HISTOPLASMOSIS: A Fungus Among Us

Histoplasmosis is one of the most frequently occurring fungal infections in the Midwest but it is estimated that approximately 90 percent of the cases go unrecognized. Histoplasmosis is caused by the fungal organism, Histoplasma capsulatum. Infections with this fungus have been noted on every continent, except Antarctica, but it is very common in the Ohio, Mississippi, and Missouri river valleys.

Histoplasma is a dimorphic fungus, which means it has two forms, and appears as a light colored, fuzzy colony in the environment but changes to a single cell yeast when it grows in the human body. People can become infected when they inhale parts of the fungus called conidia. These small fungal particles can be suspended in the air and when inhaled will travel down into the smallest chambers of the lung, called alveoli. Conidia are engulfed by white blood cells and transform into the yeast phase. Histoplasma capsulatum prefers to grow in slightly acidic, moist soils, in temperate climates with average rainfall. This makes the forests, fields, and river bottoms of Missouri an ideal home for this fungus. Fungal spores are often suspended in the air by a disturbance, such as plowing a field or construction projects that require excavation. This fungus is also found commonly in areas with high concentrations of manure, such as chicken coops, bird roosts and caves. The fungus is not carried by the birds but has been found in the digestive tract of bats. Cave explorers are at increased risk of coming into contact with Histoplasma.

Most infections are mild and many resolve without treatment. Symptoms can include fever, chills, fatigue, muscle aches, headache, and a non-productive cough. Some infections result in the formation of granulomas which are small clusters of white blood cells that organize into multiple small masses that can be seen on chest X-rays. While most histoplasma infections are mild and involve the lung, serious infections are also seen, and can involve other areas of the body such as the lymph nodes, skin, gastrointestinal tract, and other organs such as the eye, liver, and spleen. These serious infections usually occur in patients with a weakened immune system. Some cases of histoplasmosis will occur years after the patient was initially exposed.

Histoplasma infections are treated with a class of antibiotic called an antifungal. The most common antifungal used is an oral medication called itraconazole. In serious infections, an intravenous medication called amphotericin B is utilized. The length of therapy can range from weeks to months depending upon the severity of the disease. In a few cases, surgical removal of an infected portion of the lung is all that is needed and the patient does not require any antifungal therapy.

It is important to realize that while we have a fungus among us, that it seldom results in serious disease and with appropriate precautions we can still enjoy our rich Missouri outdoor heritage.

Dr. Johnson has 14 years experience as a board certified infectious disease specialist. He has been in practice in Columbia since completing his training in 2000. For the past 12 years he served as the Infection Control Committee Chairman at Boone Hospital Center. Dr. Johnson completed medical school and his infectious diseases fellowship at the University of Missouri-Columbia. While in training during 1999 and 2000, he won the Department of Internal Medicine Student Teaching Award “Outstanding Fellow.” He also completed an internal medicine residency at St. John’s Mercy Medical Center in St. Louis. Dr. Johnson enjoys working with patients with complicated infectious disease conditions, especially those with HIV. Dr. Johnson said, “My wife and I are both physicians and we made our home in mid-Missouri over 30 years ago. This is where we raised our children and built our careers and I am very pleased to be able to join Dr. Donald Miller and JCMG so that I can continue to serve my patients and colleagues here at home.”

THE ACADEMY ENCOURAGES YOU to Bite Into a Healthy Lifestyle

There is no one food, drink, pill or machine that is the key to achieving optimal health. A person’s overall daily routine is what is most important. That is why, as part of National Nutrition Month® 2015, the Academy of Nutrition and Dietetics urges everyone to “Bite into a Healthy Lifestyle.”

Each March, the Academy encourages Americans to return to the basics of healthful eating through National Nutrition Month. This year’s theme encourages consumers to adopt a healthy lifestyle that is focused on consuming fewer calories, making informed food choices and getting daily exercise in order to achieve and maintain a healthy weight, reduce the risk of chronic disease and promote overall health.

“A healthy lifestyle—one based on regular physical activity and an eating pattern chosen to meet individual nutrient needs within calorie limits—is vital to maintaining good health and quality of life,” said registered dietitian nutritionist and Academy President Sonja Connor. “This year’s ‘Bite into a Healthy Lifestyle’ theme gives consumers an easy reminder that achieving optimal health is not about a specific food or time of year, but rather a life-long commitment that leaves plenty of time for the foods and activities we love.”

The Academy strives to communicate healthful eating messages that emphasize balancing food and beverages within an individual’s energy needs, rather than focusing on any one

specific food or meal. To this end, it is the Academy’s position that improving overall health requires a lifelong commitment to healthful lifestyle behaviors, emphasizing sustainable and enjoyable eating practices and daily physical activity.

“The key is determining the lifestyle balance that provides the body what it needs, while still allowing room for the foods and activities we enjoy most,” Connor said. “This is where the expertise of registered dietitian nutritionists plays such a key role. RDNs work with consumers to find balance and create sustainable eating and physical activity plans that fit their tastes and routines.”

“Many registered dietitians, like myself, feel empathy for consumers struggling to navigate a potentially confusing maze of nutrition science and health-related messages. Helping people identify what information is helpful for them at any particular point in their overall ‘journey’ toward better health is one of the most enjoyable aspects of working as a registered dietitian for the Weight Management Center at JCMG,” says Lisa Finley, RD, LD, JCMG Weight Treatment Center.

As part of this public education campaign, the Academy’s National Nutrition Month website includes a variety of helpful tips, games, promotional tools and educational resources, all designed to spread the message of good nutrition based on the “Bite into a Healthy Lifestyle” theme.

INTERIOR UPDATES DESIGNED to Enhance Patient ExperienceWhat’s new at JCMG? Well, if we’re talking about the interior of the JCMG Medical Building, the answer is a lot!

Renovations began in June 2014 when the JCMG Building, LLC., met with an interior designer to determine the best plan to maximize the space and provide a high-quality patient environment.

Numerous updates were made throughout the eight month timeframe. Wallpaper, paint and carpet were replaced throughout the patient areas of the building. Each floor took on an independent color scheme, while coordinating with the other two levels.

Carpet has been placed in the patient waiting areas and in the stairwells to provide a cozy and sound cushioned space.

Installation of rubberized tile in the main walking areas, allows for easier cleaning and quieter travel.

Each waiting area has been accommodated with two televisions, all new planters and greenery. New, colorful plants will be switched out every quarter to brighten the area. The

furniture in each waiting area offers patients a choice in seat style, yet comfort throughout.

Reception areas have been adorned with new counter tops, clocks and wait time boards.

Some waiting areas feature Lumicor panels, to create a light and airy divider between spaces.

All public restrooms have been remodeled with the exception of the lower level, which is currently being updated with new tile, wallpaper, sinks, and stalls. A handicap entry button has been added to all of the remodeled restrooms.

All of the new features and updates have truly transformed the building. JCMG has always taken pride in providing a well-kept, modern facility and the remodel has not disappointed.

Phase two of building renovations, including the lobby, are currently planned for 2016. Phase three, which includes the clinical areas, will follow.

What happened to the fish tanks?You may have wondered what happened to the fish tanks that were once in Family Medicine and Internal Medicine. In September, they were donated to the Conservation Heritage Foundation, the non-profit of the Missouri Department of Conservation.

The Conservation Heritage Foundation plans to use them in a new nature center being planned at the Shepherd of the Hills Fish Hatchery near Branson. The hatchery grows about 1.2 million rainbow and brown trout annually.

The new building will include live fish and animal exhibits and auditorium space. The Missouri Department of Conservation hopes to have the nature center opened to the public by 2017.

IDEON Updates Medical Facility with Modular, Modern Seating

ABOUT THE PROJECTJefferson City Medical Group (JCMG) is one of Missouri’s largest physician-owned multi-specialty groups. Launched in 1993 with the merger of two local physician’s offices, it grew from the belief that the best medical decisions are made by patients working closely with their physicians. Today, the facility includes over 90 providers in 25 medical specialties with more than 600 employees. The flagship building was completed in 1998. Since it sees over 425,000 patient visits annually, the facility needed to be updated. The medical group turned to long-time business partner Samco Business Products who brought IDEON in for the solution.

WHAT MADE THE DIFFERENCE?Samco Business Products Marketing Representative Debbie Bond faced a challenge. JCMG needed seating that was durable, modular and easy to clean but also updated the look of the facility. She knew that Aviera had been specially designed with healthcare in mind, and could offer what the medical group needed.

The option of bariatric models with a weight capacity of 750 lbs. coupled with the poly arm caps, clean-out space for easy cleaning and thoughtful design for easy ingress and egress made Aviera a great option for the busy healthcare facility. And Aviera’s modular style – with its start, center and end options – means that it can grow with the facility.

Composium, with its slightly higher seat, was mixed in throughout for a stylish, welcoming atmosphere. Samco found textiles that were the same pattern in different colors for the different floors, using design to help orient patients.

And the seating is there to stay. IDEON’s field-replaceable parts mean that if the seating gets damaged, the medical group won’t have to buy all new chairs.

WHAT’S NEXTThe fresh, new seating has transformed the waiting areas of JCMG. Patients are reporting how comfortable the seating is and how effortlessly they’re able to get out of the chairs. The upholstery and wood finish have lightened the overall look and enhanced the new wall and floor coverings. And as the medical group continues its renovations, including a surgery center, Samco Business Products is looking forward to more projects with JCMG.

JCMG HEARING & BALANCE CENTER According to the Hearing Health Foundation, 48 mil-lion Americans over age 12 have a hearing loss, and this number is expected to double by 2030. “Hearing loss makes it difficult for the hearing-impaired and their loved ones; it really degrades their quality of life,” says Jaime Hampshire, Au.D. “Even a mild, 25 to 40 dBHL, high-frequency hearing loss can cause a patient to miss 20 to 30 percent of vital speech information,” says Amy Fleming, Au.D., CCC/A, F/AAA. This creates difficulties with emotional turmoil, social rejection, as well as, possible depres-sion and anxiety over hearing loss and the resulting impact on com-munication with others. Recent research has even linked hearing loss as a contributing factor to dementia and Alzheimer’s.

Studies show that patients wait, on average, five to seven years to seek treatment. JCMG Hearing and Balance Center wants to be able to help patients much earlier in the process by detecting individu-als that exhibit the consequences of untreated hearing loss. The audiologists at JCMG have a genuine interest in total patient care and provide quality professional services for patients that exhibit hearing and balance disorders. They take a laid-back, step-by-step approach to assisting patients and their families.

Dr. Fleming and Dr. Hampshire have practiced clinical audiol-ogy for a combined 21 years. Their goal has always been to improve their patient’s hearing and quality of life. “Hearing loss makes the patient trying to communicate work twice as hard just to get the correct information in order to interpret and process what is being said,” says Dr. Fleming. Audiological rehabilitation for hearing loss helps to make the patient’s communication much more effortless. Technology available today in prosthetics that are fit for the hearing-impaired have enabled many to achieve life-changing success. Due to advanced digital technology, as well as advanced audiological procedures, rehabilitation outcomes are more successful. EXERCISE...IT ISN’T JUST FOR KIDS!We all know that exercise and physical activity are beneficial to our physical and mental well-being. Many people incorporate exercise into their regular routine, while some people are hoping to get started soon. You may be one of them.

Exercise can be easy to do. All you need is self-motivation and comfortable clothes and shoes. Many communities now have fitness facilities or gyms with reasonable membership fees. These gyms offer a variety of equipment; such as treadmills, stationary bikes, ellipticals and weight machines. There are several advantages to using these facilities. People can exercise in all types of weather. They can develop social relationships with other members. Gyms have someone available to provide instructions on how to use the equipment. The staff can also offer suggestions regarding stretching and exercises that match the person’s capabilities. Some memberships provide opportunities to go swimming or participate in water aerobics, which allows exercise that is easier on the joints.

Of course there are several other places available for indoor walking that are just as beneficial and are free. Wal-Mart, Target

and the Capital Mall are a few examples. Other great places include the Runge Nature Center, Katy Trail and the Greenway. They provide wonderful opportunities to exercise while enjoying the perks of being outdoors and getting fresh air.

Did you know that exercise produces certain hormones that make us feel better both physically and mentally? In addition to healthy eating and getting adequate sleep, regular exercise helps to keep those numbers down that your medical provider is always interested in…blood pressure, blood sugars and weight. Participating in weight-bearing exercise also helps to keep bones strong while keeping our heart healthy. There are many perks for regular exercise.

It just takes 30 to 60 minutes three to four times per week to realize these positive benefits. If you are not currently exercising, be sure to get an “okay” from your physician, nurse practitioner or physician assistant before starting an exercise program. Slow and easy is the best way to start. You can gradually increase the amount of time and effort you put in to it.

You will be glad you made this change in your life and will feel much better for it.

This is your opportunity to show off those cute pictures of your family and at the same time show support of the March of Dimes. For only a $1 donation, your child’s and/or grandchild’s (of any age) photo will appear on the large screen in the JCMG Lobby during the month of April. Dr. J.P. McAleer, POD, and Dr. Afsheen Patel, FM, attended the White Coat Lobby Day at the Capitol. Advocacy helps to ensure patients continue to get access to medical care.

JCMG recently welcomed the following new employees (L to R): Kirstyn Roush, PEDS; Cheryl Fisk, ENT; Mary Foster, HR.

JCMG staff & physicians showed their support of the American Heart Association’s “Go Red for Women” Day to raise awareness of heart disease in women. Thanks to everyone who participated!

JCMG’s Employee Involvement Committee (EIC) served King Cake in celebration of Fat Tuesday.

The JCMG March of Dimes team was busy filling orders for chocolate covered strawberries during their Valentine’s fundraiser.

Longfellow’s Garden Center placed new greenery and plants in the new waiting room planters. New plants and restroom updates are part of the final stages of renovations at the JCMG Medical Building to better accommodate our patients & visitors. Daylight Saving—Great Time to Reset Your Sleep HabitsFROM THE NATIONAL SLEEP FOUNDATION

Daylight saving time, Sunday, March 8, is a great time to reset your sleep habits, as well as your clock. If you have trouble sleeping, try the following sleep tips:

• Go to sleep and wake at the same time every day, and avoid spending more time in bed than needed.

• Use bright light to help manage your “body clock.” Avoid bright light in the evening and expose yourself to sunlight in the morning.

• Use your bedroom only for sleep to strengthen the association between your bed and sleep. It may help to remove work materials, computers and televisions from your bedroom.

• Select a relaxing bedtime ritual, like a warm bath or listening to calming music.

• Create a sleep environment that is quiet, dark and cool with a comfortable mattress and pillows.

• Save your worries for the daytime. If concerns come to mind, write them in a “worry book” so you can address those issues the next day.

• If you can’t sleep, go into another room and do something relaxing until you feel tired.

• Exercise regularly ... at any time of the day that feels right for you.
