Objective

Experience
Director of Diabetes Education Program Hamilton Healthcare System 2009-present
Responsible for system wide education to diabetic patients. Provide individualized diabetic counseling with emphasis on patient empowerment with disease knowledge, nutritional counseling, and lifestyle modification with a goal towards evolution to self-management.
Responsible for coordination of diabetic management decision making with local group of 8 primary care physicians providing services within the Hamilton Healthcare System as well as regional endocrinology specialty providers.
Responsible for development and provision of group diabetic education programs as well as community outreach education.
Provide inpatient hospital consultations for uncontrolled diabetes with transitions coordination.
Certified insulin pump therapy nurse. Responsible for patient education and support for initiation and maintenance of insulin pump therapy. Coordination of care with primary care and endocrinology providers.
Responsible for patient and provider education on use of Continuous Glucose Monitoring as a tool for improved glycemic control and treatment decision-making.
2012-June 2014
n Responsible for direction/coordination of Diabetes, CHF, Cardiac Rehab, Pulmonary Rehab, Community and Employee Wellness Programs at Hamilton Healthcare System.
n Responsible for care plan oversight /coordination, educational outreach, quality assurance/ improvement for both inpatient and outpatients within 3 regional clinics and Hamilton Healthcare System.
2004-2012
n Responsible for initial program establishment, development of Cardiac Rehabilitation Program and its subsequent AACPVR certification.
n Responsible for initial program establishment, development, and subsequent AACVPR certification of the Pulmonary Rehabilitation program.
n Coordinated program expansion with addition of Pulmonary Function Testing, Exercise Stress Testing, and Ambulatory Holter Monitoring to core services.
n Developed Life Trax Wellness Transition Program for patient graduates of Cardiac and Pulmonary Rehab Programs.
n Responsible for establishing, development, and coordination of Congestive Heart Failure Program.
