ENROLLMENTINFORMATIONNecessaryFormsPupilsenteringLaVilleElementaryforthefirsttimemustpresentanofficialbirthcertificate,socialsecuritynumber,andacompletedhealthrecord.Thesehealthrecordsmustincludealldatesofrequiredimmunizationsincompliancewithstatelaw.WheneverachildenterstheUnion-NorthUnitedSchoolsforthefirsttime,ageneralhealth,family,andschoolhistoryformmustbecompleted,givingusdataforourrecords.Datacollectediskeptinachild’scumulativerecordfolder,whichismaintainedfromkindergartenthroughtwelfthgrade,thusprovidingacontinuousrecordofacademic,social,andgeneralgrowth.

SchoolEntryAgeBystatelaw,achildmustbefive(5)yearsofagebyAugust1toenterKindergartenfortheschoolyear.Addresses,Email&PhoneNumbersParentsarerequiredtonotifytheirchild’sschoolofficeofanychangesoccurringinthehousehold.Changeofaddress,telephone,email,andworknumbersshouldbereportedtotheschoolimmediately.Accurateinformationisneededincaseofanillnessoraccidenttoyourchildatschool.BookRentalTheBoardofEducationhasmadeitpossibleforstudentstosecureallnecessarytextbooksonarentalbasis.Theserviceallowsparentstoprovidethenecessarybooksforafee,whichrepresentsonlyafractionofthetotalcost.Lostbooksmustbepaidforifanotheristobeissued.Fineswillbeassessedfordamagestobooksbeyondthenormaluse.Bookrentalistobepaidatthetimeofregistration.Ifforsomereasonafamilyisunabletodothisthefamilymay:1. Setdateswiththeelementarytreasureratwhichtimeacertainamountcanbepaid.2. Ifamoneyproblemexists,aFree/ReducedLunchformneedstobecompleted.ThecriteriaforFree/ReducedlunchesisusedtodetermineiftheStateofIndianawillreimbursetheschoolforbookrentalowedbyneedyfamilies.Parentswillbenotifiedoftheamountstillowed.UnionNorthUnitedSchoolCorporation’sAcceptableUsePoliciesTheeducationalvalueofstudenttechnologyandInternetaccessisthejointresponsibilityofstudents,teachers,parentsandemployeesoftheUnionNorthUnitedSchoolCorporation(UNUSC).Sinceaccessestothetechnologicaladvancesareavaluableandlimitedresource,studentsareexpectedtoplaceapremiumonthequalityofuse.Takingupvaluablebandwidthandaccesstimetopursuefrivolousends,wouldnotbeconsistentwiththemissionoftheUNUSC.Thesestatementsareresponsibleforsettingandconveyingthestandardsthattheirchildrenshouldfollowwhenusingmediaandinformationsources.UNUSCmakesthedistrict’scompleteInternetpoliciesandproceduresavailableforreviewbyallparents,guardians,andothermembersofthecommunity.1.CareandUsageExpectationsA.Allusersmustbeconsistentwiththeeducationalmissionandgoalsoftheschooldistrict.B.Theintentoftheusepolicyistomakeclearcertaincases,whichareconsistentwiththeeducationalobjectivesoftheschooldistrict,nottoexhaustivelycountallsuchpossibleuses.C.TheSuperintendentandhisdesigneesmayatanytimemakedeterminationsthatparticularusersareorarenotconsistentwiththepurposeoftheschooldistrict.

D.ParentsandstudentswillberequiredtosigntheInternetusageformfortheirstudentstoparticipateinaccessingtheInternet.Studentswillalsoberequiredtosigntheform.E.Electronicdevicesandthecasesloanedorleasedtostudentsandstaffshallbeusedonlyforeducationalpurposesthatdirectlyrelatetoschoolprojectsorassignments,unlessotherwiseexplicitlyauthorizedbyabuildingadministrator.F.Usersareresponsibleforthepropercareofelectronicdevicesatalltimes,whetheronoroffschoolproperty,includingcostsassociatedwithrepairingorreplacingthedevice.G.Ifadeviceisstolen,areportwillbemadeimmediatelywiththebuildingprincipalandthetechnologydirector.Iftheprincipalortechnologydirectorcannotlocatethedevice,thentheparentswillneedtofileareportwiththelocallawenforcement.H.Thepolicyandrulesapplytotheuseoftheelectronicdeviceatalltimesand/orplaces,onoroffschoolproperty.Studentsareresponsibleforobeyinganyadditionalrulesconcerningcareofdevicesissuedbyschoolsstaff.I.Usersshallnotalterthedeviceconfigurationinanyway.Nosoftwareapplicationsshallbeinstalled,removed,oralteredonthedeviceunlessthetechnologydirectorexplicitlygivespermission.J.Thedeviceistobeusedonlybythestudentorstaffmembertowhomitisissued.Thepersontowhomthedeviceisissuedwillberesponsibleforanyactivityoractionperformedonthedevice.K.Thedevice,cords,andcasemustbereturnedinacceptableworkingorderbythelastdayoftheeachschoolyear,uponwithdrawalorexitdatefromtheschooldistrict,andwheneverrequestedbyschoolstaff.2.OutliningProhibitedUsesUnacceptableusesofschoolelectronicresourcesinclude,butarenotlimitedto,thefollowing:A.AccessingorCommunicatingInappropriateMaterials–Usersmaynotaccess,submit,post,publish,forward,download,scanordisplaydefamatory,abusive,obscene,vulgar,sexuallyexplicit,sexuallysuggestive,threatening,discriminatory,harassing,bullyingand/orillegalmaterialsormessages.B.IllegalActivities–Usersmaynotusetheschooldistrict’scomputers,electronicdevices,networks,orInternetservicesforanyillegalactivityorinviolationofanyBoardpolicy/procedureorschoolrules.C.ViolatingCopyrightsorSoftwareLicenses–Downloading,copyingduplicatinganddistributingsoftware,music,soundfiles,movies,imagesorothercopyrightedmaterialswithoutthespecificwrittenpermissionofthecopyrightownerisprohibited,exceptwhentheusefallswithintheFair(ExtraCarriageReturn)UseDoctrineoftheUnitedStatesCopyrightLaw(Title17,USC)andcontentiscitedappropriately.D.Plagiarism–UsersmaynotrepresentastheirownworkanymaterialsobtainedontheInternet(suchastermpapers,articles,music,etc).Whenusingothersources,creditmustbegiventothecopyrightholder.E.UseforNon-School-RelatedPurpose–Schooldistrict’scomputers,electronicdevices,networksandInternetservicesareprovidedforpurposesrelatedtoeducationalprograms,schooloperations,andperformanceofjobresponsibilities.Incidentalpersonaluseofschool

devicesispermittedaslongassuchuse:1)Doesnotinterferewiththeuser’sresponsibilitiesandperformance2)Doesnotinterferewithsystemoperationsorothersystemusers3)Doesnotviolatethispolicyandtheaccompanyingrules,oranyotherBoardpolicy,procedureorschoolrules.“Incidentalpersonaluse”isdefinedasusebyanindividualforoccasionalpersonalcommunicationsF.MisuseofPasswords/UnauthorizedAccess–Usersmaynotsharepasswords;useotherusers’passwords;accessoruseotherusers’accounts;orattempttocircumventnetworksecuritysystems.G.MaliciousUse/Vandalism–Usersmaynotengageinanymalicioususe,disruptingorharmtotheschooldistrict’scomputers,electronicdevices,networkandInternetservices,includingbutnotlimitedtohackingactivitiesandcreation/uploadingofcomputerviruses.H.AvoidingSchoolFilters–Usersmaynotattempttooruseanysoftware,utilitiesorothermeanstoaccessInternetsitesorcontentblockedbytheschoolfilters.I.UnauthorizedAccesstoBlogs/SocialNetworkingSites,Etc.–Usersmaynotaccessblogs,socialnetworkingsites,etc.prohibitedbybuildingadministrationortheTechnologyDepartment.Teachersandstudentsusingauthorizedsocialnetworkingsitesforeducationalprojectsoractivitiesshallfollowtheagerequirementsandlegalrequirementsthatgoverntheuseofsocialnetworkingsitesinadditiontotheguidelinesestablishedinthispolicy.J.WastingSystemResources–Usersshallnotusethenetworkinsuchawaythatwouldwastesystemresourcesordisrupttheuseofthenetworkbyothers.Thisincludesbutisnotlimitedtoexcessiveprinting,filestorage,onlinegames,andvideo/audiostreamingnotdirectlyrelatedtoeducationalprojects,asdeterminedbythesupervisinginstructororbuildingadministrator.K.UnauthorizedEquipment–Usersmaynotattachunauthorizedequipment,includingpersonallaptops,tablets,andhandhelddevices,tothedistrict’ssecurednetworkwithoutpermissionfromtheTechnologyDepartment.L.iPadsorcaseswillnotbedecoratedwithstickers,writings,orothermarkings.M.Usersshallnotuseschooldistrictcomputersornetworksforpurposesofpersonalprofit.N.Theuseofcamerasinanytypeofelectronicdeviceisstrictlyusedforeducationalpurposesonly.O.Camerasusedintherestroomandlockerroomarestrictlyprohibited.3.ConsequencesforMisuseA.Schooladministratorsmayrevoketheuseofdevicesduetopoorperformanceinacademics,attendance,nothavingcorporationissuedtechnologyinsuppliedcaseand/orbehavior.B.Violationofpoliciesorrulesgoverningtheuseofelectronicdevicesoranycarelessuseofthedevicemayresultinastudent’sdevicebeingconfiscatedand/orastudentonlybeingallowedtousethedeviceunderthedirectsupervisionofschoolstaff.ThestudentwillalsobesubjecttodisciplinaryactionforanyviolationsofBoardpolicies/proceduresorschoolrules.C.UseoftheInternetisaprivilege,notaright.StudentsviolatingpoliciespertainingtostandardsofconductorInternetuseshallbesubjecttorevocationofprivilegesandpotentialdisciplinaryand/orappropriatelegalaction.

4.TextbookandTechnologyFees A.Whenschoolproperty,equipment,orsuppliesaredamaged,lost,ortakenbyastudent,afeewillbeassessed.Thefeewillbereasonable,seekingonlytocompensatetheschoolfortheexpenseorlossincurred.Thedevice,cordand/orcaseapplytothispolicy.B.Achargeshallnotexceedthecombinedcostofthematerialused,freightand/orhandlingandprocessingcharges,andnominaladd-onforloss.C.Intheeventtheabovecourseofactiondoesnotresultinthefeebeingcollected,theBoardauthorizestheSuperintendenttotakethestudentand/orhis/herparentstosmallClaimsCourtforcollection.D..Thedevicemayberestricteddueto:a.Failuretopayfordevicedamagesand/orcostofrepair.b.Failuretopayfortextbook/technologyrentalbytheendofthefirstsemesteroranapprovedpaymentplan.c.Multipleinstancesfordevicedamages5.AccidentalorIntentionalDamageA.Accidental,purposeful,and/ormaliciousdamageasdeterminedbyschooladministratorswillbesubjecttothefullreplacementvalueoftheelectronicdevice.Inaddition,studentsmayberesponsibleforcompensatingtheschooldistrictforanylosses,costs,ordamagesincurredforviolationsofBoardpolicies/proceduresandschoolrules,includingthecostofinvestigatingsuchviolations.Theschooldistrictassumesnoresponsibilityforanyunauthorizedchargesorcostincurredbyuserswhileusingschooldistrictcomputers,devices,ortheschoolnetwork.B.UNUSCmakesnoassurancesofanykind,whetherexpressedorimplied,regardinganyInternetserviceprovided.Theschooldistrictwillnotberesponsibleforanydamagestheusersuffers.UseofanyinformationobtainedviatheInternetisattheuser’sownrisk.Theschooldistrictwillnotberesponsibleforanydamagesuserssuffer,includingbutnotlimitedto;

1.Lossofdataresultingfromdelaysorinterruptionsinservice.2.Accuracyorqualityofinformationstoredonschooldistrict’scomputers.3.NatureorqualityofinformationgatheredthroughUNUSC’sInternetaccess.4.Personalpropertyusedtoaccessschooldistrictcomputersornetworkforschooldistrict-providedInternetaccess.5.ForunauthorizedfinancialobligationsresultingfromUNUSC’sInternet.6.UNUSCanditsemployeesandagentsassumenoresponsibilityforillegalactivitiesofstudentswhileusingschoolcomputersorschoolissuedelectronicsresources7.Familieswillnotbechargedinsurancefortechnology.However,ifadeviceislost,stolen,orintentionallydamagedthentheentirereplacementcostwillbechargedtothestudent.Afifty-dollarmaximumchargeperincidentwillbechargedwhenadeviceisdamagedorbroken.Iftheprotectivecaseisdamagedbeyondnormalwearandtear,thenthefamilycouldbechargeduptothereplacementcostatthediscretionoftheprincipal.

6.StudentInformationSecurityA.Usersmaynotrevealpersonalinformation,includingahomeaddressandphonenumber,aboutthemselvesoranotherindividualonanyunsecuredelectronicmedium,suchaswebsites,

5.Homeworkrequestsforabsences:

Homework/MakeUpworkislefttothediscretionofeachteacher.Studentswhoareabsentmaybegivenhomeworktocompleteduringtheirabsenceiftheparent/guardiancallsintorequesthomeworkthemorningoftheabsence,priorto9:00am.Theworkcanbereadyforparent/guardianpickupby3:00pminthefrontoffice.Studentswhowillbeabsentforanextendedperiodoftime(3ormoredays)shouldhaveaparent/guardianspeaktotheAttendanceOfficer.

6.SchoolProtocolforallStudentAbsences:

Whenastudentreaches5unexcusedabsencespriortoSpringBreak,awarningletterwillbemailedtotheparent/guardian,statingthelimithasbeenreached,andinformingthemofwhatneedstooccurtokeepingoodstanding(i.e.medicalnotesforfutureabsences).Whenastudentreaches10unexcusedabsencesinaschoolyear,anotherletterwillbesentalongwithanattendancecontracttosignandfollow.Anyviolationofthecontractwithresultsinareferraltothefamily’sresidentcounty’struancypreventionprogram.

LockerInspectionPolicyAllstudentlockersarethepropertyoftheschoolcorporation.Theselockersaremadeavailableforstudentuseinstoringschoolsuppliesandpersonalitemsnecessaryforuseatschool.Thelockersarenottobeusedtostoreitemswhichcause,orcanreasonablybeforeseentocause,aninterferencewithschoolpurposesoraneducationalfunctionorwhichareforbiddenbystatelawofschoolrules.Thestudent’suseofthelockerdoesnotdiminishtheschoolcorporation’sownershiporcontrolofthelocker.Intheinterestofschoolsafety,UnionNorthUnitedSchoolCorporation’sPolicy5571onSearchandSeizurewillbefollowedtoinspectthelockeranditscontentstoinsurethatthelockerisbeingusedinaccordancewithitsintendedpurpose,toeliminatefireorotherhazards,maintainsanitaryconditions,attempttolocatelostorstolenmaterialsandtopreventuseofthelockertostoreprohibitedordangerousmaterialssuchasweapons,illegaldrugs,alcohol,ortobacco.ProhibitedArticles Problemsariseeachyearbecausechildrenbringarticlestoschool,whichareahazardtothesafetyofthemselvesorothers,orinterfere,insomewaywithschoolprocedure.Hazardousitemswouldinclude,butnotlimitedto:guns,bullets,pellets,BBs,bombs,ingredientstoassemblebombs,knivesorothersharpedgeddevices,andmatches/lighters.Disruptiveitemstoschoolprocedurewouldinclude,butnotbelimitedto:caps,bats,andhardballs.Theseitemsareprohibited.Ifbroughttoschooltheywillbeconfiscatedandreturnedonlytotheparent.Cellphonesandotherdevicesmaybebroughttoschoolbystudents,butmustbeturnedoffduringtheschoolday(thisincludesrecess/breaktimes).Textingand/orpicturetakingarenotallowedduringtheschoolday.Misuseofthedevicewillresultindiscipline.Studentsmaybegrantedpermissionunderspecificcircumstancesbyaschoolemployeetouseadevice.Anytoys,includingremotecontroltoys,orotherarticlesnotusedforeducationalpurposesbroughttoschoolbystudents,aretheresponsibilityofthosewhobroughtthem,nottheschools.Thesemaybetakenawayshouldtheyposeaproblemintheclassroomorontheplayground.Studentsarenotallowedtobringanyitemstoschooltosellortradewithotherstudents.DrugandChemicalAbuseThepossession,saleoruseofdrugs,includingalcoholandtobacco,isprohibitedinschool.Ifastudentviolatesthis,he/sheissubjecttodueprocess,whichmayresultinexpulsion.

LaVilleElementaryisfortunatetohaveacommunityofcaringparentsandthosewhocanvolunteermanyhourstoassistteachers,byprovidingtutoringandcoordinatingand/orsupervisingspecialeventsandactivities.Guidelineshavebeendevelopedtoenhancethevolunteerexperience.Parentsareaskedtocomplywiththefollowingguidelines:1.Allvolunteersmustobserveallsafetyandsecurityrulesandprocedures.Theserulesandproceduresincludesigningin,participatinginsafetydrills,signingout,accesstoclassrooms,etc.
