January 15, 2010 The information contained in this report summarizes new, interim, and final actions taken by the Oregon Medical Board between December 16, 2009 and January 15, 2010. Scanned copies of Interim Stipulated Orders, Orders of Emergency Suspension, Stipulated Orders, Final Orders, Termination Orders, Modification Orders and Voluntary Limitations are included at the end of this report in the order that they appear in the report. These orders are marked with an * asterisk. Scanned copies of Corrective Action Agreements are not posted as they are not disciplinary action and impose no practice limitations. Complaint and Notices of Proposed Disciplinary Action are not listed in this report as they are not final actions by the Board. This Order terminated Licensee's April 9, 2009 Corrective Action Order. *BRISTOL, Jyoji, Takata, MD; Applicant; Cheshire, CT The Board issued a Default Final Order on January 15, 2010. This Order denied the Applicant's application for licensure in Oregon and imposed a $10,000 fine. *CORIA-CAREW, Micaela, Ruth, LAc; AC00561; Portland, OR Licensee entered into a Stipulated Order with the Board on January 14, 2010. In this Order Licensee agreed to a reprimand, $5,000 fine, medical ethics course, and ensure that she practice under the same last name for both her acupuncture and naturopathic licenses. *LAI, Wallace, MD; MD17813; Beaverton, OR Licensee entered into a Stipulated Order with the Board on January 14, 2010. In this Order Licensee agreed to a reprimand; 90-day suspension which is stayed provided the Licensee fully complies with the terms of the Order; 10-year probation with quarterly reports to the Board; $10,000 fine; professionals boundary course; and

*MOCKOVAK, Michael, Emeric, MD; MD26938; Renton, WA Licensee entered into a Stipulated Order with the Board on January 14, 2010. In this Order Licensee agreed to surrender his Oregon medical license while under investigation and agreed to never reapply for an Oregon license. *MOREY, Peter, Samuel, MD; MD24236; Portland, OR The Board issued an Order of Emergency Suspension on January 6, 2010. This Order suspends Licensee's Oregon medical licensee effective at 6:00 p.m. on January 6, 2010. *PHIPPS, Richard, Boyd, MD; MD11864; Gresham, OR The Board issued an Order Terminating Final Order on January 14, 2010. This Order terminated Licensee's April 16, 1999 Final Order. *PURDY, Catherine, Ann, DPM; DP00162; Salem, OR The Board issued an Order Terminating Voluntary Limitation on January 14, 2010. This Order terminates Licensee's January 14, 1993 Voluntary Limitation. *READ, Ralph, Lewis, MD; MD22066; Portland, OR The Board issued a Final Order on January 14, 2010. This Order revoked Licensee's Oregon medical license; fined him $10,000; and assessed him the costs of the disciplinary proceeding. *WAN, William, Kang, LAc; AC00919; Portland, OR Licensee entered into a Stipulated Order with the Board on January 14, 2010.
