RF for VTE in hospitalized medical patients persist for
weeks-months after medical discharge:
Rationale for extended prophylaxis.
In 1,897 pts diagnosed with VTE, 73.7% of episodes occurred in the
outpatient setting; of these 36.8% occurred in persons hospitalized or
with medical illness in the preceding 3 months. 2/3 of them within 1
months after hospitalization and 1/3, 2-3 mo after (Spencer FA, et al. Arch
Intern Med 2002; 162: 1471-75)
MEDENOX: 8% of the total VTE events occurred between days
15 and 110 (of them, 4 were fatal PEs) (Samama MM, et al. N Engl J Med
1999; 341: 793-800)

surgery or surgery for cancer with extended-duration

Definition of immobilization
• Complete Immobilization: The patient is totally confined to bed or
chair. The patient may be allowed to use a bedside commode or with
assistance may be allowed bathroom privileges.
• Decreased Level of Mobility: Immobilization caused by the illness
requiring the patient to remain in bed or chair more than 50% of
the time during daytime hours.
• Ongoing Decreased Mobility: Immobilization caused by the illness
requiring the patient to remain in bed or chair during daytime hours
more than was normal and usual for the patient prior to hospitalization

• Duble-blind, double –dummy, placebo controlled trial.
• 6528 Acutely ill patients with congestive heart failure or respiratory failure or
othe medical disorders and at least 1 additional RF for VTE, hospitalized with
an expected stay of at least 3 days.
• Apixaban 2.5 mg twice daily, for 30 days, vs. enoxaparin 40 mg for 6-14 days.
• Systematic bilateral CUS on day 30.
• PRIMARY EFFICACY OUTCOME: 30-day composite of death related to VTE,
PE, symptomatic DVT, or asymptomatic proimal leg DVT

• Prospective observational cohort study carried out in Geriatric
Centres (GCs) or Hospital-at-Home Units (HHUs)
• 507 elderly (> 65 yr) medical patients bedridden for at least 4 days
due to acute medical illness were recruited from 49 Spanish centres
Mean age: 82+ yrs; 72% women
70.6% of pts treated in Geriatric Centers (GCs)
29.4% in Hospital-at-Home Units (HHUs)
• Bemiparin sodium (2,500 or 3,500 IU/d depending on the VTE risk)
for 4-5 wks (63% received the high bemiparin dose)
• VTE events, major or minor bleedings, thrombocytopenia and

EXCLAIM conclusioni
La profilassi prolungata con enoxaparina riduce l’incidenza di TEV in confronto a placebo (2.5% vs. 4%; differenza rischio assoluto a favore di enoxaparina, −1.53 % [95.8% CI, −2.54% to −0.52%]).
Enoxaparina ha aumentato l’incidenza di eventi emorragici maggiori (0.8% vs. 0.3%; differenza rischio assoluto a favore di placebo 0.51% [95% CI, 0.12% to 0.89]).
L’utilizzo di enoxaparina in profilassi prolungata ha dimostrato un rapporto rischio/beneficio particolarmente favorevole in pazienti con livello 1 di mobilità, nei pazienti > 75 anni, e nelle donne

• Inibitore selettivo, competitivo, diretto e reversibile del Xa
• Legame a sito attivo
• Inibisce l’attività del fattore Xa sia libero che legato alla fibrina e l’attività protrombinasica
• Inibizione dell’attività del Xa: dose dipendente con prolungamento del PT
• Inibisce la generazione di trombina,
• No effetto sull’aggregazione piastrinica

• Elevata biodisponibilità (circa 80%)
