Description of Root CausePossible Cause: Failure to verify location by test-hole (pot-holing)
Additional Comments
Excavator failed to refresh an expired locate ticket. The line was marked December 18, 2012, the day after the ticket was called in by the excavator. The line was marked correctly, therefore, the excavator also utilized mechanized equipment in a known tolerance zone. Overall, the excavator failed to plan an excavation to avoid damage or take reasonable steps to protect pipelines from damage.
