What you need to know: All
The study days in this prospectus are free for Macmillan and non-Macmillan health & social care professionals who care for cancer patients in acute, community and Primary Care settings. All courses are one day unless otherwise stated and the majority will have registration at 09.00hrs and finish at approximately 16.45hrs. Exact times will be sent approximately 2 months before the course starts. For any further information or advice on specific courses, please contact:

Advance Care Planning: Empowerment and Choice in End of Life Care
Nurses & AHP’s on wards, community and Primary Care settings

Key Topics:
• Be aware of key national guidance informing the process of Advance Care Planning including the End of Life Care Strategy (2008), the Mental Capacity Act (2005 and the recent Leadership Alliance for the Care of the Dying People (2014)
• Understand Advance Care Planning as a process and differentiate between advance statements of wishes and advance decisions to refuse treatment (ADRT) with regards to establishing preferences of care (e.g. withdrawal of treatment, ventilation, and rehydration).
• Identify the ‘signposts’ along a patient’s illness trajectory when it might be appropriate to initiate an ACP discussion/care plan and the possible barriers.
• Discuss effective communication skills involved in initiating a discussion regarding Advance Care Planning with a patient and their carer.
