JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, ______________, of legal age, married, Filipino, and presently residing at _______________, and ________________, of legal age, single, Filipino, and presently residing at _____________________________, having been duly sworn to in accordance with law, do hereby depose and say:

1. That we personally know EMELITA CULILI PARACAD-MAGORA, being acquaintances for a long period of time since we were children;

2. That in her Certificate of Live Birth, it appears that her first name and date of birth are “ESTELITA” and “JANUARY 26, 1960”, respectively;

3. That we declare and certify that the one named “ESTELITA CULILI PARACAD” in the said certificate refers to the person of our friend, EMELITA CULILI PARACAD-MAGORA and that she was born on the 1st day of January, in the year 1960 and not January 26, 1960;

4. That she has been using “EMELITA” and “JANUARY 1, 1960 as her real first name and birth date, respectively in all her pertinent documents and transactions since childhood;

5. That we are not related by affinity or consanguinity to EMELITA CULILI PARACAD-MAGORA; and

6. That we are executing this affidavit to attest to the truth of the foregoing statement and for whatever legal purpose it may serve.
