans will appear. Click “PPO, Choice Fund

PPO” and then click “Choose”.

The screen should read: “Selected Plan: Medical & PPO Choice

Fund PPO”, where it previously said “No Plan Selected”.

Enter your City or Zip Code under “Search Location”.

Enter your Doctor’s Name under “Looking For” or

leave blank for all providers in your area.

Click the “Search” button for results.
