GENERAL NOTES
1. ALL CONSTRUCTION INDICATED ON THESE PLANS SHALL BE IN ACCORDANCE WITH:
• THE 2010 EDITION OF AND SUPPLEMENTS TO THE RHODE ISLAND DEPARTMENT OF TRANSPORTATIONSTANDARD SPECIFICATIONS FOR ROAD AND BRIDGE CONSTRUCTION (RI STANDARD SPECIFICATIONS).

NO EXTRAPAYMENT WILL BE MADE FOR COMPLYING WITH THESE REQUIREMENTS.
7. BOTH FEDERAL AND STATE LAW (RI GENERAL LAW 39-1.2) REQUIRE NOTIFICATION OF APPROPRIATEUTILITY COMPANIES BEFORE DIGGING, TRENCHING, BLASTING, DEMOLISHING, BORING, BACK FILLING,GRADING, LANDSCAPING, OR OTHER EARTH MOVING OPERATIONS. IT IS THE CONTRACTOR'SRESPONSIBILITY TO NOTIFY ALL UTILITY COMPANIES (INCLUDING THROUGH THE “DIG SAFE” PROGRAM) TOENSURE THAT ALL UTILITIES, BOTH UNDERGROUND AND OVERHEAD, HAVE BEEN MARKED BEFORECOMMENCEMENT OF SUCH WORK. THE CONTRACTOR SHOULD UNDERSTAND THAT NOT ALL UTILITIESSUBSCRIBE TO THE “DIG SAFE” PROGRAM. 8. EXISTING AND PROPOSED DRAINAGE AND UTILITIES SHOWN HEREIN ARE FOR GENERAL INFORMATIONONLY. IT IS THE CONTRACTOR'S RESPONSIBILITY TO AVOID ANY DAMAGE TO UTILITIES, DRAINAGE ANDOTHER STRUCTURES.
9. EXISTING DETAILS, DIMENSIONS AND ELEVATIONS SHOWN ON THESE PLANS ARE NOT GUARANTEED. THECONTRACTOR SHALL BE RESPONSIBLE FOR VERIFYING THE EXISTING CONDITIONS AS PART OF THE SHOPDRAWING SUBMITTAL PRIOR TO CONSTRUCTION. ANY DISCREPANCIES REQUIRING MODIFICATIONS TOTHESE PLANS SHALL BE BROUGHT TO THE ATTENTION OF THE ENGINEER AS PART OF THE SHOP DRAWINGSUBMISSION. 10. CONTRACTOR SHALL BE AWARE OF THE PRESENCE OF TRAFFIC CAMERAS IN THE VICINITY OF SOME OFTHE BRIDGES BEING REPAIRED AND SHALL TAKE NECESSARY PRECAUTIONS TO AVOID DISTURBANCE ORDAMAGE TO THESE FACILITIES.
11. TEMPORARY EARTH SUPPORT SHALL BE INSTALLED AS SHOWN ON THE PLANS AND OTHER AREAS ASREQUIRED TO SUIT CONTRACTORS OPERATIONS. TEMPORARY EARTH SUPPORT SHALL BE DESIGNED BYTHE CONTRACTOR AND SHALL BE REMOVED UPON COMPLETION OF WORK. NO EXTRA PAYMENT WILL BEMADE FOR EARTH SUPPORT REQUIRED, BUT NOT SHOWN ON THE PLANS.
12. PLANS FOR INDIVIDUAL BRIDGES SHOW A "SUMMARY OF WORK REQUIRED". THIS SUMMARY SHOULD NOTBE CONSIDERED ALL INCLUSIVE BUT RATHER JUST A LIST OF THE MAJOR ITEMS OF WORK.
13. PAVEMENT THICKNESS ON BRIDGE DECKS SHALL BE VERIFIED PRIOR TO SAWCUTTING TO AVOIDSAWCUTTING CONCRETE DECK TO REMAIN. VERIFICATION OF PAVEMENT THICKNESS AND PAVEMENTSAWCUTTING SHALL BE DONE IN THE PRESENCE OF THE ENGINEER.
14. PAVEMENT THICKNESS ON AND OFF BRIDGES ARE SHOWN AND ARE BASED ON EXISTING PLANS.CONTRACTOR SHALL ASSUME, FOR BIDDING PURPOSES, THAT THE ACTUAL PAVEMENT THICKNESS IN THEFIELD COULD BE 2 TIMES THAT SHOWN ON THE PLANS.
15. COST OF CONCRETE SAWCUTS AROUND PERIMETER OF REPAIR AREAS SHALL BE CONSIDEREDINCIDENTAL TO THE REPAIR.
16. ALL STAGING AND STOCKPILE AREAS SHALL BE LIMITED TO AREAS DESIGNATED FOR EACH BRIDGE. AREASOUTSIDE OF THESE DESIGNATED AREAS SHALL REQUIRE APPROVAL OF THE ENGINEER.
17. ALL STAGING AND STOCKPILE AREAS MUST BE LOCATED IN AN UPLAND LOCATION. STOCKPILES OFERODABLE MATERIALS SHALL HAVE SIDE SLOPES OF 2H:1V OR FLATTER AND BE SURROUNDED BYCOMPOST FILTER SOCK (CFS).
18. DEMOLITION OF PORTIONS OF BRIDGES AS DESIGNATED SHALL BE DONE SO CAREFULLY IN ACCORDANCEWITH SECTION 803 OF THE RI STANDARD SPECIFICATIONS SO AS NOT TO DAMAGE PORTIONS NOTDESIGNATED FOR REMOVAL. ANY DAMAGE TO PORTIONS NOT DESIGNATED FOR REMOVAL SHALL BEREPAIRED BY THE CONTRACTOR AT NO ADDITIONAL COST. ALL DEMOLITION DEBRIS SHALL BE LEGALLYDISPOSED OF.
19. SHIELDING SHALL BE INSTALLED AT LOCATIONS SHOWN ON THESE PLANS AND OTHER AREAS ASREQUIRED TO PROTECT WATERWAYS, ROADWAYS, WALKWAYS, AND UTILITIES, IN ACCORDANCE WITH803.0500 OF THE COMPILATION OF APPROVED SPECIFICATIONS. NO DEMOLITION SHALL COMMENCE UNTILREQUIRED SHIELDING IS INSTALLED TO THE SATISFACTION OF THE ENGINEER.

BRIDGE GENERAL NOTES - 1
1. DESIGN SPECIFICATIONS
• THE AASHTO LRFD BRIDGE DESIGN SPECIFICATIONS, LATEST EDITION, INCLUDING ALL INTERIMREVISIONS TO DATE.
• THE RHODE ISLAND LRFD BRIDGE DESIGN MANUAL 2007 EDITION INCLUDING ALL REVISIONS TO DATE.
• ALL OTHER APPLICABLE DESIGN SPECIFICATIONS ARE REFERENCED IN SECTION 1 OF THE RHODEISLAND LRFD BRIDGE DESIGN MANUAL DATED 2007.
• THE 2010 REVISION OF AND SUPPLEMENTS TO THE RHODE ISLAND DEPARTMENT OF TRANSPORTATIONSTANDARD SPECIFICATIONS FOR ROAD AND BRIDGE CONSTRUCTION (RI STANDARD SPECIFICATIONS).

DESIGN DATA
GENERAL NOTES
20. ALL EXCAVATED AREAS AND STOCKPILE AREAS SHALL BE SURROUNDED BY COMPOST FILTER SOCK INACCORDANCE WITH SECTION 206 OF THE COMPILATIONS. THESE EROSION CONTROLS SHALL BEMAINTAINED IN ACCORDANCE WITH SPECIAL PROVISION CODE 212.1000.
21. CONTRACTOR SHALL TAKE ALL NECESSARY PRECAUTIONS RELATED TO TEMPERATURE SENSITIVE WORK.NO EXTRA PAYMENT OR TIME EXTENSION WILL BE GRANTED FOR ADHERENCE TO THIS REQUIREMENT.
22. HIGH EARLY STRENGTH CONCRETE MAY BE UTILIZED AT NO ADDITIONAL COST TO MEET THECONTRACTORS SCHEDULE. A MIX DESIGN SHALL BE SUBMITTED FOR REVIEW.
23. FORMWORK AND FALSEWORK SHALL BE DESIGNED TO SUPPORT ALL IMPOSED LOADS RESULTING FROMCONTRACTORS OPERATIONS.
24. ALL MATERIALS SHALL BE INSTALLED IN CONFORMANCE WITH THE MANUFACTURERSRECOMMENDATIONS. ALL WORK WHICH IMPACTS MATERIALS (IE. BACKFILLING, LOADING, ETC.) SHALL BEAPPROVED BY THE MANUFACTURER.
25. THE CONTRACTOR SHALL ENSURE THAT NO MATERIAL, DEBRIS, OR EQUIPMENT INADVERTANTLY FALLSINTO ADJACENT ROADWAYS, RAILROADS, OR WATERWAYS ADJACENT TO THE WORK AREA. SHOULD ANYMATERIAL, DEBRIS, OR EQUIPMENT INADVERTANTLY FALL INTO THESE AREAS, IT SHALL BE PROMPTLYREMOVED AT THE SOLE EXPENSE OF THE CONTRACTOR.
26. ANY DRILLING INTO EXISTING CONCRETE SHALL UTILIZE A PACHOMETER OR OTHER SUITABLE DEVICE TOAVOID EXISTING REINFORCING.
27. NO TRAFFIC SHALL BE ALLOWED OVER AREAS BEING REPAIRED UNTIL WORK IS COMPLETE, AND ONLYAFTER APPROVAL BY THE ENGINEER. CONCRETE SHALL ACHIEVE 28 DAY STRENGTH PRIOR TO BEINGEXPOSED TO TRAFFIC.
28. CARE MUST BE TAKEN TO MINIMIZE DISTURBANCE TO EXISTING SITE CONDITIONS. ALL EQUIPMENT,MATERIALS, AND DEBRIS SHALL BE REMOVED UPON COMPLETION OF WORK, AND THE AREA RESTOREDTO IT'S ORIGINAL CONDITION TO THE APPROVAL OF THE ENGINEER.
29. ANY CLEARING OF VEGETATION REQUIRED TO PERFORM WORK SHALL BE MINIMIZED AND SHALL NOTEXCEED 5 FEET FROM THE FACE OF STRUCTURE.
30. ALL DEBRIS WITHIN BRIDGE ROADWAYS SHALL BE REMOVED AND DISPOSED.
31. ALL WORK SHALL CONFORM TO MAINTENANCE OF TRAFFIC PLANS.
32. CONTRACTOR SHALL PROVIDE, AS REQUIRED, GRAVEL BORROW FOR BACKFILL AT VARIOUS LOCATIONSTHROUGHOUT BRIDGES INVOLVING EARTHWORK ABOVE AND BEYOND THAT SHOWN ON THE DETAILS INTHE AMOUNT OF 10 CY PER BRIDGE AT NO ADDITIONAL COST.
33. ALL WATERPROOFING ON BRIDGES SHALL BE PREFABRICATED WATERPROOFING SHEET MEMBRANE ANDSHALL CONFORM TO THE LATEST COMPILATION SECTION 813.
34. FOR BRIDGE NO. 850 AND 987, CONTRACTOR SHALL PROVIDE, AS DIRECTED BY THE ENGINEER, AN EXTRA1000 POUNDS OF REINFORCING AND 50 DRILL AND GROUT DOWELS FOR EACH BRIDGE AT NO ADDITIONALCOST. BARS SHALL BE OF VARIOUS SIZES, LENGTHS AND BENDS.
35. DECK OVER BACKWALL WORK ON ANY ONE BRIDGE SHALL COMMENCE AND BE COMPLETED WITHIN ONECONSTRUCTION SEASON.
36. THERE SHALL BE NO DISTURBANCE OR CLEARING OF VEGETATION OUTSIDE LIMITS SHOWN, NO CHANGESIN GRADE AND NO TEMPORARY OR PERMANENT FILLING.
37. HANDLING AND DISPOSAL OF ASBESTOS CEMENT PIPE SHALL CONFORM TO ALL APPLICABLE STATE ANDFEDERAL REGULATIONS.
38. PAVEMENT SHALL BE INSTALLED IN LIFTS NOT LESS THAN 112" NOR MORE THAN 2", EXCEPT CLASS 19

BRIDGE SHOP DRAWINGS AND SUBMITTALS
THE FOLLOWING LIST OF BRIDGE ITEMS OF WORK FOR WHICH SHOP DRAWINGS AND/OR OTHER SUBMITTALSARE REQUIRED IS PROVIDED FOR THE CONVENIENCE OF THE CONTRACTOR. THIS LIST INCLUDES ONLY THEMAJOR ITEMS OF BRIDGE WORK; IT DOES NOT ITEMIZE ALL SUBMITTALS REQUIRED BY THE CONTRACTDOCUMENTS. STRUCTURAL STEEL NOTES
1. FRAMING DIMENSIONS ARE GIVEN ALONG CENTERLINES OF GIRDERS AND ALONG CENTERLINES OFBEARINGS ON ABUTMENTS AND PIERS.
2. STRUCTURAL STEEL SHAPES AND PLATES SHALL CONFORM TO THE LATEST PROVISIONS OF AASHTODESIGNATION M 270 GRADE 50.
3. THE SHOPS FABRICATING THE STRUCTURAL STEEL SHALL, AT A MINIMUM, BE CERTIFIED FOR “SIMPLESTEEL BRIDGE STRUCTURES (SBR)”.
4. STEEL SHALL MEET THE ZONE 2 LONGITUDINAL CHARPY V-NOTCH TEST OF 15 FT.-LBS. AT 40°F ASSPECIFIED IN TABLE S1 "TOUGHNESS REQUIREMENTS" AS PER AASHTO DESIGNATION M 223 (ASTMDESIGNATION A 572) OF THE AASHTO STANDARD SPECIFICATIONS FOR TRANSPORTATION MATERIALS ANDMETHODS OF SAMPLING AND TESTING. SAMPLING AND TESTING PROCEDURES SHALL BE IN ACCORDANCEWITH AASHTO T 243 (ASTM DESIGNATION A 673). THE (H) FREQUENCY OF HEAT TESTING SHALL BE USED.
5. SHOP DRAWINGS FOR ALL FABRICATED STEEL SHALL BE SUBMITTED TO THE ENGINEER IN SUFFICIENTTIME TO PERMIT CAREFUL CHECKING PRIOR TO FABRICATION.
6. WELDING SHALL BE IN ACCORDANCE WITH THE LATEST STRUCTURAL WELDING CODE AASHTO/AWS D1.5(INCLUDING ALL INTERIMS TO DATE) AND APPLICABLE SUPPLEMENTAL AWS PUBLICATIONS.
7. ALL SHOP CONNECTIONS AND SPLICES SHALL BE WELDED. WELDING PROCEDURES AND TECHNIQUES TOBE USED IN FABRICATION AND ERECTION OF THE STEEL MEMBERS SHALL BE AS SHOWN ON THE SHOPDRAWINGS.
8. NO FILLET WELD SHALL BE LESS THAN 14".
9. WELDING ELECTRODES SHALL HAVE THE SAME CORROSION RESISTANCE AS THE BASE METAL AND SHALLBE FREE OF MOISTURE AT THE TIME OF USE.
10. INSPECTION OF WELDS INCLUDING RADIOGRAPHIC TESTING (RT) AND MAGNETIC PARTICLE TESTING (MT)SHALL BE IN ACCORDANCE WITH THE RI STANDARD SPECIFICATIONS AND THE AASHTO/AWS BRIDGEWELDING CODE, EXCEPT THAT THE REMAINING PERCENTAGE OF ALL GROOVE WELDS NOT RT TESTEDSHALL BE MT OR DYE-PENETRANT TESTED.
11. ALL BOLTS SHALL CONFORM TO AASHTO DESIGNATION M 164, AND SHALL BE GALVANIZED IN ACCORDANCEWITH AASHTO M232 AND SHALL BE INSTALLED IN ACCORDANCE WITH SECTION 824 OF THE RI STANDARDSPECIFICATIONS. ALL BOLTS SHALL BE PRIMED AND PAINTED AFTER INSTALLATION.
12. BOLTED CONNECTIONS SHALL SATISFY CLASS B SURFACE CONDITION AS DEFINED IN THE AASHTO LRFDBRIDGE DESIGN SPECIFICATIONS.
13. WASHERS MEETING AASHTO DESIGNATION M 293 ARE TO BE USED OVER ALL HOLES THAT ARE MORE THAN1/16” IN DIAMETER GREATER THAN THE BOLT DIAMETER AND UNDER ALL PARTS TURNED DURINGASSEMBLY.
14. STRUCTURAL STEEL SHALL BE PREPARED AND PAINTED IN ACCORDANCE WITH THE PLANS AND SPECIALPROVISION CODE 825.99.
15. SEALANT/CAULK SPECIFIED AT STEEL REPAIR LOCATIONS SHALL CONFORM TO PAINT MANUFACTURERSRECOMMENDATIONS AND SHALL BE APPLIED AFTER AND MATCH COLOR OF TOP COAT OF PAINT.
16. UNLESS NOT ACCESSIBLE OR OTHERWISE SPECIFIED, THE UPPER SURFACES OF THE END DIAPHRAGMSAND TOP FLANGES AT THE ABUTMENTS SHALL BE FREE OF PAINT, OIL OR OTHER IMPURITIES THAT WOULDIN ANY WAY REDUCE THE BOND OF CONCRETE TO STEEL.
17. PRIOR TO FABRICATION, ALL MATERIALS SHALL BE BLAST-CLEANED TO AT LEAST SSPC-SP6 TO REMOVEALL OIL, DIRT, GREASE, MILL SCALE AND OTHER DELETERIOUS MATERIALS FROM THE SURFACES OF THESTEEL TO BE FABRICATED.
18. ALL NEW STEEL SHALL BE CLEANED AND PRIMED ON ALL SURFACES IN THE SHOP AFTER FABRICATION. ALL EXISTING STEEL SURFACES DESIGNATED TO BE PAINTED SHALL BE CLEANED AND PRIMED PRIOR TOINSTALLING NEW STEEL. CONTACT SURFACE BETWEEN NEW AND EXISTING STEEL SHALL BE CLASS B.AREAS TO BE FIELD WELDED SHALL BE MASKED AND PRIMED IN THE FIELD AFTER WELDING.
19. PRIOR TO SHOP COATING, ALL CORNERS AND EDGES OF STEEL WHICH HAVE BEEN FLAME CUT OROTHERWISE HARDENED SHALL BE SOFTENED BY GRINDING OR BLAST-CLEANING TO PROVIDE A SURFACESUITABLE FOR APPLICATION OF THE SPECIFIED PAINT SYSTEM.
20. INTERMEDIATE AND TOP COATS SHALL BE APPLIED IN THE SHOP OR FIELD FOR NEW STEEL AND IN THEFIELD FOR EXISTING STEEL PROVIDED THAT THE CONTACT SURFACE BETWEEN THE EXISTING AND NEWSTEEL IS CLASS B.
21. WHEN STEEL DIE STAMPS ARE USED TO IDENTIFY PIECES AND MEMBERS, FABRICATORS SHALL UTILIZELOW STRESS STAMPS.
22. ALL EXISTING PAINT ON BRIDGES SHALL BE ASSUMED TO BE LEAD BASED PAINT.
CONCRETE NOTES
1. ALL CONCRETE SHALL BE HIGH PERFORMANCE CLASS HP, AS DESCRIBED IN THE RI STANDARDSPECIFICATIONS AND THE SPECIAL PROVISIONS OF THE SPECIFICATIONS.
2. THE CONTRACTOR MAY, AT THE APPROVAL OF THE ENGINEER, PROPOSE THE USE OF SELFCONSOLIDATING CONCRETE FOR ANY CLASS OF CONCRETE ON THIS PROJECT. SECTION 606 “SELFCONSOLIDATING CONCRETE (SCC)”, CONTAINS THE REQUIREMENTS FOR MODIFYING ALL CLASSES OFCONCRETE MIX DESIGN FOR SELF-CONSOLIDATING APPLICATIONS.
3. ALL PORTLAND CEMENT CONCRETE SHALL BE AIR-ENTRAINED.
4. ALL REINFORCING STEEL SHALL BE GALVANIZED. ALL WIRE TIES AND MISCELLANEOUS HARDWARE USEDFOR PLACEMENT OF REINFORCING SHALL ALSO BE GALVANIZED.

THE ENTIRE TOPSIDE SURFACES OF ABUTMENT AND PIER CAP BEAM SEATS , AS WELL AS VERTICAL FACESOF BACKWALLS, FOR BRIDGE NOS. 722 NB, 766, 840, 853, 856 SOUTH ABUTMENT, AND 978, SHALL BEPROVIDED WITH A FILM-FORMING SEALER (M12.03.1) CONCRETE SURFACE TREATMENT-PROTECTIVECOATING IN ACCORDANCE WITH SECTION 820 OF THE RI STANDARD SPECIFICATIONS. ADDITIONALLY, ALLEXPOSED PORTIONS OF ABUTMENTS AND WALLS FOR BRIDGE NOS. 722 NB AND SB, 766, 840, 853, AND 856SHALL RECEIVE THIS TREATMENT.
12. ALL EXPOSED EDGES AND REENTRANT CORNERS NOT OTHERWISE DETAILED ON THE PLANS SHALL HAVEMINIMUM 3 4" CHAMFER.

ANYCONCRETE STAINS OR DISCOLORATION OCCURRING PRIOR TO ACCEPTANCE OF THE SURFACES SHALL BEREMOVED BY THE CONTRACTOR AT NO ADDITIONAL COST TO THE STATE.
15. UNLESS OTHERWISE NOTED ON THE PLANS JOINT FILLER IS TO BE PREFORMED, NON-EXPANSIVE,NON-EXTRUDING TYPE IN ACCORDANCE WITH SECTION M.02.11.1 OF THE RI STANDARD SPECIFICATIONS.
16. PLACEMENT, FINISHING AND CURING OF BRIDGE DECK CONCRETE SHALL BE IN ACCORDANCE WITHSECTION 814 OF THE RI STANDARD SPECIFICATIONS.
17. ALL DECK FORMS SHALL BE OF THE REMOVABLE TYPE THAT WILL PRODUCE THE DIMENSIONS SHOWN ONTHE PLANS.
18. EMBEDMENT LENGTHS FOR DRILLED AND GROUTED DOWELS SHALL BE IN ACCORDANCE WITH SECTION819 OF THE RI STANDARD SPECIFICATIONS.
19. IN ACCORDANCE WITH THE RI STANDARD SPECIFICATIONS, ALL METAL TIES OR ANCHORAGES WHICH AREREQUIRED FOR CONCRETE FORMWORK SHALL BE SO CONSTRUCTED THAT THEY CAN BE REMOVED TO ATLEAST TWO INCHES BELOW THE EXPOSED SURFACE OF THE CONCRETE WITHOUT CAUSING DAMAGE TOTHE CONCRETE SURFACE. SNAP TIES MAY BE USED ONLY IF APPROVED BY THE ENGINEER. IF THECONTRACTOR PROPOSES TO USE THEM, A CATALOG CUT AND OTHER NECESSARY INFORMATION MUST BESUBMITTED TO THE ENGINEER TO DEMONSTRATE THAT THE TIES WILL SNAP-OFF FAR ENOUGH INTO THECONCRETE TO ALLOW FOR PROPER PATCHING.
