APPENDIX A: Ruger JP. (2009) Health and Social Justice, Oxford University Press, Oxford, U.K.

This book offered a new paradigm for analysis of health disparities addressing complex issues at the intersection of economics, ethics and politics in health. The book is important for improving the theoretical foundations and empirical investigations for the evaluation of health disparities.

The book was the focus of a special panel on the “three leading general ethical theories of public health and health policy” stating that “bioethics has never before had at hand three book-length treatments of justice and health” at the 2010 American Society for Bioethics and Humanities Annual Meeting, comparing it to Just Health by Norman Daniels (Harvard) and Social Justice: the Moral Foundations of Public Health and Health Policy by Madison Powers (Georgetown) and Ruth Faden (Johns Hopkins).

The book was also the sole book for a panel with analysis by four public health scholars at the 2010 American Public Health Association Annual Meeting.

The book has been endorsed and reviewed by leading economists, philosophers, bioethics, medical and public health scholars.

Tom Beauchamp, Professor, Georgetown University, called the book “an impressive and rare union of theory and practice” that will “push forward current debates about health care reform and its theoretical foundations.”

Julio Frenk, Dean and T & G Angelopoulos Professor, Harvard School of Public Health, said the book “will become a beacon for the debates on health system reform in the United States and around the world.”

Amartya Sen, Thomas W. Lamont University Professor, Harvard University, Nobel Prize in Economic Sciences, said that “by producing a book of such richness concerning a major area of human agency and policy, Jennifer Prah Ruger has substantially advanced the reach of public reasoning, not just about health care, but about social justice in general.”

Arthur Caplan, Drs. William F. and Virginia Connolly Mitty Professor, NYU Langorne Medical Center, wrote that it “… is an important book not just as a guide to current debates, but for understanding how to navigate future challenges in the rapidly evolving environment of health policy in the United States and other nations.”

Thomas Pogge, Leitner Professor, Yale University, identified the book as “an attractive, concrete vision of a health society, strongly grounded in philosophy, economics and public health.”

Joseph Newhouse, John D. MacArthur University Professor, Harvard University, labeled the book “a must read for all serious students of health policy.”

In a review in Bulletin of the World Health Organization, Lawrence Gostin, O’Neill University Professor, Georgetown University, called the book a “seminal work” that “incorporates the philosophical, economic and political to make a compelling case that all societies … can design and build effective institutions and systems to achieve health capabilities …”

Health and Human Rights described it as “a new theoretical model.”

A review in Journal of the American Medical Association said it provides “a different perspective on the meaning of health and the importance of true collaboration on the micro as well as macro levels.”

A review in Medicine, Health Care and Philosophy asserted that it “... overcomes some theoretical divides, most importantly those between outcomes and procedures as well as freedom and welfare.”

In its review, Harvard Law Review called it an “important book” adding “... readers would be wise to heed [her model's] wisdom.”

David Orentlicher, Samuel R Rosen Professor, Indiana University, wrote in Inquiry “readers will benefit from the impressive interdisciplinary nature of Ruger's analysis. The range of her work cuts easily across political philosophy, political science, economics, law, public health, and medical ethics."

A review in CHOICE states, “Ruger combines sophisticated philosophy, concrete policy proposals, and astute observations [w]ith theoretically sophisticated and realistic policy analysis, this work will be an important read …”

A review by Kristin Whitehill Bolton of the University of North Carolina states, “Ruger’s ‘health capability paradigm’ eloquently illustrates a model capable of eradicating the current barriers to health and social justice. … Ruger’s health capability paradigm represents a anovel theoretical framework to guide future health care policy. … Overall, this book is impressive, as it is well researched and insightful.”

In International Sociology, Su-ming Khoo, National University of Ireland, Galway, writes, “Ruger’s interdisciplinary and substantive schema moves well beyond theoretical minimalism to elaborate a collective vision for equal access to quality healthcare, taking in problems, surrounding collective norms, individual responsibility and positive-health agency.”

Health and Social Justice was the feature of a special book symposium with analysis by eight scholars in the Journal of Human Development and Capabilities.

Paul Hunt, Professor and former UN Special Rapporteur and coauthor Joo-Young Lee described the book as a “major contribution…” and “the health and human rights communities are enriched by Ruger’s … health capability paradigm.”

Anita Allen, Henry R. Silverman Professor, University of Pennsylvania and member of the Presidential Commission for the Study of Bioethical Issues labeled my book “[A]n original theoretical framework … [A] fresh, systematic, forward-looking paradigm …”

Uwe Reinhardt, James Madison Professor, Princeton University concluded that he “view[s] Ruger’s book as the product of monumental scholarship that undoubtedly makes a significant contribution to future academic research on social ethics in healthcare and to academic courses focused on that subject.”

Ruhi Saith, Research fellow, Jawaharlal Nehru Institute of Advanced Study, Jawaharlal Nehru University, Delhi writes “Ruger’s work is a welcome addition and systematically explores in detail the theoretical basis for applying the capability approach as a theory of social justice to health. A reading of the book in the context of the developments in public health highlights the contributions that her work is able to make, amongst other aspects, to the problem of moving from a maximising to an equity oriented measure of health.”

Ruth Faden, Philip Franklin Wagley Professor of Biomedical Ethics, Johns Hopkins University and Madison Powers, Professor of Philosophy, Georgetown University, note “A subsidiary point that Ruger makes as part of her list of arguments for the focus on capabilities is that this focus incorporates within the account of the ends of political action a prominent role for individual responsibility … its policy implications are suggested in her emphasis on health agency, the ability of individuals to make better health choices for themselves, and the opportunity to have both the resources and supporting social conditions for maintaining commitment and adherence to those choices.”

Dr. Keerty Nakray, Queen’s University Belfast, Northern Ireland, states, “‘Health capability’ is a unique analytical concept proposed by Ruger, which seeks to distinguish between ‘good health policy’ and ‘a policy for good health’”

James Evans, Webster University, called it a “thoughtful and comprehensive book” and states, “Ruger demonstrates that she has… thought long and hard about multiple aspects of the ethical, legal, and political environments that impinge on public health and health care policy (that they are not the same thing she makes abundantly clear), and despite the interdisciplinary breadth of her research, I cannot think of a page of it that is extraneous… Her approach, which she calls the ‘health capability paradigm’ (HCP)… explains how a democratic society can implement health policies, including medical insurance and health care reform, to realize the health capabilities of its citizens…”

Mario Zangrando, Doctors with Africa CUAMM, writes, “Ruger presents a new theoretical model of universal health care[; the] “health capability paradigm”: an original synthesis of all these disciplines that illuminate a way forward toward a more rational health policy and health policy process.”

APPENDIX B: Ruger JP. (2018) Global Health Justice and Governance, Oxford University Press, Oxford, U.K.

This book lays out the critical problems facing the world today and offers a new theory of justice and governance as a way to resolve these seemingly intractable issues.

The book was the sole book for a special roundtable at the 2018 International Studies Assocation (ISA) Annual Convention, San Francisco, CA.

The book was the sole book for a special panel at the 2018 World Congress of Bioethics Annual Meeting, Bengaluru, Karnataka, India.

The book was the sole book for a book launch and roundtable discussion at the 2018 Human Development and Capability Association (HDCA) Annual Conference, Buenos Aires, Argentina.

The book has been endorsed and reviewed by leading political scientists, economists, philosophers, bioethics, medical and public health scholars.

Beth A. Simmons, Andrea Mitchell University Professor of Law, Political Science and Business Ethics, University of Pennsylvania states "Jennifer Prah Ruger has launched nothing less than a devastating critique of the shameful way human society tolerates unequal life-chances and allocates health resources world-wide. Her vision of shared health governance will strike some as idealistic, but it is a bold moral response to the global patchwork-of-a-system that has left so many people's lives at early risk."

Michael Marmot, Professor of Epidemiology at University College London; Chair of WHO Commission on Social Determinants of Health states "Inequalities in health abound within and between countries and are the major challenge of global health. Why should we act on them, and how should we bring various actions together? This work of both humanity and truly impressive scholarship puts human capabilities and flourishing at the centre and builds outwards. It is exactly the book I needed to give both theoretical structure and practical action for global health."

Amartya Sen, Thomas W. Lamont University Professor at Harvard University; Nobel Laureate in Economic Sciences writes "In this vigorously reasoned analysis of the injustices in global health situation today, Jennifer Ruger provides both insightful causal investigations and identification of promising ways and means of overcoming the problems that have to be addressed. Combining conceptual and analytical concerns with critically assessed proposals of remedial reforms, Ruger has made a major advance towards a better understanding of some of the most distressing aspects of the unequal world in which we live. This is an essential reading not only for health care specialists but also for concerned citizens across the world."

Henry S. Richardson, Professor of Philosophy at Georgetown University writes "Building on her influential health capability paradigm, Ruger here deepens its normative grounding, elaborates a global division of responsibilities for seeing to it that health justice is achieved, and pinpoints where the relevant agents are falling short on these responsibilities. She proposes and defends specific reforms of international law and of the ways that global institutions, nation-

states, and individuals go about securing health. Only Ruger, equally at home with foundational normative arguments, theories of governance, and epidemiology, could have built such a formidable and compelling edifice."

Ernesto Zedillo, Frederick Iseman '74 Director of the Yale Center for the Study of Globalization at Yale University; Former President of Mexico states "At a time when many relatively poor countries have decided to move seriously towards Universal Health Coverage while a rich one is trying to move farther away from it, Ruger's encyclopedic treatise on the ethical, justice and global dimensions of healthcare is pertinent and illuminating."

Ravi Kanbur, T.H. Lee Professor of World Affairs at Cornell University states "This book on will be an invaluable resource to a range of intersecting readerships, including students, analysts, and policy makers in global health, global justice and global institutions. The author gives a comprehensive account of how the current system works, its shortcomings, principles for redesign, and practical ways forward. She brings rigorous scholarship to bear on one of the major policy issues of our time."
