Welcome from Lincolnshire County CouncilWelcome to the 2012/2013 edition of the Lincolnshire Care Services Directory. I hope you will find it a useful document telling you about the support choices available across the county, either for yourself, a member of your family or a friend.

We know making decisions about the help, support and care can be daunting for you, particularly if you have had no involvement with social care before. This Directory runs alongside the new website www.mychoicemycare.org.uk for the first time; both providing clear, easy to understand information so that you can be confident you are making the right choices and decisions.

The County Council is responsible for the development of a wide range of support. We are passionate about making a real difference to the well-being of everyone who needs this support. In our experience, most people want to remain independent for as long as they can. Wherever possible, we will give you the ability to choose to stay in your own home and maintain your independence if that is what you wish to do. We will work closely with you and your family/carer, as we recognise the value of their understanding on what support will make a difference to you.

One of the main areas we are focusing on for the future of Adult Social Care is ‘Personalisation’; which is where the person is put at the very heart of the process, ensuring that your voice is heard and you have choice and control of how your needs are met. You will find out more regarding your entitlement to a personal budget and how this money can help you make the right choices for your care. More is explained on this later in this Directory.

I am sure you are aware of the different financial pressures which face local authorities, and it remains our highest priority to make sure that the most vulnerable members of our communities continue to receive high-quality and appropriate care for their individual needs. www.MyChoiceMyCare.org.uk is Lincolnshire County Council’s new website which enables you, your friends, carers and family, however distant, to research the options available to you in the place where you live in Lincolnshire.

There is a wide range of information on MyChoiceMyCare which will help you or the person you care for find the support and services you require to help you lead a better quality of life. If you need help and don’t know where to start; you can complete a short assessment and someone will

Many people will use this Care Services Directory looking for advice and information about adult social care. The Directory aims to give an overview of all aspects of adult social care to everyone who is looking for support.

Thinking about the care or support either you or someone else in your life will need can be daunting and making any change in your life will require making decisions. Finding appropriate care or support for yourself or someone else you care for can seem difficult or daunting. This publication has been designed to help you identify relevant care providers in your preferred area of the county and to give you a starting point.

Care Choices’s free advice line runs in association with the charity FirstStop Advice and provides a comprehensive advice and information service encompassing care, accommodation, funding and rights. A personalised report can be generated for

contact you. If you are a carer looking for support, or you have received a personal budget and want some help with the next steps; there are providers that can help. You can consider a range of options that may be able to help and you can find out about providers of different services and contact them directly.

There are a number of services available to support you if you are experiencing poor health. They can prevent you from going to hospital, or offer extra care and support after you have been discharged.

Lincolnshire County Council and other local authorities have made changes to the provision of care and support to older people and people with learning and/or physical disabilities. This is a national requirement that sets out the way people and those who care for them should be supported.

In the traditional system of social care, professionals made many of the decisions about the support people could have and who would

How does it work?If you or someone you know or care for are eligible for social care, Self-Directed Support allows you to have more choice and control over your life as you are able to make decisions about the support you need. It asks how you want to live your life and what support will best enable you to do this. Support can be given if you are a carer to help you continue your caring role and maintain a life outside of caring.

We have now introduced personal budgets across the whole of Lincolnshire for all eligible users of social care services following assessment. They bring flexibility and transparency of funding to the provision of social care services.

A personal budget is an amount of money that

provide it. As a result, people often had limited choice about the services they might receive.

A system called ‘Self - Directed Support’ seeks to replace this old approach of prescribed care, by putting people in control of the support they need to live the life they choose. It recognises that some people can manage their support on their own whilst others need help ‒ from family or friends or people who are paid to help.

If you or the person you care for are eligible got social care support, Self-Directed Support gives you a Personal Budget based on an assessment of your eligible social care needs. You may need support or help to manage everyday tasks such as washing and dressing, help with eating and drinking or getting out into the community. You can be as creative as you want, as long as the money is spent to meet your needs.

We use guidance from the government to decide which people are eligible to receive help funded by us. Everyone who comes to us to request help will have an assessment to see if they meet the eligibility criteria ‒ previously known as Fair Access to Care Services. Sometimes these assessments will be over the phone where people have less significant difficulties and sometimes they are carried out face to face.

We provide services to people with ‘substantial’

and ‘critical’ levels of need. People who do not meet the eligibility criteria will receive information and advice about how their needs can be met through services available to all in the community such as those available through voluntary agencies or private organisations.

• A Managed Budget - you know how much money is available to you and you still have choice and control to choose but Lincolnshire County Council will arrange and pay for your support on your behalf.

• a Direct Payment (DP) ‒ Lincolnshire County Council pays you an amount of money. The money can then be spent on solutions and services of your choice to meet your eligible social care needs, and you are in control of this money;

• If someone is not able to consent to a Direct Payment, but where a Direct Payment is deemed to

Direct Payments are financial payments made to people who have been assessed as eligible to receive support from Lincolnshire County Council. We can make a Direct Payment to most people who need our support, including:

• people who have been assessed as needing community care services (including those with Mental Health needs) aged 16 and over, incorporating short as well as longer term needs;

• carers over 16 for carer’s services; and

• parents of disabled children, for children’s services.

Instead of us arranging or providing services directly, you could use these payments, either by yourself or with assistance, to purchase support that you consider most appropriate to meet your assessed social care needs. Also, if you are looking after someone else as a carer, you could also receive Direct Payments to help you in your caring role or to have some time for yourself. Why would I want Direct Payments?Direct Payments allow more control over the decisions that affect your life. They provide more flexibility and choice, as you can buy appropriate support tailored to your individual needs. Another option is for us to source a portion of the care/support needs through services commissioned directly by Lincolnshire County Council and also provide a Direct Payment to arrange those remaining services that you wish to have more control over. What can Direct Payments be used for?If you receive a Direct Payment, the money is used to arrange support to meet eligible needs. You

may choose to employ someone directly, to buy services from an independent or voluntary sector provider or purchase equipment to help live more independently. With Direct Payments, people or agencies employed are accountable to you and not to Lincolnshire County Council. Therefore, Direct Payments enable more control, choice and flexibility over how you plan your support to fit with your life.

It is important that your plans meet your assessed needs. Therefore, provided the money is used lawfully, and eligible needs are met, the flexibility that these payments allow means it is impracticable to outline what the money can be spent on. However, there are certain items on which these payments cannot be spent, for instance: • on services that should be provided by Health; and• on routine living expenditures: utility and household bills for instance.

Most Direct Payments are made to meet regular ongoing support needs, however they can also be made as a single payment, for example, to purchase equipment or a short respite break. What do I do next?If you are interested in Direct Payments and are currently receiving social care support from Lincolnshire County Council, please contact your local social care team for more information.If you don’t already have services provided by us there is a lot of support available to advise and assist you with Direct Payments. What care is available?This section is about the guidance, support and care that we can provide. It describes all parts of the social care process, from someone who might be leaving hospital, worried about how they are going to cope at home, to advice about specialist care for someone who might have very complex needs

but wants to stay at home. Some of these sections may not be relevant to you, others may be of more interest. Feel free to go straight to the section that interests you, or browse through the whole section to give yourself a full flavour of the support available.

Help after leaving hospital

Reablement

Health & Adult Services and the NHS in Lincolnshire work closely together to ensure that if you are a hospital patient, you are discharged safely and are able to return to home life as quickly as possible.

Our staff are available to help with information and advice when you are ready to leave hospital.

No matter what hospital you are in, you can request an assessment of your needs to help you stay independent and confident when you return home. A social care worker will come and speak to you to arrange an assessment which can either be done whilst you are still in hospital, or once you return home.

The assessment involves asking you about your needs and what is required to help you live safely and independently at home. There will also be an

What is reablement and how can it help you?Reablement is about giving you the opportunity and confidence to re learn and regain some of the skills you may have lost because of poor health, disability, following a spell in hospital or problems at home. After events like this you can lose your confidence or worry about your ability to cope at home again. Reablement is a very personalised approach ‒ the kind of support given is tailored to your individual needs. It focuses as much on your emotional and social needs as on your medical needs.

Reablement provides support for a short time to help you become as independent as possible. You may need support with aspects of day to day living such as:

• Personal care• Preparing meals• Shopping• Teaching and practicing exercises

explanation of any charges that may be made.A relative, carer or a member of the hospital staff

may also approach the hospital social care team and ask them to carry out an assessment, but this will not go ahead unless you agree.

If you already have a social care service in place we will make an assessment of any other needs you may have after your stay in hospital.

If you are a carer, you may also need help and support and should also be offered an assessment. We can also introduce you to local voluntary organisations who can help, with a variety of services such as laundry and domestic cleaning.

How does the reablement service work?Following a referral to the service, a member of the reablement team will visit you to discuss your needs. A member of the team will be your ‘Key Worker’ who will be your contact throughout your reablement journey.

Together we will agree what goals we are working to achieve. Your goals are written down in a reablement care plan. Reablement support will take place in your own home.

Your key worker will arrange for your care to be delivered by the reablement support workers. They will review your progress towards your goals, oversee your care and agree the right time for discharge from the service.

What happens when your reablement ends?At the end of reablement your key worker will assess your ongoing needs. If you still need continued support we will discuss this with you and if appropriate, refer you for an Adult Social Care Assessment.

If you have been through a period of reablement, and require ongoing support, we will work with you to assess your needs, Even if you haven’t been through the reablement process we can provide you with an assessment.

An assessment is simply a discussion with you that will help us understand what help and support you might need coping with life on a day to day basis and living independently in your own home.

Where does it take place?It usually involves a chat over the phone, or in your own home and we’re very happy if you have a friend, relative or carer with you. If you are in hospital, we’ll come and talk to you before you are discharged to see about any help you might need in the home whilst you are recovering. The assessment is a two way conversation. We’ll ask you to tell us something about yourself and you can tell us how you think we can help and support you. We might also ask the opinion of other professionals who know and work with you, like your doctor, but we will only do this with your permission. We will also take account of the needs of your family or carer and assess them separately if they ask us to.

Remember we are only coming to see you to understand how you are coping day to day and how we might help to improve your quality of life.

How quickly will this happen?If your situation is very urgent and you are in a crisis or at serious risk of harm, we will aim to see

you within 24 hours. If your carer is finding it very difficult to cope, we will try to see you within seven days. In any other circumstances it will be within 28 days of you asking for the assessment.

What about confidentiality?We respect your privacy at all times. Any information you give us will be treated as confidential and will only be shared with anybody else involved in your care with your permission - other than in exceptional circumstances where the law says we must show it to someone else.

Will I be able to get help?After the assessment we will identify and agree your needs with you and discuss the help and support available to meet them, according to the seriousness of your situation. There may be a charge for services; the amount will depend on your capital and income. We will always give you full information before a service is provided.

If you are eligible for help from Adult Social Care we will aim to arrange this within seven days of the assessment being carried out. If you are not eligible for help, we will still give you information and advice on other services that are available to help you to continue to live independently and to maintain your quality of life.

If you are not happy with the outcome of the assessment you can ask us to look at your situation again with you and if you are still not happy you can get in touch with us to make a comment, suggestion or complaint.

First Contact

If you are aged 60 and over you can now get in touch with a range of essential services through one point of contact to help you to stay safe and well in your own home. From January to April 2013, this service will be piloted so all working age adults can access this service.

The First Contact service, which is run in partnership by a range of organisations across Lincolnshire, works by encouraging people to complete a checklist in a First Contact leaflet.

The checklist addresses issues such as fire and community safety, personal care, housing, heating, falls prevention, money and benefit entitlements, transport, social isolation, home care and caring for others.

Organisations that provide help through First Contact include:

• Adult Social Care• Lincolnshire Fire and Rescue• District Council housing teams• Age UK• Lincolnshire Police• The Pension Service• Volunteer centres.

First Contact leaflets are widely available in the community from places such as libraries, local Community Access Points and GP surgeries. Alternatively, to request a leaflet or to find out more about the First Contact service you can visit the website at www.firstcontact.org.uk or phone First Contact on 01522 782172.

Living at homeways in which you can help yourself if you are living at home. Be it accessing the support you need directly yourself on www.mychoicemycare.org.uk or through the First Contact checklist and contacts below.

There are many different forms of support that can enable you to remain in your home for as long as possible ranging from equipment to people who can come in to support you in your own home. Starting with First Contact below, there are many

1. Do you have a working smoke alarm on each floor of your home and know what to do in case of fire?

2. Do you have any repairs that need doing to your home or any other housing needs not mentioned on the form?

3. Do you ever find it difficult getting around the house, getting in and out of bed, washing yourself or getting up and down stairs?

4. Are you able to keep your home warm?

5. Are you worried about having a fall and/or have you fallen recently?

6. Would you like advice on crime reduction and personal/home safety?

7. Would you like advice on money and benefits that you may be entitled to?

8. Would you like to receive information about local transport in your area?

9. Would you like information on becoming a volunteer and being more involved in your local community?

10. Would you like advice on domestic help available such as cleaning, gardening, shopping and prescription collection?

11. If someone looks after you in an unpaid capacity or you look after another person you are entitled to a Carers Assessment. Are you interested in receiving further information about the assessment process and the support available for unpaid carers in Lincolnshire?

Living at home continued

• Telecare can raise an alert that the cooker hob has been left on;

• Telecare can detect gas leaks from the cooker/fire and raise an alert;

• Telecare can provide early warning of a sink or bath overflowing;

• Telecare can raise a call to a monitoring centre who can keep a phone line open to you in an emergency;

• Telecare can give people space and time on their own, instead of being checked up on ‘just in case something has happened’. One lady said, ‘I love my family but I just want to have some time to myself;’

• There is new equipment that specifically helps prompt and remind people with memory problems.

If a problem is detected then the system sends a signal via the lifeline to a call centre where an alarm can be raised 24 hours a day. The call centre uses trained operators, who can speak to you over a speaker on the lifeline, will have your details and know who to call in an emergency. This could be family, friends, carers or the emergency services. Alternatively, you could choose to have calls made directly to your chosen emergency contact.The telecare alert process:1. You press the lifeline button or the sensor raises

an alert.2. The lifeline unit is activated and alerts the call

centre via the telephone line. 3. The call centre answers your call and talks to you

through the lifeline.4. A response is arranged appropriate to your

agreed needs.

There is a small weekly charge for using telecare, depending on your circumstances. However, if you meet the social care criteria we will loan you the equipment.

Telehealth easily fits in with your life and routine. For example, taking your blood pressure and using ‘Flo’ to remotely submit the results to your GP - Tel: 01522 782155.

There are many different options available within the range of equipment that can help you with the everyday tasks around the home. You might not even be aware that there are so many different ways equipment can help you maintain your independence.

Telecare uses a range of simple sensors matched to your personal needs which can help you to continue to live as independently and safely as possible. These are usually linked through a small unit to your telephone. Telecare can help you and your family in a number of ways, all of which are designed to support your health and wellbeing.

One example is a bed sensor. This can be used in a range of situations, i.e. for people at risk of falling, or to alert a carer to assist. It detects a change in pressure, so if you have got up but are unable to get back to bed, perhaps after a fall, the sensor can raise an alert. However, if you get up at night and usually sit in a chair for a while or have a drink, then the system can delay the alert. This avoids the risk of someone falling and being left on the floor all night, cold and in pain.

Following an assessment of your needs, telecare could be part of a support plan to assist with things you might find difficult. It can support you or the person you care for by alerting you to certain situations, such as where a vulnerable person leaves the property at an inappropriate time, for example, the early hours of the morning. Or it can react to situations such as epileptic seizures or fire, where you need help from family, carers or the emergency services. Some examples of how telecare can be used are given below:

• A short term measure when you need some support, i.e. after ill health to help you become as independent as you can;

• A longer term measure to make you feel safer in your own home. One lady said, ‘I don’t feel alone when I am alone. The telecare can let someone know if I have a problem’;

• Telecare can remind you to take medication;

Your health professional can help you to manage many health conditions at home using a system called ‘Flo’ short for Florence Nightingale. ‘Flo’ helps you to help yourself. Ask your health practitioner for more details.

Adapting your homeLincolnshire Home Improvement Agency is an award-winning local charity which helps older people and people with disabilities to live independently and safely in their own homes. The service helps to ensure your home is accessible, secure and comfortable. You may find your mobility decreasing or be having trouble maintaining your property and the Agency may be able to help you

to improve security, ease of access and undertake repairs and improvements.

If you have a permanent disability and need an adaptation to your home to help you stay independent, you may qualify for a grant from your district council. Our occupational therapy service will be able to provide you with more information - Tel: 01522 782155.

Other equipment availableOccasionally, complex equipment may require setting up and the health or social care professional will provide some training and demonstrate to the service user and carer how to use the equipment.

As the service is a loan service there are facilities in place to return ‘no longer’ required items. The telephone number to arrange collection of equipment is 0845 121 2031, you can either speak with a person or leave a message.

Every effort is made to refurbish equipment to allow it to be recycled for re-issue to someone else. This reduces our costs and helps preserve our environment. Whilst every effort is made to collect items within 10 full working days after the request is logged, priority is given to deliveries.

Lincolnshire’s integrated community equipment service includes the following partners - Lincolnshire County Council, NHS Lincolnshire (Primary Care Trust), Lincolnshire Partnership NHS Foundation Trust and the United Lincolnshire Hospital Trust.

In addition to other services that enable you to stay at home, we have an Integrated Community Equipment Service (ICES) which provides short and long term loans of equipment, ranging from simple walking sticks, through to larger and more complex items such as pressure relieving mattresses and hoists. Equipment may also be designed to help you or to assist carers with the safer delivery of care. The service can also include installation, servicing and maintenance depending on the type of equipment specified.

Assessments of your equipment needs and the ordering of items are carried out by practitioners such as occupational therapists, social workers, nurses or other health/social care professionals.

Common items of equipment are held within a central warehouse. We have a targeted delivery timescale to deliver the equipment to the service user within seven working days after the order is placed. Technicians are trained to unpack and remove any transit packaging to enable the item to be used.

Occupational therapy

Occupational therapists can help you lead a more productive, active, and independent life using a variety of methods, including the use of adaptive equipment. They can assess your home for hazards and point out things that contribute to falls.

Visiting you at home, an occupational therapist will be able to suggest adaptations that will improve your mobility. Perhaps handrails up stairs would be useful? Would a ramp up to your front door help if you are in a wheelchair?

Major alterations such as fitting a downstairs bathroom or widening doorways could be paid for with a Disabled Facilities Grant (DFG).

An occupational therapist will talk you through any new equipment you may be given, as well as help you apply for funding for major adaptations, if you need them. To decide what sort of help you need, we will carry out a Community Care Assessment. This will determine whether your needs are eligible for overlap. If so, the assessment will look at what resources you are entitled to.

The support plan is a document which belongs to you, you can create the support plan yourself or choose whomever you like to help you, this can include our staff. The support plan sets the outcomes that you want to achieve and identifies the means by which you will achieve the outcomes to meet your eligible needs.

You will need to know how much you will have to pay if anything. This is called your ‘contribution’. To find out if you have to pay towards your support you will need to have a fairer charging assessment undertaken.

Reablement is about working with you to maximise your independence, this is usually a free service of up to six weeks duration and explained on page 12. If at the end of Reablement you still meet the eligibility criteria and require ongoing support you will need to complete a Resource Allocation System. You can do this yourself, with help from us or with help from any other people you choose, but the form will always be checked by us. Preparing mealsCommunity Meals, often known as ‘meals on wheels’, can help ensure that people receive proper nutrition on a daily basis, whatever their circumstances.

If you are aged 18 and over, or an older person, who is unable to prepare and cook one hot meal day, having your meals delivered could help make life easier. You made need this service due to a permanent situation such as sight loss, a physical or learning disability, illness or a temporary situation such as recovering after a stay in hospital, to help you maintain your independence and quality of life.

A Country Kitchen is a meals on wheels service that provides hot, healthy and nutritious food to your door every day. They already provide meals for Age UK and can provide them for people who need them in the Spalding and South Holland area. The Lincolnshire Kitchen is a meals on wheels service that also provides locally sourced hot food and even a sandwich supper to your door in the North Kesteven area.

• How many carers would the agency assign to care for you and would you see the same one every day? If not, how does the staff rota operate and what happens if your carer goes on holiday or is sick, will you be notified in advance that a different carer will be attending?

• How can you contact the agency in an emergency or outside office hours?

• How hard or easy would it be to make a complaint and how are things then put right?

• If this is a private contract ask for a copy of the agency’s contract terms and read these carefully with someone you trust. Get as much help as you can going over it and ask any questions you may have before signing anything. The Citizens Advice Bureau or Age UK (previously called Age Concern) could help with this.

These questions may be useful when considering using the services of a home care (domiciliary) provider to help you build up a picture of how your care needs will be met in your own home.

The Lincolnshire Carers Partnership and the Lincolnshire Young Carers Partnership are here to support unpaid carers across

Lincolnshire, in their caring role.Our aim is to reach out to hidden and hard to reach

carers, estimated at 75,000 in Lincolnshire alone. If you are an unpaid carer living in Lincolnshire,

there are six important things you should know about in order to be linked in to the support available.

A Carer’s Assessment ‒ It can be difficult to recognise yourself as a carer, you might think you’re just being a good friend, neighbour or spouse and are, ‘just doing your job’. In reality, being a carer can be a difficult and lonely job and a good place to start is with a carer’s assessment. This is an opportunity for you to share your experience as a carer with a dedicated support worker who can guide you towards the help and support you are entitled to as an unpaid carer. It explores the impact your caring role has on your life, and helps to identify the right support to meet your needs.

Carer’s Emergency Plan ‒ What would happen to your loved one if you were unable to care in the event of an emergency? Your Carer’s Emergency Plan is created by you, and provides detailed instructions for others to follow in an emergency situation, giving you peace of mind. Even better, it’s free to set up!

Carer’s Information Pack ‒ This pack, designed by carers, is an excellent information source for all carers, including an A ‒ Z booklet of help and advice on many subjects relevant to carers, a caring journal to record your interactions with professionals, and much more. An information insert for carers of people with memory problems

Looking after yourself as a carer is really important not only for the person you look after, but for your health and well-being too. You may not even identify yourself as a ‘carer’ but the information

and dementia is being developed with help from people who have lived experience. This will be available in 2013 and offered at the same time as the Carer’s Information Pack, where needed.

Carer’s Newsletter ‒ This bi-monthly newsletter, created and edited by carers, is full of carers’ experiences, stories, news and events, advice and fun content, and aims to inform and entertain carers across Lincolnshire. It is a great way to keep in touch with carers’ issues and be linked into the Carers’ Partnership.

Carers’ Support Groups and Carers’ Partnerships ‒ Together with the Lincolnshire Carers’ Partnerships, many support groups, carer organisations and charities are working to support carers in Lincolnshire ‒ which one could help you? Anyone can join the Lincolnshire Carers’ Partnerships, and you can be involved as little or as much as you wish.

District Council Carer Champions ‒ All seven District Councils across Lincolnshire have a Carers Champion. They know how important carers are, and are working hard in their district to help raise awareness. Do you know the name of your District Council Carer Champion?

Day opportunities and day care

Day opportunities By ‘day opportunities’ we mean the things or activities people may want to do during the day. This covers all opportunities for people whether it be in the day, evening or at the weekend. We used to call this day services and/or day care.

We want you to have more choice and control about what you do during the day. Personalisation will enable you to make such choices and control the services you choose to receive.

Whilst the list below focuses on traditional buildings-based day centres, you may prefer support to do things in your community. You are able to use your personal budget to buy support directly from a care and support provider and helps you to do the things you most enjoy. We may be able to help you to develop such opportunities and tell you what alternatives may be available in your area.

Day care is a popular service offered to people who need some help with personal or practical care and are unable to get out and about by themselves.

Day care is usually available in a resource centre, community building or a residential home and offers more support than an ordinary day centre can provide. Professional caring staff can support you with personal care, such as going to the toilet, reminding you to take medication or help with washing or bathing. You may also be able to have a hot meal and drinks during the day and a chance to socialise and to take part in a number of activities.

Day opportunities often provide valuable help in sustaining independence for people. They are provided in a variety of locations including care homes, specially adapted premises and purpose built

centres or in the community. These services aim to help you retain independence in your own home, live healthily and sometimes give a break to a carer or family member who is helping to support you.

Some services may be specialised, catering for example for people with dementia, or other high dependency needs. In all cases, good day services should provide a stimulating environment able to address individual needs, including physical or mental frailty and still be met through a personal budget.

Access to day opportunities may be arranged privately in many cases by approaching the service provider directly. Some providers will tailor the support to meet your needs if you want to do something more suited to you as an individual.

The kind of day services we might arrange will depend on your needs. Priority is given to those whose independence is most at risk.

We are currently working with service providers, micro providers and personal assistants to develop day activities.

Please note that day opportunities services are chargeable. You will be offered a financial assessment to ensure you are not asked to contribute more than you can afford, and some people pay nothing. However if you have more than £23,250 in savings you will be asked to pay the full or maximum cost of your day services.

What if day care or a day centre isn’t for you?

Transport services

Practical support is available to help you if you have a learning or physical disability, or a sight and hearing loss problem, to become more involved and use the amenities in your local community. An assessment to work out just what type of practical support is needed to help you get out and about.

There are a number of different bus companies running throughout the county which can help you get out and about. If you are finding it difficult to use public transport due to a disability or mobility problem, community transport offers a number of different ways to get out and about to the shops and other local amenities.

Sheltered housing

Sheltered or retirement housing is suitable if you like living independently but want the reassurance of knowing that assistance is on hand if there is an emergency, or if you expect to be away from home for long periods and need to know your home is safe. Often called retirement housing, there are many different types of sheltered schemes, available both to rent and to buy. Schemes usually consist of between 15 and 40 dwellings. These may be bedsitting rooms, self-contained flats, bungalows or luxury apartments.

Some schemes are simply housing designed to meet the needs of older people and linked to a community alarm. The majority of schemes have a scheme manager/warden and community alarm service. There are often communal facilities such as a lounge, laundry, guest flat and garden. Meals are not normally provided but a few schemes include a restaurant and some can arrange hot meals.

Care homes and housing options

Extra Care

Extra Care housing supports you to live independently in your own home for as long as possible. Extra Care provides a secure environment with care services available 24 hours a day in your own home. Each apartment has a kitchen, bathroom, up to two bedrooms and a living room. The developments provide communal facilities, such as a restaurant and a hair salon, as well as areas to hold regular social activities.

Although it varies from scheme to scheme there is the opportunity to rent, part purchase or fully own an apartment. Whichever of these options you choose, your apartment will be your home. Should you currently benefit from financial help paying your rent or receiving support, this will still be available subject to your circumstances.

In Lincolnshire there are currently Extra Care schemes in Louth, Lincoln, Bourne, Sleaford, Grantham, Boston and Sutton-on-Sea. We are working closely with the developers to increase the number of schemes, which are managed by registered social landlords.

Choosing a care home

All care providers in the country must be registered with the Care Quality Commission. All services are inspected by the Commission, who report on their findings. These inspection reports are available from the care home or the Care Quality Commission website at www.cqc.org.uk

Care homes offering personal care onlyThese are commonly referred to as residential care homes. If you are reasonably active but would like greater security and care, subject to a needs assessment, a care home only offering personal care may be the option you choose. Personal care includes bathing, feeding, dressing, help with moving but it must be paid for if your capital/savings exceed £23,250. For more information about fees and charges for care homes please see page 44.

Care homes with nursing These are homes previously called ‘nursing homes’. If

you think you may need nursing care in a home, you will need to be visited by Adult Social Care to access the care you may need. This visit might be in your own home or in hospital if you have been ill. You will be fully involved in planning your care needs.

If a care home providing nursing care is the best solution for you, we will give you information to help you find a home that meets your requirements. The cost of the nursing care part of your fees is paid by the NHS to the home directly. Currently this is a flat rate of £108.70 per week. It is important that you get your nursing assessment done before you go into the home. You can contact the nurse case management teams for your area at the Primary Care Trusts (PCT).

Personal care is not paid for by the NHS and must be paid for by you if your capital/savings exceed £23,250. See page 46 for further information.

• Are there local facilities near the home, such as shops, places of religious worship or pubs that you will be able to access, and to which staff will take you when you want?

• Does the home have its own transport to enable you to get around, and how does it use it?

• Is public transport easily accessible?

• Does the home help you to keep in touch with your friends and family? Will staff help you read letters and write them, help you make phone calls? Are there private phones, is there email and internet access that is accessible to individual need?

The building

• Is there a garden or outdoor area that residents can safely access?

• Are there smoke detectors or fire extinguishers visible throughout the home? Are fire exits and escape routes clearly marked?

• What arrangements are there in place for making sure that people are safe in the event of a fire?

• Are there call bells in the bedrooms and communal areas?

• Is it easy to find your way around? This may be particularly important with regard to dementia.

• If your health gets worse can you continue to live at the home, with increased support from staff?

• Is it a dual registered home, that can provide qualified nursing care should your needs require this?

• What is the registration of the home and will it meet your needs on an ongoing basis?

• Who runs the home and is there a photo of them?

• Are they the owner or the manager?

• How long have they been running the home?

• How and when can they be contacted and what happens if you need to contact them outside of their normal working hours?

CARE HOMES - Residential dementia & memory problems care checklistThings to look for in care homes specialising in dementia. Please use in conjunction with the care homes checklist on page 33.

The points below give you a good starting point of things to look for in care homes specialising in dementia care. You can use this checklist with the one for residential care on page 33 to help you feel more confident in the choices you are making for your loved one or the person you are caring for.

RESIDENTS

The best indication of a good home is that the residents appear happy and responsive. Ask for more information on dementia specific activities, i.e. reminiscence or features throughout the home such as specialised signage that makes it easier for people with dementia or memory problems to identify where they are.

ACCESS

If the person with dementia or memory problems is likely to need equipment or adaptations you may want to check:

• There is adequate signage as described earlier throughout the home, to act as prompts and cues.

• What the home’s security policy about locking external doors or being supervised and supported in outside areas?

BEDROOMS

What room will the person with dementia or memory problems have?

• It can be worthwhile considering how easy it would be for someone to find their way back to their room, if there is one available on the ground floor to avoid the risk of falls.

• Ask if you can bring any personal items of furniture or personal possessions such as photographs that are particularly important to the person you are looking for. This can increase familiarity with their surroundings, trigger memories and help them settle in better.

ACTIVITIES

In any home there should be a dedicated activities programme that has been designed with all residents’ capabilities in mind, and generally a dedicated activities coordinator is employed to help deliver this with the other care staff in the home. It is important to ask which of these activities have been planned specifically for those who suffer from dementia or who have other memory problems.

• Are reminiscence activities available?

• Ask about memory boxes.

• Will staff compile life story books with you and the person you are looking for to give them a better understanding of their life?

STAFF

It is important to note whether the staff seem friendly and caring towards residents and whether they treat them with dignity and respect.

• Ask if staff have specific training and experience in caring for people with dementia.

• Who will their key worker be? This should be someone you and they can go to who will have detailed knowledge and particular responsibility of their care.

MANAGER/HEAD OF HOME

A manager who is caring as well as efficient can make all the difference to the atmosphere of a home. It is important for you and the person to have a good relationship so you feel confident in the general running of the home. It is worth talking to the manager and any deputies they may have on how long they have worked in the home, and their general experience. Points to ask about:

• Do they have detailed knowledge and training of dementia? Can they deal with any difficulties that may arise in an understanding way?

• Will they be conducting the initial assessment of the person requiring care in their own home or current place of residence?

Continuing care

Intermediate care

Continuing care is the name given to the care needed by an adult who because of disability, accidents, illness or the effects of getting older, requires help over a period of time to manage their daily life. It is a package of care that involves services and funding from both the NHS and social services and is available in a range of settings. All NHS healthcare is provided free of charge. Social services care provision is means tested and you pay either a contribution or the full cost depending on your financial situation and the setting in which your care is provided.

An assessment is done by health and / or social

care professionals to identify and prioritise your needs. The assessment will be used to determine who is responsible for providing and funding the services to meet your assessed needs. Decisions on how and where services will be provided will be made with your involvement.

If you have been assessed as requiring care in a registered care home with nursing and you are not eligible for NHS continuing healthcare the NHS will pay a contribution towards the cost of your nursing care. This is known as NHS-funded nursing care and is paid directly to the nursing home. The current amount is £108.70 per week.

Based around a programme of therapy and treatment, intermediate care is designed to make you as independent as possible. The structured programme of support may mean that you do not have to go into hospital at all, or may enable you to get home faster after you finish hospital treatment, but cannot manage independently at home. Ask your care manager for further details.

Intermediate care differs from respite care. While respite care provides care on a temporary basis, intermediate care is designed to rehabilitate you and enable you to manage by yourself. NHS continuing healthcare

NHS continuing healthcare is a package of continuing care that is arranged and funded solely by the NHS. If your assessed needs identify that you have a ‘prime health need’ you will be eligible for NHS continuing healthcare. This means that care services to meet your assessed needs will be provided and funded by the NHS.

The National Framework for Continuing Healthcare and NHS-funded Nursing Care 2009 (revised) sets out the eligibility criteria and the assessment process for accessing NHS continuing healthcare funding. Following a multi-professional

assessment of your needs the Primary Health Care Trust that your GP is registered with will decide whether or not you are eligible for NHS continuing healthcare.

If you are eligible your care will be provided free of charge to you in any setting where your assessed needs can be met. In your own home this means that the NHS will fund all the care required to meet your assessed needs. In a care home the NHS makes a contract with the home and pays the full fees for both your care and accommodation.

Alzheimer’s Society recently (26 March) published Dementia 2012, its most comprehensive review of how well our country supports those people with the condition. The results clearly show that people in the UK are still not living well with dementia. Almost half a million people are battling depression, loneliness or anxiety as they struggle with little support. The general public also revealed major concerns. 75 per cent of people without dementia said they do not think society is geared up to deal with the condition. One third of people with dementia live in a care home and at least two thirds of care home residents in the UK have dementia. Alzheimer’s Society wants care home staff to understand a lot about their residents and know what helps them cope and enjoy the best quality of life every day. Ensuring quality care for those living in care homes is central to

improving services for people with dementia. Launched on the same day as Dementia 2012, the Prime Ministerial Challenge marks a turning point in the fi ght against dementia. Doubling funding for research, tackling diagnosis and calling for a radical shift in the way we talk, think and act on the condition will help to transform lives. On care homes David Cameron is focusing on improving the quality of care. Ten care providers, like Bupa and Barchester Healthcare, have come together to set out what good dementia care should look like, pledging to live up to these standards and providing a good practice example for other care homes. In the near future a Care and Support White Paper will be published which will shine a light on the quality of care in every care home including feedback from family and friends.

The treatment of people with dementia is possibly the biggest challenge facing social care professionals. Jeremy Hughes, Chief Executive of Alzheimer’s Society, looks at the national picture and outlines what to look for when searching for dementia care options.

Keeping people safeSafeguarding vulnerable adults

Safeguarding and protecting vulnerable adults from abuse or neglect has traditionally been seen as a major role of those working in the social care sector. Now, more than ever given recent cases that have made national news, it is becoming an issue for everyone in society, not just for those people who have a professional responsibility to safeguard. Far too many vulnerable adults suffer abuse, neglect and worse, often at the hands of relatives and carers, those who you would least expect to treat them in that way. A vulnerable adult is a person aged 18 years or over who may be unable to take care of themselves, or protect themselves from harm or from being exploited.

Some people are more vulnerable than others because they are old and frail, or have a sensory impairment, a disability, a mental health problem, or have some form of illness. This means they may have difficulty in making their wishes and feelings known and this may make them vulnerable to abuse. It may also mean that they are not able to make their own decisions or choices. A person may also be vulnerable because of a temporary illness or difficulty.

What is adult abuse? Abuse is when somebody or some people do or say things to you or somebody else that makes you or them feel threatened or frightened. It is

mistreatment by somebody or some people that violates a person’s human and civil rights.

It may be neglecting a person’s needs, treating somebody disrespectfully or causing physical harm. It may be an isolated incident or it may be repeated incidents. It may be that the abuse is deliberate or it may be unintentional and due to ignorance or lack of training or understanding. It may be that a person is at risk of abuse or being abused in several ways.

It can happen anywhere - in a care home or a care home with nursing, a hospital, in the workplace, at a day centre or educational establishment, in supported housing or in the street.

Forms of abuse could be physical, sexual, psychological, emotional or financial. It could also cover the issues of neglect and institutional abuse ‒ where the abuse affects more than one person within an organisation or a service and is not addressed by the service’s management. This can be repeated incidents of neglect or poor standards of care that are being ignored. It may be that you are not treated with the dignity and respect that you deserve.

Who might be causing the abuse?Anyone may be an abuser. It may be somebody you know well or it may be a stranger. It may be more than one person. It might be a family member, a friend, a neighbour, a paid or voluntary carer, a nurse, a social worker, a GP, a visitor to your home, or somebody who you don’t know. If you live in a residential setting or visit a day centre, it may be another user of that service, a visitor, or a carer.

What should you do if you suspect abuse or are worried about abuse?Doing nothing is not an option. Lincolnshire County Council operates a zero tolerance of all forms of abuse. All adults should be able to live their lives free from fear and harm.

Your course of action will depend on the situation.

You will be able to speak to a sympathetic adviser who will listen to your concerns and take them seriously. They will offer support and direct your concerns to a specialist worker who will arrange to meet with you.

You may have a relative, friend or independent advocate present if you wish. Together you will discuss the issues and together you will agree a plan to keep you as safe and independent as possible. You will be able to have a copy of this plan. We will agree further contact as is needed.

We may involve other agencies to help support you safely, but we will agree this together. Where the abuse is a crime, the police need to be involved to prevent other people from suffering as well. We will jointly help to support you.

We will listen to your concerns and take them seriously. We will treat you with dignity and respect at all times. Our main aim will be to help you to live

safely whilst maintaining the maximum possible level of independence, choice and control.

If the abuse is also a crime such as assault, racial harassment, rape or theft you should involve the police to prevent someone else from being abused. If the police are involved we will work with them and with you to support you.

Making a complaint about care services continued

Lincolnshire LINk is a network bringing together local people, organisations and groups who want to improve health and social care services throughout the County. The LINk gives everyone in our community the chance to express their views and relate their experiences of local health and social care services.

As part of the Government’s on going health and social care reforms, the LINk will evolve into a local Healthwatch from October 2012.

The new HealthWatch body will be an independent group of volunteers representing the public. It will not only react to feedback from patients and service users to resolve issues

If you are concerned about the care that you, a friend or a relative is receiving, it makes sense to speak to the manager before you take any further action. The problem may be resolved quite easily once they are made aware of it.

However, if you need to make a formal complaint, you should initially contact the registered owners of the service. They have a duty to respond to any complaints made. If your complaint is about a breach of regulations, contact

and signpost for further assistance, but will also proactively monitor the quality of services to promote improvements for children and adults in health and social care. HealthWatch will also provide information and advice to people who need to make choices about the services they access. From spring 2013 they will also offer an independent complaints advocacy service.

Care home contracts for self-funding residents

If you are self-funding your care and have a contract with your care provider, it is important to speak to solicitors with experience in this field. Any contract should explain: the cost of care, what services are included in the price and how often this is reviewed; what complaints procedure is in place; what your rights are and the obligations of the care provider under the contract; the procedure for cancelling a contract and the administering of medication.

What is a self-funder?

What options are available to me as a self-funder?

If you have capital or savings of more than £23,250 you will have to pay for any care services you require but you are still entitled to an assessment of your needs by us.

If you have assets of less than £14,250 you will be entitled to assistance with your funding from us. Between these two figures, a sliding scale operates whereby you must pay £1 per week for each £250 you have above the lower figure.

Before you think about the likely costs of any care and support, it is important that you get information and advice about the options open to you, both regarding the type of services that might suit you, and whether your local council might contribute to the costs. If you don’t you may make

poor decisions about either the care you organise or how you pay for it or both.

MyChoiceMyCare.org.uk will feature a financial calculator under the ‘Information, Legal and Financial Issues’ lozenge from the end of 2012. This function will allow you to determine exactly how much you may have to contribute towards your care fees in the privacy of your own home. If you are a friend, relative or carer this facility will also give you the opportunity to help someone calculate their liability towards paying for care before an assessment is conducted. If you are paying for the full cost of your support needs, you are known as a ‘self- funder’ which means:

• you have chosen not to ask us for support; or

• you have been assessed but do not meet our eligibility criteria; or

• You can still contact us and ask for an assessment of your needs; they can help you to identify your support and outcomes to help you plan for your support needs.

• You can have a financial assessment even if you think that we will not make a contribution to your support needs.

• You may find that you are entitled to support with the costs or be entitled to benefits you are currently not receiving.

• you have been assessed as eligible for support but are moving to a care home and will not qualify for help with the fees because your income, savings or capital assets exceed £23,250 (please note this capital limit usually changes every April), or you are eligible for care and support at home but due to your level of capital/savings or income you have been assessed to pay the full chargeable cost or the maximum charge, and you have therefore decided to fund your care and support yourself.

• If you have to pay the full cost of your support, we can offer information, advice and guidance on support and opportunities available to you.

• We can offer information, advice and support to some people to help to try to regain your independent living skills through the use of equipment and technology.

• If you have a carer, your carer is entitled to a carer’s assessment.

• We can offer information, advice and guidance on preventative services; we have a number of services within the community that can support your health and well being ‒ see First Contact, Excellent Ageing.

• Reablement ‒ you could be eligible to time limited support especially after being in hospital, having a fall etc.

Assuming you will be moving into care and you meet the eligibility criteria, you may be entitled to some of the following financial assistance and support:

Twelve-week property disregard If your former home is included in your financial assessment but your other capital is less than £23,250, and your income is not enough to meet your care home fees, we will help with the cost during the first twelve weeks of permanent care provided we agree that care is needed.

Deferred Payment Agreements After the twelve-week property disregard period, any financial help from us will be charged against the value of your home and recovered once your house has been sold. A legal charge will be placed on your home, and there will be some legal and administrative costs incurred in the set up of the agreement, it is important to seek independent legal and financial advice in these circumstances. Please see the ‘Deferred Payment’ leaflet available from us for more information.

However, we may limit how much we will pay and it may affect your entitlement to pension credit if your property is not seen to be on the market and becomes treated as capital by the Department of Work and Pensions.

Attendance Allowance This is a non-means tested, non-taxable benefit from the Department of Work and Pensions paid at

the lower rate of £51.85 per week for those needing care by day or night, and at the higher rate of £77.45 per week for those needing care both during the day and night. Everyone can, and should, claim this benefit. Please note that entitlement to this benefit does change upon admission to care so it is important that you contact the Attendance Allowance unit with any change in circumstance to avoid any overpayments in benefit being made.

NHS community healthcareYou can receive community healthcare services in any setting, including your own home or in a care home. This care is usually provided by a community nurse and is free of charge. Healthcare services may also be provided by therapists and specialist nurses or through organisations such as Marie Curie or your local hospice. Again these services are free of charge.

What will you have to pay for your care?

care you will be entitled to a non-means-tested NHS Nursing Care Contribution (currently £108.70 per week for the standard rate) towards the cost of your nursing care. This is paid directly to the home. Access to NHS continuing healthcare funding is through an assessment process to determine if you have a prime health need as set out in the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care 2009 (revised). If you are eligible for this funding the NHS will fund the cost of your care in any setting where your assessed needs can be met. This funding will cover the cost of your board and accommodation if you live in a care home. It is not means tested. However your eligibility to receive this funding will be reviewed by NHS staff on a regular basis and may be withdrawn following a review and a period of notice.

Do you have more than £23,250 in capital and savings?

What is ‘capital’?The value of your home is included when assessing capital unless:• Your partner or:• A relative who is over 60 or disabled or:• A child under 16 who you or a former partner

maintain still lives there.

NOWe will contribute financially towards your care costs.

How much?To work out how much we will pay, you must arrange a Community Care Assessment. This will define your care needs and how they will be best met and your financial situation. Whichever homes you consider must be able to meet these needs.

Your choice of care home will be limited to those that accept our funding level. More expensive homes will expect you to arrange a third party to ‘top-up’ the difference. You are not allowed to do this yourself if your capital is below £23,250.

Third party payments

If the care home you eventually choose costs more than the rate we usually pay for a person with your needs, someone will have to make up the difference. This extra payment is often referred to as a ‘top-up’ or ‘third party top-up’.

Except in limited circumstances, the law states that you are not allowed to make this additional payment yourself. The responsibility for this often falls to a member of your family or a benevolent sponsor such as a charity. Once this person or organisation has been confirmed, they must sign an agreement formalising the arrangement.

It is important that whoever agrees to do this for you is made aware that the amount could increase in the future and they must be confident any increases will be met. Should payments stop for any reason, you should seek help and advice from us immediately.

Inspection and registration of care services

The Care Quality Commission registers, inspects and reviews all adult social care and healthcare services in England in the public, private and voluntary sectors. This includes care homes, care homes with nursing, home care agencies and NHS services, amongst others.

Since October 2010, all care providers have been registered under a new system, introduced by the Health and Social Care Act 2008, which will bring

adult social care, independent healthcare and the NHS under a single set of essential standards of quality and safety for the first time.

The Care Quality Commission will hold and publish up to date information about the compliance of health and social care providers with essential standards of quality and safely.

Following an inspection, each care home and home care agency is given a report of how it rates

How solicitors can help

A solicitor can give you impartial advice about wills, making gifts, estate planning and Powers of Attorney. Some can also offer guidance on immediate and long-term care plans, ensuring (if applicable) the NHS has made the correct contribution to your fees.

Lasting Powers of Attorney (LPA) allow you to appoint someone you trust to make decisions about your personal welfare, including healthcare and consent to medical treatment, and/ or your property and financial affairs. A LPA is only valid once registered with the Office of the Public Guardian. It allows for a person of your choice to make decisions on your behalf at a time when you may be unable.

The Court of Protection can issue Orders directing the management of a person’s property and financial affairs if they are incapable of managing their own affairs should they not have a LPA. The Court procedure is presently very slow and the fees are quite expensive so preparing a LPA is always advisable, providing you have somebody sufficiently trustworthy to appoint as your attorney.

An ‘advance directive’ allows you to communicate your wishes in respect of future medical treatment but it is not legally binding. You may instead wish to make a living will, properly known as an ‘advance decision’ setting out treatment that you do not want to receive in specified circumstances, which would legally have to be followed, even if you die as a result.

Any proposed gift out of your estate needs careful consideration of the benefits, risks and implications, particularly on any future liability for care costs or tax liability

Long-term care: whether you remain in your own home or move into sheltered or residential care, you may qualify for financial assistance in the form of social care and NHS-funded care and welfare benefits.

If you don’t have your own solicitor, ask family or friends for their recommendations. Contact several firms, explain your situation and ask for an estimate of cost and an idea of time scales involved. Many firms will make home visits if necessary and will adapt their communications to meet your needs. It’s important to find a solicitor who specialises in this area of the law.

The guide has been developed through partnership working between ourselves, NHS Lincolnshire, Lincolnshire Police and Age UK (previously called Age Concern).

The first edition of the guide was published in late 2008 with the second edition published in January 2010. The Good Life Guide is funded by the
