Manual of Gastroenterology. 2nd ed. ByG L Eastwood, C Avunduk. (Pp 432;illustrated; £22.95.) Boston: Little, Brown,1994. ISBN 0 316 19992 3.

The arrival of a request to review a book canbe a mixed blessing. Nice, that someone hasremembered that you are still around, butnasty, that you will have to find the time toread the book, and critically. That the bookunder review turned out to be a manual was adefinite blessing, as such a book is notdesigned to be read cover to cover, but to bedipped into. We decided to use it for eightweeks and refer to it as clinical problemsarose.

This turned out to be a big mistake as thisapproach led to a well thumbed chapter onirritable bowel syndrome and constipation.Berkshire bowel has yet to be written up as aclinical syndrome, but meant we didn't get achance to read anything on abetalipopro-teinaemia. So it was back to the cover to coverapproach.The one important draw back is that the

manual is written by Americans and forAmericans and thus the abbreviations can gettedious at times. There are also some varia-tions in treatment, such as using mineral oilfor the treatment of constipation and usingGolytely to treat hepatic encephalopathy. Ifyou grit your teeth this is a draw back worthovercoming as this is an excellent manual atan excellent price.The format works well, being spiral bound

with reasonably lightweight paper but shouldstand up to being tossed into a briefcase, butwon't stand up to being used to prop up thebed head if you get gastro-oesophageal reflux.We advise a multivolume text book of gastro-enterology for the latter.

It is absolutely crammed full of informa-tion, which is easy to assimilate, concise butnot too brief, and amazingly full of commonsense. In the section on peptic ulcer and dietthe authors comment that 'some patients feeltheir treatment is inadequate unless a strictdiet is prescribed. These patients require askilful synthesis of the art and science ofmed-icine'. Obviously a manual written by doctorswho understand their patients and also listento them. There is also a section on the chang-ing expectations of doctors and patients overthe past 50 years, with respect to consulta-tions, diagnosis, and treatment. This makesinteresting and thought provoking reading, inview of the increasing number of complaintsthat are received by all doctors at present. Wethink that the next edition should include achapter on how to deal with complaints!We whole heartedly recommend this man-

ual to those in training in gastroenterology, atall levels, and it will also be an asset to anyward based library. It is written in a refresh-ingly unstuffy style and is amazingly cheap.May the next book that comes our way be asuseful.

Patient information publications are centralto the treatment of chronic conditions. Thishas long been recognised in the field of dia-betes and is now increasingly acknowledgedin gastroenterology. During the past decadethere has been a proliferation of such book-lets, particularly about inflammatory bowel

disease. In more recent times leaflets havealso appeared on upper gastrointestinal disor-ders. The success of booklets published by theBritish Digestive Foundation has confirmedthe need for this type of publication.

Initial anxieties about the effect of suchinformation on patient anxiety levels andincreased consultation rates have not beenconfirmed. Indeed, it seems probable thatbetter educated patients make more informeduse of medical resources. Indeed, the wholedirection of clinical care is now towards betterinformed patients. The concept that 'doctorknows best' is no longer valid. Indeed,perhaps the patient knows best.

Against this background you would expectthat a booklet such as Indigestion to be of par-ticular value in the treatment of patients withdyspepsia and gall stones. This booklet wasspecifically prepared for an American reader-ship and has been described by Dr LesterColeman as a 'brilliant and concise treasuretrove of information'. I would have to say thatit is an excellent summary of our currentunderstanding of both gastroduodenal, bil-iary, and pancreatic disorders. For medicalstudents it provides a quick and easily readreview. I seriously doubt, however, whether alarge number of British patients would findthe text one they could understand, or indeedone they could afford. The text is well writtenand fluent. It is clearly on a par with the typeof information found in quality newspapers.Unfortunately most of my patients readtabloids rather than broadsheets so it is quiteclear that effective patient information mustbe written in precise but simple and shortsentences. Readability of patient informationis critical to its success and it does not corre-late with well constructed long sentences. Icannot believe that the average patient willgain greatly from reading this text

ABIM announcement regarding boardeligibility

Gut publishes original papers, short rapid communica-tions, leading articles, and reviews concerned with allaspects of the scientific basis of diseases of the alimen-tary tract, liver, and pancreas. Case reports will only beaccepted if of exceptional merit. Letters related toarticles published in Gut or with topics of general pro-fessional interest are welcomed. Authors should includethe names and addresses of four experts whom theauthors consider suitable to peer review their work.

COMMUNICATIONS Two copies of the manuscript andfigures should be addressed to the Editor, Gut, BMAHouse, Tavistock Square, London WC1H 9JR, UK.Manuscripts should follow the Vancouver conventions(see BMJ 1979; i: 532-5. Gut 1979; 20: 651-2). Theyshould be in double spaced typewriting on one side ofthe paper only. The title page should include the nameof the author with initials or distinguishing first nameonly, and the name and address of the hospital or labo-ratory where the work was performed. The papermust include a precise summary of the work ofless than 200 words, which should be divided intosix sections headed background, aims, patients(or subjects), methods, results, and conclusions.Keywords (maximum six) should be included. Useof abbreviation is discouraged. Short rapid communica-tions should not be more than 10 double spaced A4pages including references, tables, and figures. Thesepapers will be subject to peer review in the normal way.The interval from acceptance to publication will bemuch shorter. A covering letter should include arequest for the paper to be considered in this categorywith valid reasons for that request. A separate coveringletter signed by all authors must state that the data havenot been published elsewhere in whole or in part andthat all authors agree to publication in Gut. Previouspublication in abstract form must be disclosed in a foot-note. Papers must not be published elsewhere withoutprior permission of the Editorial Committee. Ifrequested, authors shall produce the data upon whichthe manuscript is based for examination by the editor.

ACKNOWLEDGEMENT OF MANUSCRIPTS Manuscripts willonly be acknowledged if an addressed postcard isenclosed.

ILLUSTRATIONS Photographs Unmounted photographson glossy paper should be provided. Illustrations shouldnot be inserted in the text but marked on the back withthe figure numbers, title of paper and name of author.All photographs, graphs, diagrams should be referred toas figures and should be numbered consecutively in thetext in Arabic numerals. The legends for illustrationsshould be typed on a separate sheet.

ETHICS Ethical aspects will be considered in the assess-ment ofpapers (see the Medical Research Council's pub-lications on the ethics of human experimentation, andthe World Medical Association's code of ethics, knownas the Declaration of Helsinki (see BM3t 1964; ii: 177)).

SI UNITS All measurements except blood pressure areexpressed in SI units. In tables and illustrations values

are given in SI units. For general guidance on theInternational System of Units and some useful conver-sion factors, see The SI for Health Professions (WHO,1977). NB: Such conversion is the responsibility ofthe author.
