Linking Documents: This is the process by which you add new documents to your survey and link new or existing documents to an element.

Step 1: Click on the document library tab

Step 2:  Click on add document to the library on the lower right corner of the screen

Adding New Documents1. Name document in the “Document Name” box

2. Click  “Browse” to find document in computer system; click to attach

3. Click  “Save”. Number of documents in the Library will increase

4. Click  “Save and Attach More” to continue attaching documents

Step 4:  Once you have attached your document to your survey tool, youcan now link it to an element. Please follow the steps below.

Link Documents to an Element

1. Click on “Documents” button for that element

2. Click on “Link Document”

3. Click “Link Existing” to link a document from the Document Library.

4. Click “Link New” to add a new document from your computer.

This is an alternate path to steps 1-3 to document and it will link the document to the element at the same time.

Step 5:  You can link one document to more than one element. The same document can be used for more than one element.

1. Click on “Document Library” tab. 2. Click on “Link to New Element.

How to Upload DocumentsVerify Document Path:

1. Click “Browse”

2. Highlight document and click on “Open”

 

(Note: Please make sure you are uploading the same exact document, with the same document name and the same file path)

Tips for Trouble shooting ISS 

Make sure that the documents are located on your shared drive (meaning being accessed by multiple users within the organization.

Internet speed (Dail up can be too slow).

No special characters in the file name and folder such as %. //.

Working with internet explorer rather than Firefox.

Making sure it’s trusted sites. 

Avoid uploading documents all at once but rather in batches.

Made recommendation browsing to the documents (when uploading) rather than cut and pasting the file path.

Name documents clearly and avoid duplicate names.

Consider making your final Word or Excel documents “read only” to ens ure no changes will be made.

Don’t scan everything – this creates large files that may be challenging for you to upload and for the surveyors to open.

Uploaded documents (including those in your document library and documents already transferred to NCQA servers) cannot be edited. Prior to submission, if you need to edit a document linked to your document library or uploaded to the NCQA server, you must remove, edit and re-upload it. After submission, you may not edit uploaded documents.

Be certain you’ re set to go before you submit your final Survey Tool. Prior to submitting your Survey Tool, use the “completeness Check” to be sure you’ve filled out all the portions of your Survey Tool.The “Completeness Check” can be found under the “Utilities” Section.
