Country Roads – existing issues

■ We wanted to change the focus of care for our residents from acute to homelike

■ When the move from the old building to the new

occurred 10 years ago ingrained cultures and terminology came with the move

Country Roads – planning & implementation

How do we change ? ■ The opportunity arose when the facility had a VMO 3 days a

week, leading to a reduction in acute admissions; we could focus our time and energy with our residents

■ The ACI’s Living Well in MPS’ collaborative methodology and learning sets gave us the tools and enthusiasm to tackle the above

■ We did the surveys, reflected on our main issues and started to change small things using a PDSA cycle

Country roads – outcomes & embedding change ■ We to began to create an environment that our residents felt

more at home ■ We did the little things………….. ■ We began to understand that respecting their wishes were

of utmost importance ■ We are looking at getting know them better by changing

the admission paperwork ■ We haven't 'stopped doing the little things………….. ■ We aint there yet, but we understand what is required better

than we did before

Country Roads – take home message

■ Changes can be small & simple ■ Continue to try new things – use the PDSA cycle …..just

because it didn't work first-time around doesn’t’ mean it won’t work the next time with a few adjustments

■ Failure - we can learn from & it can lead to success

Remember to bring both cars to work …. You will be guaranteed to be working on the highway & country

road in the same shift
