PTAS Sdn Bhd – Negara Brunei Darussalam September 2004 to January 2007 Group HSE and Quality Manager

Employed to manage the Group’s, HSE and Quality Management System which includes seven business operating units and fourteen joint venture operating units both in Brunei Darussalam and Australia. Duties included the complete review of the operational activities within the business units and to prepare, implement and maintain a single management system for the Group. Prepared, implemented, maintained and achieved certification to an Integrated Management System in compliance with ISO 9001, ISO 14001 and OHSAS 18001 to which PTAS Group were the first in the South East Asian region. Duties further included supervising a team of HSE Officers, QA Managers, QC Engineers and Inspection personnel. Ensure that work is carried out in compliance with international standards, client specific requirements and by competence and trained personnel. Thiess TKK Joint Venture – Darwin LNG Project September 2003 to August 2004 QA/QC Manager- Brisbane, Qld and Darwin, NT Employed as a contractor/consultant for the Darwin LNG Storage Tank. Thiess TKK

were awarded the Design, Procurement and Construction contract for the fabrication and erection of one 188,000 cubic meters LNG Storage Tank. Duties included the establishment, implementation and co-ordination of quality control programs for construction consistent with design standards, specifications, standards, codes, statutory regulations and the contractual obligations. Allocate responsibilities and supervision of all QC personnel and co-ordinate Inspection and Test groups.

Ensure all construction process and installations are performed in accordance with procedures and quality control programs in particular to the 9% Nickel Steel. Provide expertise and assistance to site activities including queries about QA/QC requirements. Develop specialised QC procedures for site activities. Responsible for the overall organisation and management of the site audit program and reporting any actions that arise from audit findings, identifying and scheduling audits and the identification/selection of audit teams and scope. Ensure that all quality assurance and quality control documentation generated by Suppliers, Contractors and on-site is compiled progressively for final handover.

Leighton Contractors Pty Limited – Stanwell Magnesium Project April 2003 to August 2003 Site Quality Manager – Rockhampton, Qld Duties included the establishment, implementation and co-ordination of quality

control programs for construction consistent with design standards, specifications, standards, codes, statutory regulations and the contractual obligations. Allocate responsibilities and supervision of all QC personnel and co-ordinate Inspection and Test groups. Ensure all construction process and installations are performed in accordance with procedures and quality control programs. Provide expertise and assistance to site activities including queries about QA/QC requirements. Develop specialised QC procedures for site activities. Responsible for the overall organisation and management of the site audit program and reporting any actions that arise from audit findings, identifying and scheduling audits and the identification/selection of audit teams and scope. Advise the Project Director of any quality related problems and prepare technical data for satisfactory resolution and ensure that the resolution of the problems are implemented and monitored. Ensure that documentation generated by Contractors is compiled progressively for final handover. Due to financial problems, the project have been mothballed into a care and maintenance mode indefinitely.

Kellogg Joint Venture – Train 4LNG Expansion Projects June 2002 to March 2003 Senior QC Inspector – Karratha, WA Duties were responsible for ensuring all welding, non-destructive testing (NDT)

performed at field adheres to Project specifications & procedures and providing assistance to Construction Engineers in inspecting works. Oversee and assist the Construction Engineers in the inspection of civil, electrical, structural steel, piping works and other inspections and supervise field inspection personnel and monitor inspection activities at field to verify compliance with approved Erection Contractor ITPs. Designation of workload assignments for Welding and NDT Inspectors, maintaining all records relating to welding, welder qualifications, welding procedures, NDT, weld tracking, material traceability and other related documents. Evaluation of welding quality problems and provide recommendations, solutions and corrective action.

United KG – Various Fabrication Contracts February 2002 to June Quality Superintendent (Workshops) – Kwinana, WA

Employed to manage all quality related activities on all fabrication work being carried out at the 3 main workshops based at Kwinana. Duties included but not limited to facilitate the development and implementation of the Division’s quality management systems, providing technical support and co-ordination of the planning, execution and recording of process and product verification, consistent with the achievement of both the client and statutory requirements. Key responsibilities included the assessment and reporting on system performance, review the suitability and effectiveness by division management as a tool for initiating ongoing improvement. Provide technical and quality input to tender submissions, facilitate the development and implementation of quality plans, inspection and test plans.

Coordination of inspection and test activities records undertaken in-house and at suppliers and maintain a program of review and analysis of products/process conformance/non-conformance and associated corrective/preventative actions.

United Construction Pty Ltd – Ridgeway Gold Mine-Concentrator Project May 2001 to January 2002 Quality Superintendent – Orange, NSW Employed for the construction and commissioning of the project. Managed all

QA/QC activities on and offsite, comprising of piping, structural steel, civil, earthwork, electrical and instrumentation, NDT, buildings, mechanical equipment installation, surface treatment from the fabrication, erection and commissioning stages. Responsible for the preparation and review of all quality assurance documents i.e. procedures, ITP’s, welding specifications etc. including subcontractors. Supervision of all quality inspection personnel including sub contractors. Review, compile and approved the MDR prior to submission to the client upon completion of the project.

Bradken Mining – Various Engineering, Fabrication and Foundry Contracts April 2000 to April 2001 Western Region Quality and Environment Manager – Perth, WA Responsibilities include the implementation and maintenance of the Quality and

Environmental Management System in accordance with the requirements of AS/NZS ISO 9001 and AS/NZS ISO 14001. Due to the recent major changes within Bradken and Smorgon Steel, my duties included the review of three separate Quality Management Systems (two ISO 9002 and one ISO 9001) to combine into a single Quality Management System for Bradken Mining, which was achieved in 12 weeks to AS/NZS ISO 9001-1994. Actively involved in the continuous improvement of processes and systems with Senior Management.

Bradken WA – Foundry Works August 1999 to March 2000 Consultant – Perth, WA Appointed to review the Quality Management System in place at both the

Bassendean and Welshpool Plants. Due to the inefficiency and lack of responsibilities associated with the Quality Management System, it was necessary to restructure the complete Quality System, which included rewriting all the procedures, job instructions, generating forms and delegating specific responsibilities to individual Managers, Supervisors etc. Upon completion of the above, was approached by Senior Management to review the current work practices carried out in the workshop and maintenance contracts in order to recommend any changes or improvement which will increase productivity, cost reduction, improve safety and quality of the finished product.

Raj Management Services – Consultancy Services January 1999 to July 1999 Consultant – Perth, WA Providing QA/QC, 3rd party Inspection and Consultancy services to various clients in

the Oil & Gas and Petro-Chemical Industries. Duties included providing technical assistance in preparing procedures, specifications, contract documents, vendor

United Construction Pty Ltd – Consultancy Services June 1998 to December 1998 QA Consultant – Kwinana, WA Employed on contract to advise, assist and review Manufacturers Data Records for

shop fabrication of the topside’s pre-assembled units on the Lamanaria “FPSO” project for Woodside Offshore Petroleum. Responsible for the detail review of the MDR and closing out of all Engineering Changes pertaining to Quality related issues prior to submission to Woodside and the Certifying Authority on a progressive basis till completion of the project.

Flour Daniel Pty Ltd – Nickel and Cobalt Processing Plant Project April 1998 to May 1998 QA Manager – Murrin Murrin, WA Employed initially on contract as a Quality Manager to replace the former Quality

Manager on the Anaconda Nickel / Cobalt Mine Project with a value of Australian 0ne Billion dollars. Due to the near completion and nature of this fast track project, various serious discrepancies were encountered on the Project Management System. Approached by Senior Management to conduct an assessment on the project management system, which was in place on site. Carried out a detail assessment covering Engineering, Procurement, Construction and Quality Assurance. Produced a detail report in three weeks highlighting areas of concern and recommending corrective actions, which are to be implemented immediately.

Bouygues Offshore Australia – Sulphuric Acid Plant Project February 1998 to March 1998 Test Engineer – Murrin Murrin, WA Appointed to compile and produce test packs comprising of hydrostatic, pneumatic

and service tests on the Sulphuric Acid Plant for Anaconda, which was being constructed by Monsanto-Bos Australia Joint Venture. Duties included the review of these packs against P&iDs to isometrics, marking up of test limits, NDT clearance, generating punch list items on critical components prior to test, witnessing and re-instatement of these lines in accordance to specification and contractual requirements. Preparing maintenance and servicing manuals for various equipment and systems to cover the warranty period.

Bouygues Offshore Australia – Stag Offshore Production Facility Project August 1996 - to February 1998 Quality Manager/Construction Coordinator – Perth, WA and South Korea Appointed to manage all QA/QC activities on the Stag - Central Production Facility

for Apache Energy Limited. BOS Australia were awarded the contract for the design, procurement, supply, manufacture, fabricate, assemble, painting, testing, pre-commissioning, certification, load-out, sea fastening, transportation, installation, hook-up, system commissioning, final handover and the project management. Responsible for the implementation of the project quality plan, assign and coordinate inspection, NDT, verification and certification activities, coordinating the preparation, approval and implementation of quality control procedures and ITP’s to cover the entire scope of work including subcontracted works. Duties included conducting internal and external audits, identify and assist in the resolution of non-conformances, ensure all documentation is compiled progressively to meet certification and statutory requirements.

United Construction Pty Ltd – Hot Briquette Iron Project November 1995 to August 1996 QA/QC Superintendent - Perth, WA

Appointed to set up a quality management system for the Port Hedland Hot Briquetted Iron Project for BHP Direct Reduced Iron Pty Ltd. United Construction were awarded to fabricate and erect the concentrator facility which included electrical and instrumentation, civil, earthworks, structural steel, piping, mechanical installation, conveyors, pressure vessels, surface treatment, non- destructive testing, HVAC etc. This project was awarded and carried out as an alliance contract between United Construction and BHP. Responsible for the preparation and review of project specific procedures, quality plan and inspection and test plans in accordance with ISO 9001 and contract specifications. Managing all QA/QC activities including sub contractors for the various discipline of work to ensure proper control during fabrication, erection and commissioning stages. Supervision of all QC personnel and document controller’s to ensure that work is being carried out as per the contract. Conduct 3rd party audits to ensure project compliance.

United Construction Pty Ltd – LPG Processing Plant Project August 1994 to November 1995 QA/QC Superintendent - Perth and Karratha, WA Employed on contract on the LPG Process Facility project for Woodside Offshore

Petroleum. Managed all QA/QC activities on and offsite, comprising of piping, structural steel, civil, earthwork, electrical and instrumentation, HVAC, fireproofing, NDT, buildings, mechanical equipment installation, insulation and surface treatment from the fabrication, erection and commissioning stages. Responsible for the preparation and review of all quality assurance documents i.e. procedures, ITP’s etc. including subcontractors. Supervision of all quality inspection personnel including sub contractors. Review, compile and approved the MDR prior to submission to Woodside on completion of the project. Upon completion of the project was approached by Woodside to prepare a detail yearly Inspection and Test requirements for maintenance programs and shutdown requirements covering piping systems, equipment and monitoring activities.

Chiyoda Corporation – LNG Train ‘F’ Project March 1992 to September 1993 Chief QC Engineer/Construction Superintendent - Bontang, Indonesia Responsible for all QC matters on the Train ‘F’ LNG expansion Project for

PERTAMINA on the 57kms 36” underground gas pipeline from the Badak Field to

the plant in Bontang. Duties include supervision of a team welding, civil earthwork, coating and NDE inspectors. Acting as a Construction Superintendent when required for the co-ordination of manpower, equipment and construction activities. Chiyoda Corporation were the Project Managers on this project. Assisted PERTAMINA to train local Inspection Engineers for surveillance work.

United Construction Pty Ltd – Maintenance Contract LNG Plant June 1990 to January 1992 QA/QC - Welding Engineer - Karratha, WA Attached on the maintenance contract on the LNG plant for Woodside Offshore

Petroleum. Duties include inspection of all fabricated products and major shutdowns both on the plant and the North Rankin ‘A’ platform. Responsible for all QA/QC, welding, and planning activities especially on major shutdowns on quality and welding related matters. Assisting Woodside Planning Engineers and Inspection Engineers for yearly inspection and testing of piping systems, equipment, condition monitoring etc.
