Kentucky Premier League 2016-17 Season Team Application Instructions

Please review entire document prior to beginning application process:

In order to submit an application for the Kentucky Premier League (KPL), teams must be created by the CLUB REGISTRAR in the 2016-17 Seasonal Year of Affinity Sports. A coach or a Team Admin must also be assigned to the team by the club registrar in order to submit the application.

Applications must contain at least a team manager or a head coach as contacts. To begin the application process, follow the steps below.

Click on the Affinity Sports Link Login into an existing account OR Click Forgot Password

o As a team manager or coach Log into your Existing Account on the left side of the page: Enter username and password and click ‘Login’ Forgot Password –

Click on Forgot password and input the requested information You will be emailed a link with information on how to change your Username and your

password. You will be able to login immediately after you have updated your login information. If you need further assistance with obtaining login information, please contact Affinity Sports at

855-259-3466 After logging in, you should see all of your family members as well as several tabs. If you are not already on the

‘teams’ tab, click on that tab.

You will see a list of teams you are assigned to for the 2016-2017 seasonal year. If you do not see any teams for

the 2016-2017 seasonal year, you may still need to submit your information for a background check and be added to the team by the club registrar (Visit the Risk Management webpage on kysoccer.net for more information on completing the background check registration). If you do see your team, click on the ‘Team Info.’ Link to the right.:

You will be taken to your team page, click on the tournament tab,

Click on the drop down bar, then choose the 2016-17 Kentucky Premier League and click the register button (see

next page for screen shot).

Application Submission information: You will have to fill out the following sections:

o Team info: Please note that the information in the Team Info tab is MANDATORY for filling out the application. If this information is not completed, the application will be considered INCOMPLETE.

o Player Roster Tab: This is where your roster will show up (if it’s not in the system at the time of the application, you will be able to sync the roster at a later date). Hit continue.

o Comments Sections: You do not need to enter anything here unless there is specific information you would like to add. Hit continue.

o Review Application: i. Review the application in its entirety to ensure that all information is correct. When finished

click “Submit Application” at the top, right of the page. o Payment: The fee for the 2015-16 KPL season is $375. League fees will be returned to teams that are

not accepted in the KPL. o Please choose method of payment by the drop down menu and hit continue (credit card, e-check,

check). ii. Credit Cards will not be charged until teams are accepted to the Kentucky Premier League.

iii. If you wish to mail in a check, choose check, hit continue, type in the check number (or team name if you do not have a check number) and hit continue again. Review and click “Submit Payment” Please click “View Payment Summary Coupon” and print. Include payment coupon with a check made payable to Kentucky Youth Soccer and mail to the following address: Kentucky Premier League 158 Constitution St. Lexington, KY 40507

b. You will receive a confirmation email that your application was received.

c. In order to edit your application, log in to your account and click the edit button that is next to the team name under “Tournament Application/Game Scoring”
