• Form 157A• $535 (from 1 st January 2012)• Confirmation of enrolment (COE) or Letter of Offer (must have CRICOS course code)• Overseas Student Health Cover (OSHC)• Medical Examination and Chest X-ray. Your application will be assessed FASTER if you

have already undergone medicals when you lodge the application. (Medical and X-rayresults are usually only valid for 12 months).
