The International Society for Heart and LungTransplantation (ISHLT) invites you to exhibitat our next meeting, the ISHLT 32nd AnnualMeeting & Scientific Sessions, April 18-21,2012 in Prague, Czech Republic at the PragueCongress Centre.

The ISHLT membership is comprised of over2600 members from over 45 countries, repre-senting more than 15 different disciplines involvedin the management and treatment of end-stageheart and lung disease; our members are con-sumers of a wide variety of medical and phar-maceutical supplies, devices and services. Ourmeeting offers exhibitors direct contact withmedical professionals who make or heavily influence decisions regarding the purchase ofproducts and services provided by businessessuch as yours.

The ISHLT meeting will feature the latest researchresults through Featured Abstracts, SatelliteSymposia, Concurrent Abstract Sessions, PosterSessions and Invited Lectures presented by scien- tific leaders. Exhibits will be open the evening of Wednesday, April 18 (in conjunction withthe Exhibit Hall Opening Reception),and all day on Thursday, April 19 andFriday, April 20.

PURPOSE OF THE EXHIBITIONThe exhibits portion of the meeting is designed to update the healthcare professional’s knowledge of products, services, literature andequipment pertinent to the fields of heart and lung transplantation,advanced heart and lung failure, transplant related infectious diseases,pulmonary hypertension and related diseases and therapies. Thereis no better or more direct method of reaching these key contactsthan the ISHLT Annual Meeting and Scientific Sessions.

MEETING LOCATIONThe ISHLT 32nd Annual Meeting andScientific Sessions will be held at thePrague Congress Centre, 5 Kvetna 65,140 21 Prague, 4, Czech Republic. TheCongress Centre is located on the sub-way Red Line and is adjacent to twolarge hotels where ISHLT has blockedsleeping rooms. It is a 30 minute walkfrom the City Centre. Situated at thetop of one of Prague’s nine hills, it offersstunning 360 degree views of Prague.

EXHIBIT LOCATIONThe exhibits will be located in the Con-gress Hall foyer on the 2nd floor of the Prague Congress Centre. The final Exhibit Hall floor plan will be designedto accommodate actual exhibitor spacerequirements.

HOTEL RESERVATIONSISHLT has arranged for a large blockof sleeping rooms at the Holiday InnPrague Congress Centre, SheratonPrague Charles Square Hotel, HotelYasmin, Radisson BluAlcron Hotel, Intercontinental Praha, CorinthiaHotel Prague, Hotel Palace Praha andPrague Imperial Hotel. You must makeyour hotel reservations using the Offi-cial ISHLT Housing service, Czech-In,which can be accessed from the ISHLTwebsite (www.ishlt.org). Additional information regarding hotel accom-modations can be found there as well.Companies wishing to secure a blockof 10 or more hotel rooms may do soby signing a sub-block contract withISHLT. WHAT’S INCLUDED IN THE EXHIBIT FEE?Booth size is 2.5 x 3 meters minimum.Island booth prices include the floorspace only. Non-island booth prices include one 2.5x3 meter shell schemeexhibit booth, an identification signlisting the company name as ordered,exhibit booth carpeting and unlimitedexhibit booth personnel registrationbadges. Individuals registered as exhibitbooth personnel must be employed bythe exhibitor and may be required toprovide proof of such. The exhibit boothpersonnel badge allows access to theexhibit area only. Individuals desiring

to attend any of the scientific sessionsmust register for the meeting at theregular registration rate.

ASSIGNMENT OF SPACEApplications for exhibit space must bemade on the Official Exhibit ApplicationContract. Initial assignment of spacewill be made on January 23, 2012 andwill be based on total support for theSociety, prior participation in Societyexhibitions, date of receipt of application,booth size requirements and locationof space requested. Assignment of spaceto exhibitors applying after January 23,2012 will be made in order of the dateof receipt of the application. Exhibitorswishing to avoid assignment of spaceadjacent to that of a particular com-petitor should note that on their appli-cations. Careful consideration will begiven to all such requests, but accom-modation of such requests is not guar-anteed and can affect booth assignment.

SECURITYA security guard will be furnished dur-ing all hours that the exhibit hall isclosed. However, the safekeeping of theexhibitor’s property is the responsibil-ity of the exhibitor.

OFFICIAL EXHIBIT SERVICECONTRACTOR ISHLT has selected Czech-IN as the official Exhibit Service Contractor. Theywill furnish all Exhibitors with an Ex-hibitor Information and Service Manualin late January 2012. The Manual willcontain order forms for labor, boothfurnishings, accessories, spotlights, signs,audio-visual, electrical, telephone, internet,shipping, etc. Orders must be receivedby March 1 in order to receive discountedpricing. Czech-In will staff an Exhibitor’sService Center during all move-in,move-out and show hours.

The exhibit booth application,

properly executed by Exhibitor,

shall, upon written acceptance and

notification of space assigned by

ISHLT, constitute a valid and bind-

ing contract. The following rules

and regulations have been designed

for the benefit of all exhibitors.

PAYMENT AND CANCELLATIONSWhen applying for space, the Exhibitormust submit payment in full with theapplication. No space assignment willbe made prior to receipt by ISHLT ofpayment in full. This contract may becancelled by the Exhibitor by writtennotice to ISHLT. Refunds will be madefor cancellations as follows:

Prior to Jan 1: 100% of booth cost

Jan 1 to Jan 31: 75% of booth cost

Feb 1 to Feb 28: 50% of booth cost

On or After March 1: No refund

All refunds will be processed after themeeting.

BOOTH CONSTRUCTION AND DIMENSIONS The exhibition area has natural daylight(windows facing north). A tenta tivefloor plan is provided in this booklet.

Booth size is 2.5 x 3 meters minimum.Island booth prices include the floorspace only. Non-island booth prices include one 2.5x3 meter shell schemeexhibit booth, an identification sign

listing the company name as ordered,exhibit booth carpeting and unlimitedexhibit booth personnel registrationbadges. All furnishings, equipment,services, electrical, etc., required byeach Exhibitor shall be at his/her ownexpense and responsibility, and may be ordered through the official ExhibitService Contractor. Exhibits must conform to the following standards: no sidewalls or counters may exceed 1 meter in height; Exhibitors mustarrange exhibits so as not to obstructthe general view or hide other exhibits;no partitions other than the back andside dividers provided by the ExhibitService Contractor will be allowed unless specifically approved by ISHLT;backgrounds are limited to 2.5 metersin height; obstructions more than 1meter tall may extend no more than 1 meter from the back line of the booth.Signage must be part of the exhibitbooth structure.

The maximum building height is 2.50meters. However, some ceiling heightsvary. Please contact Czech-IN for meas-urements in a specific location. The di-mensions of the pillars are approximately45 x 45 cm.The floor is covered withwhite/light grey marble. Your exhibitionspace will be carpeted. The floor is veryfragile, so exhibitors will be held respon-sible for damages to the floor. The floorbearing capacity (net load) is 400 kg/sq. m. Power supplies and other utili-ties will be supplied into your standvia the ceiling or via the floor. Sus-pension of banners/signage from theceiling is not allowed. A ramp must beprovided for access for the handicappedfor floors of 5 cm or more in height. Itis strictly forbidden to store anythingbehind your stand. Stands must face

the exhibit aisles. If using pop-up/um-brella stands, cabling should be prop-erly covered (e.g. by renting curtains,wall, plants…) at your own cost. Standdesign plans (PDF format or paper copies)need to be sent to Exhibition Agent/Czech-IN not later than March 1, 2012.

IMPORTANT: Unused literature andother waste must be removed from exhibition area by the end of the meet-ing. In case of non-compliance, an additional cost will be charged to theexhibitor for removal. A detailed sketchof your stand location with exact dimen-sions and locations of pillars can be requested from Czech-IN. For exhibitionregulations, please see further details inthe exhibitor manual.

ISLAND DISPLAYSIsland booths must be constructed sothat there is at least 50% visibility ofthe exhibition hall from any one side.The following restrictions are waivedfor island displays: 2.5 meter height re-striction, 1 meter sidewall restriction.Island booths that are 6 meter x 6 meterand larger must submit a floor plan toISHLT by March 1, 2012 for approval.

INSTALLATION AND DISMANTLING OF EXHIBITSExhibitors must install all exhibits anddisplay materials by the opening hourof the exhibition. Under no circum-stances will the delivery or removal ofany portion of an exhibit be permittedduring the exhibition without writtenpermission from ISHLT. Under no circumstances may dismantling occurbefore 5:00 pm, Friday, April 20, 2012.Exhibitors will have until 10:00 am onSaturday, April 21, 2012 to remove theirexhibits from the facility.

SUBLETTING OF SPACEThe Exhibitor agrees not to assign, sub-let or apportion the whole or any partof the space without the knowledge andwritten consent of the ISHLT.

USE OF BOOTHS/LITERATUREDISTRIBUTIONThe Exhibitor is permitted to demonstrateequipment and to make informationalpresentations regarding products orservices in his/her booth only. Alldemonstrations and exhibits must beconfined to the exhibit booths respon-sible for such demonstrations or exhibits.All literature and promotional materi-als may be distributed only within thebooth space assigned.

HANDOUTS AND/OR GIVEAWAYSNo bags of any kind may be distributedfrom any exhibitor’s booth. Distributionof descriptive product literature, note -pads, pens and pencils is permitted.Other items may be distributed from thebooth only if approved in writing byISHLT. Requests for such approval mustbe received in writing by ISHLT no laterthan March 1, 2012. No unapproveditems may be distributed. Any exhibitorfound distributing unapproved itemswill be required to cease distribution.Exhibition Handout/Giveaway ApprovalForms can be obtained from the ISHLT

web site (www.ishlt.org) under the Annual Meeting Exhibition section.

SALE OF GOODS IN EXHIBIT BOOTHSExhibitors shall not conduct or permitany sale of goods or services of any kindin the exhibit or Annual meeting area.Order taking is permitted.

EXHIBIT HALL STAFFING AND ACCESSExhibits must be staffed at all timesduring exhibit hours. Exhibit personnelwill be permitted on the exhibit floorone hour prior to opening and may remain in the hall one half-hour afterclosing. ISHLT will have sole controlover all admissions of persons to theMeeting and the exhibit area.

OBJECTIONABLE MATERIALAND ACTIVITIESAll booth personnel, models anddemonstrators must be registered asExhibitors. Except for a Society sched-uled event, no alcoholic beverages arepermitted in booths or elsewhere inthe Exhibit Area. ISHLT reserves theright to request modification of anyquestionable exhibit or activity and torefuse to permit distribution of sou-venirs, advertising matter or anythingelse which it may consider objection-able. Contests, games of chance, lotter-ies and raffles are strictly prohibited.

REJECTIONS AND PENALTIESISHLT reserves the right to restrict, reject, prohibit or eject any exhibit orExhibitor which, because of noise,safety hazards or for other prudent

reasons, becomes objectionable or forbreach of any of these rules and regula-tions. Such termination may becomeeffective during the meeting, at whichtime the Exhibitor agrees to close his/her exhibit upon receipt of a notice oftermination, and thereafter remove his/her exhibit from the exhibition hall assoon as possible without disruption ofthe meeting. Expulsion of an Exhibitorshall not give rise to a claim for a refundof the fees paid by such Exhibitor.

ASSIGNMENT OF SPACEAssignment of space shall constitutean acceptance of Exhibitor’s offer tooccupy space.

LIABILITIESThe Exhibitor, by signing this contract,agrees to hold harmless and indemnifythe International Society for Heart andLung Transplantation, the Prague Con-gress Centre, Czech-IN and their respec-tive employees, representatives, agents,successors and assigns, for any and alldamages to its property and for any andall injuries to any person resulting fromits exhibiting at this Meeting. Exhibitorswill be held responsible for any damagedone to the buildings by it or its employ-ees. No nails, tacks or screws should bedriven into the floor, walls or woodworkof the building.

The International Society for Heart &Lung Transplantation, its agents and employees, will not be liable for failureto hold the Meeting as scheduled. Pay-ments for the booth space will be re-turned in the event of cancellation of the

meeting, except that any actual expensesincurred or committed in connectionwith the Meeting will be prorated anddeducted if the Meeting is cancelled be-cause of fire, an act of God, the publicenemy, strike, war, epidemic or any lawor regulation of public authority whichmakes it imossible, or impractical, or illegal to hold the Meeting.

FIRE AND SAFETY REGULATIONSExhibitor agrees to conform to all fire,safety, and other rules and regulationsas provided and interpreted by the offi-cial Exhibit Service Contractor and thePrague Congress Centre representatives.

All exhibit decorations must be fire retardant. No open flames or tanks of gas may be used, nor may canopies beerected over exhibits. Exhibitor agreesto take necessary measures to safeguardvisitors from any hazards associatedwith its exhibit.

EXHIBIT LABORAll work involved in the erection,touch-up painting, dismantling andrepair of all exhibits may fall withinthe guidelines of union jurisdictionand require Union labor to performsuch work. This work is to include anyuse of power or battery operated tools,hammers, ladders or saws; wall cover-ings, floor coverings, pipe and drape,painting, hanging of signs and/or decorative materials from the ceiling,placement of all signs and the erectionof platforms used for exhibit purposes.

FREIGHT HANDLING, DELIVERY AND SHIPMENT OF MATERIALSExhibitors agree to ship and store theirmaterials at their own risk and expense.Shipping, storage and delivery arrange-ments may be made with the officialexhibit services contractor, Czech-IN.Complete shipping information will be available in the Exhibitor ServicesManual.

INDEPENDENT CONTRACTORSExhibitors who plan to use a servicecontractor other than Czech-IN mustnotify ISHLT in writing no later thanMarch 1, 2012. An Independent Exhibit

Contractor form can be obtained fromthe ISHLT web site (www.ishlt.org) underthe Annual Meeting Exhibits section.Such contractors are required to com-ply with all rules and regulations of theshow, including the local labor rules andregulations. Such contractors must sup-ply a Certificate of Insurance and formto ISHLT no later than March 1, 2012.

EXHIBITOR PERSONNEL REGISTRATIONISHLT registration badges must be wornat all times during meeting/show hours.An exhibitor personnel registration formwill be provided to exhibiting compa-nies in February 2012. All individualsaffiliated with an exhibit booth must beregistered as exhibit personnel. Indi-viduals registered as exhibit personnelmust be employed by the exhibitor.The exhibit personnel badge allows accessto the exhibit area only. Individuals

desiring to attend any of the scientificsessions must register for the meeting atthe regular registration rate. Badgesmust be picked up on-site at the ExhibitorRegistration Desk during publishedregistration hours. If desired, Exhibitorsmay arrange to have ONE representa-tive from the company collect ALL ofthe exhibit personnel badges for thecompany. Please indicate this on theExhibit Personnel Registration Form.

MUSIC LICENSINGThe Exhibitor shall be responsible forsecuring all necessary licenses or con-sents for and paying all royalties andfees for any uses of copyrighted worksor intellectual property owned by athird party and used, directly or indi-rectly, by the Exhibitor.

CORPORATE EVENTSA Corporate Event is any activity (social,education, marketing, etc.) which in-volves any physician, surgeon or alliedhealth delegates attending the ISHLTAnnual Meeting and which is funded,organized or offered by any for-profitcommercial organization and held inthe Prague area between Tuesday, April17 and Saturday April 21, 2012. Inves-tigator Meetings for registered clinicaltrials are not considered CorporateEvents, nor are hospitality suites andactivities which involve only companyemployees.

Companies wishing to conduct Corpo-rate Events must submit an applicationfor such to ISHLT and receive approval.Upon receiving approval, companiesmay contact their preferred venue tomake arrangements. All costs for cor-porate events are to be borne by thecompany. When space in the PragueCongress Center is used, space may in-clude complimentary use of AV stagingand equipment in place for the ISHLTScientific Sessions.

Corporate Events may be conductedonly during approved times.

Only companies exhibiting at the ISHLT2012 Annual Meeting may conduct Cor-porate Events. If a Corporate Event takesplace without advance approval fromISHLT, the sponsoring organization, thesupporting organization, and the organ-izer of the event will be prohibited fromconducting a Corporate Event at the fol-

lowing year’s meeting and may be barredfrom exhibiting at future ISHLT meetings.

Companies desiring to secure meetingspace for hospitality suites, investigatormeetings for registered clinical trials orevents involving only company employeesmay do so via the Affiliate Event SpaceRequest Form.

An application to request space for a cor-porate event is available at www.ishlt.orgon the Annual Meeting page.

AFFILIATE EVENTS/MEETINGSPACE REQUESTSAffiliate events include:

• All non-ISHLT-sponsored, non-com-pany-sponsored/funded events/meetings/activities conducted by or for delegatesat the ISHLT Annual Meeting and takingplace in the Prague area between and including Tuesday, April 17, 2012 andSaturday, April 21, 2012. Such eventsinclude meetings and events organizedfor university alumni, special interestgroup events, institutional meetings, etc.

• All events/meetings/activities conductedby commercial interests at the ISHLTmeeting and involving only companyemployees/consultants/investigators/board members and taking place in thePrague area between and includingTuesday, April 17, 2012 and Saturday,April 21, 2012. Such events include salesand/or staff meetings, advisory and otherBoard meetings, meetings of investigatorsfor registered clinical trials and Hospi-tality Suites involving invitational, or-ganized activities with designated startand stop times to which physicians areinvited. Educational, marketing, socialand other activities that are conductedby commercial interests and to whichISHLT delegates are invited are notconsidered affiliate events. Companieswishing to conduct such events mustsubmit a Corporate Event RequestForm available on the ISHLT web site(www.ishlt.org).

Application for all affiliate events mustbe made using this form, must be ap-proved by ISHLT, and may take placeonly during the approved times listedbelow. If space is requested at the PragueCongress Centre, the Holiday Inn or theCorinthia Towers Hotel, ISHLT will notify the venues if space release forthe specific event requested has beenauthorized by ISHLT. All subsequentarrangements regarding the event are tobe made by the event organizer directlywith the venue. Requests for corporateaffiliate events will not be approvedunless the company is an exhibitor. Exceptions will be considered forcompanies which do not have an approved product.
