You are enrolled in a Delta Dental PPO plus Premier plan. You and your family members may visit any licensed provider, but you will receive the greatest out-of-pocket savings if you see a Delta Dental PPO provider. PPO Provider – Payment is based on the PPO provider’s allowable fee, or the actual fee charged, whichever is less. Premier Provider – Payment is based on the Premier Maximum Plan Allowance (MPA), or the fee actually charged, whichever is less. Delta Dental PPOSM plus Premier

With the Delta Dental PPO plus Premier plan, you and your family members may visit any licensed dentist, but you will receive the greatest out-of-pocket savings if you see a Delta Dental PPO provider. Participating providers file claims directly with Delta Dental and accept Delta Dental’s reimbursement in full. You are responsible only for your deductible and coinsurance (as determined by your plan), as well as any charges for non-covered services up to Delta Dental’s approved amount. If you choose to see an out-of-network provider, you will incur additional out-of-pocket expenses, and you will be billed the total amount the provider charges (called balance-billing). When you see a Delta Dental PPO or Premier® provider, you are protected from balance-billing.

Advantages of the Delta Dental PPO plus Premier plan:

SAVINGS: Delta Dental PPO providers offer subscribers the greatest savings. And, in some areas, you will still save money if you need a service that is not covered. Non-covered services will be billed at a discounted rate if you go to a PPO provider.

CHOICE: If you choose to visit a Premier provider, you will still save money because Premier providers also accept discounted fees (however, discounts are not as great as if you see a PPO provider).

NETWORK: Delta Dental’s dual network has nearly 103,000 PPO providers and 155,000 Premier providers nationwide.

If you choose a PPO Only dental plan, you must see a PPO provider in order to receive benefits.* If you receive treatment from a non-PPO provider, you will be responsible for all fees charged. Of course, with more than 2,200 PPO providers practicing across the state, you have many choices.

When looking for a provider using our Find a Dentist online tool, limit your search to PPO providers.

PPO dentists submit claim forms directly to Delta Dental of Colorado. With this PPO Only plan, you are responsible for only a portion of the cost of each procedure. You can determine your coinsurance by looking at your explanation of benefits or by contacting our customer service department using the information below. You will be billed only for your portion of the cost after each procedure instead of having to be reimbursed.

Remember, it makes sense to find out how much your portion of expensive procedures will be, so ask your provider to submit a pre-treatment estimate. Delta Dental will review the treatment plan and tell you exactly how much you are responsible for. This way, you will have a clear understanding of your cost before you decide to proceed with the treatment.

*Please note that if you are in the middle of orthodontic treatment and your provider is not in the Delta Dental PPO network, your treatment will not be covered under the PPO Only plan.

TIP: You can choose to make your ID card available on your device without logging in.

FIND A DENTISTSearch for providers who are in your area and have the qualities that matter most to you. Find an office close to work or home, and filter by gender, language spoken, or accessibility features. Once you’ve found a provider who fits your needs, save his or her information to your contacts, call to schedule a visit, or navigate directly to their office with the touch of your finger.

TIP: Our tool defaults to your plan network (when logged in) and General Dentist to make searching for an in-network provider simple. You can also choose to use your device’s GPS to determine your location or search using your provider’s last name to see if he or she is in our network.

MY COVERAGE AND CLAIMSSimply click My Coverage or My Claims within the top left menu bar to check your coverage information or see claims status. Within the coverage section, you can review your plan type, benefit levels, and contact information for your Delta Dental company, as well as details on your deductibles and maximums. Within the claims section, you can check the status of your most recent dental claims. Click on a specific claim to view details of that visit with the option to email the claim information.
