A message from your President:…

Probing a New Future with Change

I t is with great excitement that I am able to write about the recent changes to our dental hygiene regulations in this Journal issue. Thank you to the many, many

members who worked so hard for so long to make this change a reality. Thanks especially to our Governmental Relations Council and Morgan Plant for the years worth of effort that made this possible. The ability to provide local anesthesia (Pennsylvania now joins 43 other states!) and work as a Public Health Dental Hy-giene Practitioner (PHDHP) is not only a great accomplishment for ourselves but

for our clients and those Pennsylvanians in need. Please carefully consider the training and/or application to receive your certifications today! This issue of the Journal provides detailed information on how to com-plete the process and requirements necessary for the applications. This Journal issue is also being made available to all Pennsylvania hygienists as we “go green” and allow everyone to view and/or print the infor-mation from our website – be sure to have www.PDHAonline.org as one of your favorite pages.

PDHA has other exciting things going on in addition to the changes in our regulations. Sealant Sat-urday time is upon us and we look forward to the camaraderie of these events and the services we are able to provide for so many. Please be sure and volunteer your time and expertise to this prevention-oriented and worthy cause. Get the word out in your communities that these dates are available to help make each one a success. Our association has also moved into the future with our very own Facebook page! Be sure to check us out and comment on your own dental hygiene status. This is a great place to network and to find old classmates and new friends. After only a few weeks online, we have over 200 friends!

So it seems this new year proves to be about changes after all. Sometimes these changes can be positive and other times just difficult. I have found in dental hygiene and in education that sometimes you need to just go with the flow of change. Schedules change at the last minute, people come in late or come in early, children get sick and need you to stay home and take care of them (which is what’s happening in my house right now), snow falls or many other changes happen and you need to be flexible. That’s how it is in our profession and in life and I wouldn’t have it any other way! If things never changed, think how boring it would be to never be able to show a new product to a client or have a day where your biggest pri-ority is to be a mom at home holding your child in the rocking chair. The changes come, ready or not, and I know our members will embrace these changes in dental hygiene as we’ve worked so hard to get here but please encourage others to go with the flow of change as well. You will need to talk to your employers, your clients, and all those you work with so that they understand the positive aspects of these changes.

I’m proud to say that dental hygiene is moving in a forward direction. As your president, I’m work-ing daily to keep things moving and I am fortunate to have a wonderful team of talented people to help with every step. I am also inspired every day by the students that I work with; their enthusiasm for our pro-fession is infectious. Students all over the state work so hard and happily embrace the new along with the old. Sometimes they are the ones who show me how to change and grow and I feel blessed to be a part of their learning and of their future.

Happy New Year to everyone reading this letter; this is a time for you to make a change and ex-pand your abilities. This is a chance for you to join or renew your membership and help us to continue to make positive changes in our profession. As always, we want to hear from all of you – make the change you want to see in your profession as we probe a new future together.

PDHA Governmental Relations Co-Chair On December 12, 2009, new regulations were published in the Pennsylvania Bulletin in final form. These new regulations revise the supervision, continuing education, and services of the dental hygienist. They also provide for the issuance of certificates/permits both to practice as a public health dental hygiene practitioner and for the administration of local anesthesia for dental hygienists. It is important to read the exact wording of the new regulations to understand what you can and cannot do and what requirements are necessary. Go to www.pabulletin.com under the issue dated December 12, 2009. Application forms to obtain the certificate to practice as a public health dental hygiene practitioner or the permit to administer local anesthesia are available on the State Board of Dentistry’s website at www.dos.state.pa.us/dent. The local anesthesia courses for graduate dental hygienists will be posted on PDHA’s website under the “members only” sections as they become available. The dental hygiene faculty members who attended the “Train the Trainers” course are waiting for their local anesthesia permits to arrive so they can teach the remainder of their dental hygiene faculty. They are moving as quickly as possible to address your requests for local anesthesia courses.

Information on the new Dental Hygiene regulations Submitted by:

Legislation was published on December 12, 2009, allowing dental hygienists in Pennsylvania to expand their scope of practice with local anesthesia, Public Health Dental Hygiene Practitioner (PHDHP), radiology, and general supervision changes. Please go to the PDHA website, click on the Legislative Update which opens PDHA President Megan Brightbill’s letter, and click on the Pennsylvania Bulletin link for the entire publica-tion. It is important that you review the entire wording as it applies to your state licensure. Here are highlights from the changes enacted as of December 12, 2009. Quotes are from the PA Bulletin: For local anesthesia, a permit is required, with the application fee of $20, and biennial renewal at $40, on the same cycle as the dental hygiene license. The injections include local infiltration, and “intraoral nerve block an-esthesia limited to the 2nd (maxillary) and 3rd (mandibular) divisions of the trigeminal nerve.” You must hold a current dental hygiene license in good standing and current certification in Basic Life Support (BLS). “Certification of education by the dental or dental hygiene program or … a form provided by the board verify-ing that the dental hygienist successfully completed, within the 5 years immediately preceding the filing of the application for local anesthesia permit, a course consisting of a minimum of 30 hours of didactic and clinical instruction in the administration of local anesthesia sponsored by a dental or dental hygiene education program accredited by the Commission on Dental Accreditation (CODA).” Continuing education must include 3 hours out of the required 20 hours of education in “courses related to the administration of local anesthesia, including pharmacology or other related courses.” The Public Health Dental Hygiene Practitioner requires an application fee of $20 and biennial renewal of $40. Requirements are a current PA dental hygiene license in good standing, certification of “3600 hours of practice as a licensed dental hygienist under direct supervision of the licensed dentist … and has obtained professional liability insurance or is a named insured covered by a group policy in the minimum amount of $1,000,000 per occurrence and $3,000,000 per annual aggregate.” Reduced fees for liability insurance are available to ADHA members. The 20 hour continuing education must include at least 5 hours in public health education. Practice settings include public and private educational institutions, correctional facilities, health care facilities, personal care homes, domiciliary care facilities, older adult daily living centers, continuing-care provider facilities, feder-ally-qualified health centers, public or private institutions under the jurisdiction of a Federal, State, or local agency, and fee and reduced-fee nonprofit health clinic. Further specifications including radiographic proce-dures are presented in the complete PA Bulletin. Changes are presented for radiographic procedures for dental hygienists where they can take radiographs with-out a dentist on premises under certain conditions. “Dental hygienists may perform radiologic procedures in any setting under (b) the general supervision of a licensed dentist. For the purpose of this subsection, ''general supervision'' means supervision by a dentist who examines the patient, develops a dental treatment plan, author-izes the performance of the radiologic services to be performed within 1 year of the examination, and takes full professional responsibility for performance of the dental hygienist.” General supervision has been changed for dental hygiene practice. According to the PA Bulletin, “General super-vision—In a dental facility, supervision by a dentist who examines the patient, develops a treatment plan, author-izes the performance of dental hygiene services to be performed within 1 year of the examination, and takes full professional responsibility for the performance of the dental 33.205(c)(2) and (3) (relating to hygienist. In facili-ties identified in § practice as a dental hygienist), general supervision is defined in 33.205(d)(2).” It states later in the document that “A dental hygienist may provide the professional services identified in (i) subsection (a)(1) under the direct supervision of a dentist, except that these services may be provided under general supervision if the dentist has reviewed the patient's dental records and medical history and has written a prescription or given an order for the placement of subgingival agents by the dental hygienist.” Please read the PA Bulletin

for full details regarding general supervision.

Actual wording for the regulation of the application of subgingival agents is: “A dental hygienist may pro-vide the professional services identified in (i) subsection (a)(1) under the direct supervision of a dentist, except that these services may be provided under general supervision if a dentist has reviewed the patient's dental records and medical history and has written a prescription or given an order for the placement of subgingival agents by the dental hygienist.” The dentist writes the prescription for or orders placement of subgingival agents. It states in the regulation “A dental hygienist is prohibited from establishing or maintaining an office or other workplace for the provision of dental hygiene services separate or independent from the office or other workplace in which the supervision of a dentist is provided.” PDHA does not support the idea of in-dependent practice. The issues presented are to aide in providing services to our patients in the dental of-fice and those with access to care issues.

Local Anesthetics for Dental Hygienists

HACC, Central Pennsylvania’s Community College is pleased to

offer Local Anesthetics for Dental Hygienists.

This course is designed to provide the licensed dental hygienist with the necessary knowledge and skills to admin-ister local anesthesia in a clinical setting.

The specific requirements set by the PA State Board of Dentistry will be addressed in this training.

After successful completion of this program, the student will be eligible for certification by the State Board of Den-tistry to administer local anesthetics.

Students must provide the following items prior to the start of class: ·Copy of current DH license ·Copy of current CPR/BLS certification ·Proof of current liability insurance Students will be required to present a photo identification on the first day of class.

IMPORTANT: PDHA is going green…

It’s time PDHA goes “green” with our Journal. Not only is this going to be the best use of our dues money, but it is also the next step to helping our environment.

Here is how it will work: • Three times a year, March, May, and December the Journal will be placed on the PDHA website (www.PDHAonline.org) in the ‘members only’ section. • The August issue (Annual Session issue) will be sent by USPS. • You will use your membership number to access the current issue. • We will also have a section for archives in the ‘members only’ section for you to go to back issues. You will be able to print out the entire Journal or only those sections you choose.

Here is what you need to do: As a member, go to the ADHA website “members only” section and be sure all your information is up to date in-cluding and most importantly your email address. This is the method PDHA will use to alert you when a new issue of the PDHA Journal will be ready for review. The website is: http://www.adha.org/news/real.htm

You, as a PDHA/ADHA member, must take it upon yourselves to alert ADHA and PDHA of your email changes so they can keep your records updated. Without this current information, it will be easy for you to get lost in the shuffle.

PUBLIC HEALTH DENTAL HYGIENE PRACTIONER (PHDHP) Submitted by: Beth Zimmerman, RDH, BS, MEd

The new dental rules and regulations for dental hygienists practicing in Pennsylvania have been passed and posted. The Public Health Dental Hygiene Practitioner (PHDHP) is now a reality. Now begins a new day of access to care for the underserved populations in Pennsylvania. With all the enthusiasm and anticipation coming to a climax, it is with caution that all those wishing to practice as a PHDHP begin. First things first: Read the rules and regulations carefully to see what sites qualify to practice. Complete all forms and documentation first before beginning practice. It is a good idea to have a business plan, policy and procedures manual and set fee schedule before practicing. Make sure to keep careful patient records and adhere to HIPPA regulations. At this time there is no insurance company state wide, either private or public, that will reimburse for fees. The best thing to do is start small and, if possible, talk to potential sites and build a good relationship with those sites. Most of the practice will be done on portable equipment which is a major investment. You can discuss with the sites the potential help of purchasing this equipment and possible monetary compensation. To continue practicing you will need to have continuing education courses specific to public health. Courses that have public health somewhere in the title are probably a safe course to take and can count towards your total number of credits needed but please if you have any questions, contact the PDHA’s Academy of Dental Hygiene Studies for advice. It is an exciting time to be a dental hygienist in Pennsylvania and we are looking forward to helping those that are underserved in community and public health settings. Keep the questions coming and we look forward to hearing from all of your experiences, both positive and negative.

ACADEMY OF DENTAL HYGIENE STUDIES The Pennsylvania Academy of Dental Hygiene Studies was formed in 1979 to encourage members to participate in continuing education courses and maintain their professional competence. The Academy provides a formal structure in which continuing education course completion can be recognized. Academy guidelines help ensure quality continuing education. Your participation not only benefits you, but it states to your patients and to your colleagues that you value education itself and care enough to continue staying current within your chosen profession. With mandatory continuing education needed for license renewal, you will be one step ahead since all collected information is computerized and kept on permanent record. The courses attended must have been approved by the Pennsylvania Academy of Dental Hygiene Studies in order to apply toward membership. Forms are available for Self-Study and Independent Study courses (courses taken with other sponsor approval) and may be submitted to the Academy for individual consideration. An Academy member must continue to provide proof of having attended seven (7) hours of Academy approved courses biennially plus the twenty (20) hours required for license renewal (27 total.) All Academy members must be a member in good standing of the American Dental Hygienists' Association. BENEFITS: • Your continuing education will be tracked with the Academy’s computerized recordkeeping • At no cost, you will receive one (1) personalized printout of your Academy courses each year • Reduced fees for all continuing education courses sponsored by the PDHA and the Academy • Certificate of Membership and Academy Pin • New members are received and inducted at the Academy meeting held during PDHA Annual Session APPLICATION PROCEDURE: Candidates are required to complete the application form on the bottom of the page and include the following information as appropriate for membership.

1. Applicants are required to list, on a separate sheet, a minimum of 30 PDHA Academy approved CEUs in continuing education coursework within the past three years. The listing must include the course name, date, PDHA number and the amount of CEUs awarded. 2. Proof of attendance at these courses must be submitted through photocopies of your Certificates of Attendance. 3. A letter of recommendation from a component, state or national officer or another Academy member. Candidates should obtain the letter sealed in an envelope and enclose with the application. 4. Please include a $25.00 filing fee -- make check payable to PDHA. 5. Application deadline is May 31st of the year of application. Academy membership applications are only processed in the spring of the current year.
