Welcome! Welcome to the KPMA 2018 Annual Scientific Meeting! Dear Colleague: Thank you for supporting our state association by attending our annual scientific meeting. The 2018 Scientific Meeting will feature a diverse program of lecture topics. They will be presented in a didactic lecture format and will feature presenters from across the country. The purpose of this seminar is to provide a broad overview of developments in the field of Podiatric Medicine. The lectures will be geared toward licensed podiatric physicians. Participants will receive continuing medical education credits that will satisfy the licensure requirements of the Commonwealth of Kentucky. If you have any ideas or suggestions for speakers or topics for future meetings, please send them to the association office. We appreciate your participation and support. Thomas King, DPM Scientific Chair Registration Information Member $350 Early bird/$400 late fee Non-member $650 Early bird/$750 late fee Doctor registration includes continental breakfasts, breaks, lunch, up to 20 hours of CE, and admission to the exhibit hall. CECH Hours The KY Podiatric Medical Association is approved by the Council on Podiatric Medical Education as a provider of continuing education in podiatric medicine. KPMA has approved this activity for a maximum of 20 continuing education contact hours: Friday offers 8.5 total hours. Saturday offers 5 total hours. Sunday offers 6.5 total hours. You will be required to sign-in two times per day for attendance verification! You must turn in your CECH signature sheet at the end of each day. No commercial interest provided financial support for this continuing education activity. Conference Attendance Registration and payment does not automatically result in the issuance of any CECH credit. The CECH signature sheet attests that you have attended the program for the number of CECH hours awarded. A signed copy is required in order to receive the certificate of attendance. Cancellations for DPMs Cancellations must be submitted in writing to the KPMA office with the reason clearly stated. The CECH Chair and KPMA staff will review each request individually. Audience The intended audience for the meeting is physicians, resident physicians, and students of podiatric medicine. Presentations are presented at a post-graduate physician level.

Presenter Biographies Patrick A. DeHeer, DPM, is a member of the APMA, and a fellow of ASPS, ACFAS, and Diplomate of American Board of Foot & Ankle Surgery certified in foot surgery and reconstructive foot and ankle surgery. He is in private practice in Central Indiana and is the team podiatrist for the Indiana Pacers and Indiana Fever. He is on the

Editorial Board of Podiatry Today and a regular blogger for Podiatry Today. Dr. DeHeer has published several articles and is an international and national lecturer. He is the inventor of the EQ/IQ brace and several other medical inventions. He is the President and Founder of Step-By-Step Haiti and a medical missionary with over 25 trips worldwide.

Tina C. Del Buono, PMAC, is the Director and Practice Management Consultant with Top Practices Virtual Practice Management institute as well as the founder and CEO of Practical Practice Management. Tina has been a practice manager for 21 years and has written over 400

articles on the topic. For the past 13 years she has been the Educational Chair for the California Podiatric Medical Associations’ Assistant and Practice Management educational programs. She is a past President of the American Society of Podiatric Medical Assistants and served on their board for 12 years.

G. “Dock” Dockery, DPM, FACFAS, who likes to be called “Dock,” graduated from the Kent State University College of Podiatric Medicine in Cleveland, Ohio, as Valedictorian, and performed surgical residency in foot and ankle surgery at the Waldo Podiatric Surgical Residency Program in Seattle. He is Board Certified in Foot and

Ankle Surgery by the American Board of Foot & Ankle Surgery and is Board Certified in Podiatric Orthopedics by the American Board of Podiatric Medicine. Dr. Dockery was voted as “America’s Most Influential Podiatrists” in 2012, and Top Podiatrists for 2017 by the Podiatry Management Magazine Editors & Advisors. He has published numerous scientific articles and textbook chapters in peer-reviewed medical journals. He founded the International Foot & Ankle Foundation for Education and Research in 1979 and is currently the Chairman of the Board and Director of Scientific Affairs.

Michael Dujela, DPM, FACFAS, serves as the national chairman of the Education and Scientific Affairs Committee for the American College of Foot and Ankle Surgeons and is a peer reviewer for the Journal of Foot and Ankle Surgery as well as a consultant for orthopedic industry companies lecturing on emerging

technologies. Dr. Dujela has also been appointed to the executive board of the Global Foot and Ankle Community comprised of an international group of orthopedic and podiatric foot and ankle surgeons. He has trained with some of the world’s leading orthopedic surgeons through two international orthopedic surgery fellowships in Germany and Switzerland. His pride and joy are his wife Liliana, and their two children Christian and Maddie who enjoy travel, soccer, skiing and the amazing quality of life in the Pacific Northwest.

Eric H. Espensen, DPM, DABMSP, attended The Ohio College of Podiatric Medicine and completed his residency at the Southern Arizona Veterans Administration Health Care System. He opened and served as Director of the first Diabetic Foot Center in Southern California and was the Director of the Amputation Prevention Center in

Los Angeles, California. Dr. Espensen also served as Director of the University of Arizona: Southwest Academic Limb Salvage Alliance (SALSA). He helped develop and open two podiatric residency programs and served as the Residency Director as The University of Arizona. For 10 years, he was the Chief of Foot and Ankle Surgery at Providence Saint Joseph Medical Center. He currently works with Caremore to develop a podiatric medical/surgical care line.

Rem Jackson is the Founder and CEO of Top Practices, LLC, a company dedicated to helping professionals reach their full potential through innovative marketing, sales, management, and professional development programs. He led the team that founded Classroom Connect, a high-tech company dedicated to

helping K-12 educators integrate technology into their instruction. He contributed to the sale of Classroom Connect to Reed-Elsevier in 2001 and continued to serve as Vice President of Professional Development. His ability to guide participants through challenging content in an interesting and informative style has contributed to his national reputation as an entertaining, informative, and dynamic speaker.

Carl Kihm, DPM, earned his doctorate in Philadelphia at the Temple University School of Podiatric Medicine. He completed a three-year surgical residency training program at DeKalb Medical Center in Decatur, Georgia. He a board-certified fellow of the American College of

Foot and Ankle Surgeons and Diplomate of the American Board of Foot & Ankle Surgery. Dr. Kihm is a faculty member of the Podiatry Institute, an international teaching foundation for reconstructive surgery. He has authored surgery textbook chapters and many journal articles and completed seven surgery mission trips in Nepal and Guatemala.

Stephen A. Mariash, DPM, FACFAS, is a Fellow of the American College of Foot & Ankle Surgeons and Board Certified in Foot and Ankle Surgery from the American Board of Foot & Ankle Surgery. Dr. Mariash completed his Residency at Atlanta Hospital, (PSR-36). He serves as Faculty

and Board of Medical Advisors, International Foot & Ankle Foundation for Education and Research. He also serves as Faculty, American Academy of Foot and Ankle Osteosynthesis (AAFAO). Dr. Mariash practices at St. Cloud Orthopedic Associates in St. Cloud, Minnesota.

The child of two passionate physicians dedicated to a healthy perspective on pain, Danesh Mazloomdoost, MD, watched his parents build one of Kentucky’s first multidisciplinary pain clinics. He then went on to seek anesthesiology and pain management fellowships at Johns

Hopkins and MD Anderson. Dr. Danesh returned to Kentucky in 2010 and has been working to address the opioid epidemic and the neglect of proper treatments for painful conditions. He is now the medical director of Wellward Regenerative Medicine, a center dedicated to sustainable, non-surgical treatments for pain.

Jasmine N. Neal, DPM, was born and raised in Atlanta, GA. She attended Spelman College in Atlanta, GA where she received a Bachelor of Science degree in Biology. She then completed her medical education at California School of Podiatric Medicine and her three-year surgical resident training at Dekalb Medical Center

in Decatur, GA. While in residency, Dr. Neal lectured as a resident member of The Podiatry Institute of which she is now a Faculty member. Dr. Neal is now an inaugural fellow at Kentucky/Indiana Foot & Ankle Specialists specializing in foot and ankle reconstruction. While in fellowship, Dr. Neal hopes to continue enhancing her knowledge base. She also plans to continue lecturing with The Podiatry Institute.

Tracey C. Vlahovic, DPM, FFPM, RCPS, is a Clinical Associate Professor and J. Stanley and Pearl Landau Faculty Fellow at the Temple University School of Podiatric Medicine (TUSPM) in Philadelphia. She is a member of the Faculty of Podiatric Medicine of

the Royal College of Physicians and Surgeons. Dr. Vlahovic graduated from Yale University with a BS in Molecular Biophysics and Biochemistry and received her medical degree from TUSPM. She then finished her surgical residency at St Luke’s University Health Network in Allentown, PA. She is board certified in foot surgery by the American Board of Podiatric Surgery. Dr. Vlahovic currently teaches the Fundamentals of Dermatology course at TUSPM and has authored several articles in peer-reviewed journals.
