Provider DirectoryThis directory provides a list of Kaiser Permanente Medicare Plus’s network providers.
This directory is for the service area of our Mid-Atlantic States Region, which includes
the following counties and partial counties: District of Columbia; Maryland: The City
of Baltimore, and the counties of Anne Arundel, Baltimore, Carroll, Harford, Howard,
Montgomery, Prince George’s, Calvert,* Charles,* and Frederick; Virginia: The cities of
Falls Church, Fairfax, Alexandria, Manassas, and Manassas Park; the counties of Arlington,
Fairfax, Prince William, and Loudoun. Counties with an asterisk * are only partly covered
by our service area. If you live in a partly covered county, please see the service area
description inside for the zip codes that are in our service area.
This directory is current as of 01/2017. Some network providers may have been added
or removed from our network after this directory was printed. We do not guarantee that
each provider is still accepting new members. To get the most up-to-date information
about Kaiser Permanente Medicare Plus’s network providers in your area, you can visit

Kaiser Permanente is a Cost plan with a Medicare contract. Enrollment in Kaiser Permanente depends on contract renewal. The provider network may change at any time. You will receive notice when necessary.

SECTION 1
IntroductionThis directory provides a list of Kaiser Permanente Medicare Plus’s network providers . To get detailed information about your health care coverage, please see your Evidence of Coverage .
The network providers listed in this directory have agreed to provide you with your health care and vision services . You may go to any of our network providers listed in this directory, however, some services may require a referral . If you have been going to one network provider, you are not required to continue to go to that same provider . In some cases, you may get covered services from non-network providers .
Use this directory to learn about selecting your Primary Care Physician . This directory will also explain which specialty care services require a referral, how to get a referral, and where to go for urgent care and emergency care .
Your physician network includes more than 1,200 physicians in the Mid-Atlantic Permanente Medical Group, P .C . (Permanente physicians) who practice in Kaiser Permanente medical centers . Your network also includes affiliated physicians who do not practice in Kaiser Permanente medical centers, but are available in areas where a medical center may not be convenient .

Your primary care physicianYou will have to choose one of our network providers who are listed in this directory to be your Primary Care Physician (PCP) . The term “PCP” will be used throughout this directory . Generally, you must get your health care services from your PCP .
Establishing a relationship with your physician ensures that you get consistent, personalized care . Adults should select a PCP who specializes in internal medicine or family practice . You may also choose a family practitioner to care for your entire family . Women up to age 75 will choose a personal obstetrician/gynecologist (Ob/Gyn) in addition to their PCP .
Note: You may also choose an affiliated physicians for primary care and Ob/Gyn in some areas . Affiliated physicians are in private practice in the community .
Choosing or changing your doctorChoose or change your doctor by calling us at 1-888-777-5536 (TTY 711), seven days a week, 8 a .m . to 8 p .m . or by visiting kp.org/doctor . Read about our doctors on kp.org/doctor, including where they went to school, what specialty training they’ve had, and what other languages they speak . You can change your doctor at any time, for any reason .

Self ReferralsYou don’t need a referral from your PCP for the following specialties . Just call 1-888-777-5536 (TTY 711), seven days a week, 8 a .m . to 8 p .m . for an appointment .

For other types of specialty care, your doctor will refer you .
Referrals for specialty careSome services may require a referral . If your PCP decides that you require services from other providers, you will usually be referred to an in-network specialist . Should you require services not available in-network or where such services do not meet regulatory requirements for access, you must be referred to an out-of-network provider in the community for that service .
You must have an authorized referral for specialty care for us to cover the services and/or supplies provided . If you don’t have an authorized referral before you receive services, you will have to pay Original Medicare cost-sharing amounts .
Receiving services from out-of-network providers will be authorized only if the services are not available in-network . If a referral to an out-of-network provider is authorized, you pay only the cost share you would have paid if an in-network provider had rendered the service and/or supplies .
* Not all Kaiser Permanente Medicare Plus plans include optometry and/or dental benefits. Please refer to your Evidence of Coverage for a complete list of benefits.

Non-plan providersAs a member of Kaiser Permanente Medicare Plus, you must use Kaiser Permanente plan providers for in-network care . However, you may also use your Original Medicare coverage separate from your Medicare Plus benefits and obtain care from non-plan providers . If you do so, ask the non-plan provider to file a claim directly with Medicare . You will be responsible for Medicare coinsurance, any unmet portion of deductibles, and for charges from providers who do not participate in Medicare, to the limiting charge .
When a non-network provider sends you a billThere may also be times when you get a bill from a provider for the full cost of medical care you have received . If that happens, you should send this bill to us instead of paying it . We will look at the bill and decide whether the services should be covered . If we decide they should be covered, we will pay the provider directly . Bills can be sent to

Emergency CareYou are covered for emergency care .* If you think you are experiencing an emergency, immediately call 911 or go to the nearest emergency facility. Emergency care can always be obtained in or out of the service area from the nearest available provider .
If you are unsure if your medical problem requires an emergency visit, call 1-800-777-7904 (TTY 711) . Specially trained nurses are available 24 hours a day, seven days a week, and will direct you to the most appropriate place to receive care .
After the emergency is over, it is important that you notify us by calling the Medical Advice line so that we can be involved in the coordination of your follow-up health care . You can reach the Medical Advice line toll free at 1-800-777-7904 (TTY 711), 24-hours a day, seven days a week .
What is an emergency?A “medical emergency” is when you, or any other prudent layperson with an average knowledge of health and medicine, believe that you have medical symptoms that require immediate medical attention to prevent loss of life, loss of a limb, or loss of function of a limb . The medical symptoms may be an illness, injury, severe pain, or a medical condition that is quickly getting worse .
Post-stabilization carePost-stabilization care is medically necessary services related to your emergency medical condition that you receive after your treating physician determines that your condition is clinically stable . Your follow-up post-stabilization care will be covered according to Medicare guidelines . In general, if you emergency care is provided by non-plan providers, we will try to arrange for plan providers to take over your care as soon as your medical condition and circumstances allow . If our plan does not cover the post-stabilization care, but your Original Medicare does, you will pay the Original Medicare cost-sharing amount . Refer to your Evidence of Coverage for more information .
* Medicare members should check their Evidence of Coverage (EOC) for more details. In the Mid-Atlantic States Region, all Direct Pay Kaiser Permanente Medicare Plus High Option and Standard Option plan members have worldwide emergency care and urgent care coverage, and all Direct Pay Kaiser Permanente Medicare Plus Basic Option plan members have emergency care and urgent care coverage while inside the United States and its territories only.

Urgent careUrgently needed care is medical care you need right away because of an illness, injury, or condition that you did not expect or anticipate, but your health is not in serious danger .
Inside our service areaWhen in our service area you must use plan providers for urgent care, except in unusual and extraordinary circumstances, such as if our providers are temporarily not available or inaccessible (such as major disaster, epidemic, war, and riot) .
If you require urgent care during normal office hours, call the appointment and advice line . The nurse or representative will find the best location for you to receive the care you need . If your PCP is not located in a Kaiser Permanente medical center, we might ask you to call your physician’s office directly .
Outside our service areaIf you need urgent care while outside of the service area, we cover medically necessary services from a non-plan provider . You must be temporarily outside our service area and you must reasonably believe that your health would seriously deteriorate if you delayed treatment until you returned to our service are . In addition, when out of the service area, you can obtain dialysis treatment from any qualified dialysis provider accepting Original Medicare . You will be responsible for paying the Original Medicare cost-share amounts (such as coinsurance and deductibles) .
