Procedures. Complete Checklist 2. Pre-operation Safety Check Untrained operator Training on forklift design, controls and

instrumentation. Training on the forklift stability and the proper way to transport, load, and stack on the forklift.

3. Cranking & Driving Rolling the wheels off the edge or ramps and loading docks.

Buckle your seat belt when you’re in the truck. Stay well back from the edge. Never turn around on the slope. Drive with your load on the uphill side. This means you drive up slopes with your load in front and drive down slopes in reverse.

4. Operating the forks Running into obstacles. Stay within the vehicle lanes. Slow down and sound your horn at corners, intersections and places with limited view. Slow down for turns. Drive in reverse if your vision is obstructed. Make sure that you have enough overhead clearance.

5. Loading Load falling off forklift. Damage to material by loading improperly. Overloading Pinching hands between the truck and other objects

Spread forks as wide as possible for transporting the load. Tilt the mast slightly forward and align the forks with the base of the load. Proceed slowly and insert the forks into the base of the load. Ensure that at least 2/3rd the length of the fork enter the base of the load. 6. Transporting the load Running into objects Collisions with other equipment

Be Alert Wear gloves to protect your hands. Keep your arms and hands in the truck. Keep the controls and your hands clean and dry.

7. Unloading Injury to other personnel Damage to other equipment

Position the truck at the desired location. If stacking loads, adjust the height of the load as appropriate. Tilt the mast forward to place the forks parallel to the ground. Once the load is set, slowly back away from the load. When clear of the load, place the forks at the proper height for traveling.

8. Parking the Forklift Running over and crushing the operator. Never leave the truck until you lower the lifting mechanism, put controls in neutral and set the brakes. If you’re 25 feet (7 steps) away or can’t see the truck, turn the truck off. Never give rides or let anyone stand on the forks. Always administer the emergency brake.

JOB STEPS POTENTIAL HAZARDS RECOMMENDED SAFE JOB PROCEDURES Break down the job into its basic steps, e.g., what is done first, what is done next, and so on. You can do this by: 1. Observing the job 2. Discussing it with the operator 3. Drawing on your knowledge of job 4. A combination of the three. Record the steps in their normal order of occurrence. Describe what is done, not the details of how it is done. Usually three or four words are sufficient to describe each basic job step. For example, the first basic job step in using a pressurized water fire extinguisher would be: 1. Remove the extinguisher from the wall bracket.

For each step, ask yourself what accidents could happen to the employee doing the job. You can get the answers by: 1. Observing the job. 2. Discussing it with the operator 3. Recalling past accidents 4. A combination of the three Ask yourself; can he/she be struck by or contacted by anything; could they strike against or come in contact with anything; could the employee be caught in, on, or between anything; can they fall; be over exerted; or be exposed to anything injurious such as gas, radiation, welding rays, etc.? For example, acid burns, fumes.

For each potential accident or hazard, ask yourself what safeguards should be provided for the employee and how should the employee do the job step to avoid the potential accident, or what should they do or not do to avoid the accident. You can get your answers by: 1. Observing the job for leads 2. Discussing precautions with experienced job operators 3. Drawing on your experience 4. A combination of the three Be sure to describe specifically the provided safeguards and precautions an employee must use. Don’t leave out important details. Number each separate recommended precautions with the same number you gave the potential accident (see center column) that the precaution seeks to avoid. Use simple do or don’t statements to explain recommended precautions as if you were talking to the employee.
