Develop ADRC Programs in 2 Counties (One Urban and One Rural Model) That Will Be Visible and Trusted by Both Older Adults and Adults with Disabilities

Empower Older Adults, Adults with Disabilities, Caregivers and Other Consumers to Make Informed Choices about Their Care and Identifed Needs (Consumer Directed) NC care Link (up an running Sept 2007) –website

of a uniform screening tool/potential to share client data among data contributors using NC care Link thru client tracking software (under development )

Sustainability- Moving toward Rebalancing LTC -
