IPACT
IPACT is a website developed by the WV
Division of Vital Registration (a/k/a Division of
Vital Statistics, or DVS) and the WV BCSE.
It provides quick and efficient access to WV
birth, marriage and death indexes, expedites
paternity determinations and permits electronic
requests for birth certificates, paternity affidavits
and death certificates.
IPACT: Limit on Records
IPACT birth and death
records go back more
than 20 years.
When a child turns 22,
or someone died over
20 years ago, their
information may no
longer be accessible.
IPACT: Signing In
The website address for IPACT is

Users:

IPACT: Passwords
Passwords must have 8 to 15 characters
and contain at least:
One upper case letter
One lower case letter
One number
Only One special character (! # $ % ^ & *
or ?)

After you have established a password,
when entering the system again, you will
have 3 attempts to type the correct
password.
After 3 attempts, you will be locked out
and will have to contact your State
Database Administrator
Passwords expire after 60 days of
inactivity.

After 10 minutes of
inactivity, IPACT will
automatically log you
out and you will have
to sign in again to
continue searching.

Click
“Search”
to
search
indexes
Search defaults to birth records. You may also select
Death Indexes or Marriage Indexes, view Order
History, view FAQ or Exit.

IPACT: Searching Hints
If one search criteria fails, try another.
Spelling must be a perfect match, so try
different spellings.
Use as many criteria as possible.
To erase the search criteria and try
again, left click once on the Reset
button.

If a child was born before 1994, or you
do not know the SSN, use the name and
date of birth.
HINT: Try using the mother’s maiden
name for the child’s last name.
When using the mother’s or father’s
SSN, all of the births recorded in WV for
that parent will be listed.
IPACT: Searching for Birth Records
If your search was successful, the
following information appears:
Child’s Information.
SSN (available for children born after
1994.)
The birth certificate number.
The child’s name.
The child’s date of birth.
IPACT: Social Security Numbers
Child’s SSN recorded for births from 1994 forward.
Father’s SSN recorded for births from 1991 forward.
Mother’s SSN recorded for births from 1989 forward.
No match – try another criteria.
Child’s SSN listed as “U” – Social Security was
unable to assign a number – will be updated later.
SSN’s blank – SSN is unknown.

IPACT: Multiple Listing for Same Child
If a child is listed more than once, the
birth certificate may have been
amended, or
For some years, birth records were
entered under the mother’s married
name as well as her maiden name.
Regardless of which birth certificate you
choose to order, DVS will send the
correct one.

Out-of-Wedlock Fields
The DVS started
entering Paternity
Affidavits and
“Born Out-of-
Wedlock”
information in
1994.
IPACT: Paternity Affidavits
The “Paternity Affidavit” (PA) field will be
coded “Yes” if a paternity affidavit was
completed and accepted by Vital Statistics.
Note: If the “PA” field is blank, Vital Statistics did not accept a paternity affidavit.

Out-of-Wedlock Fields
If an affidavit is rejected by DVS, it does not legally
establish paternity.
This may happen if the mother is married to another
man at the time of the child’s birth or if a wrong form
was used.
NOTE: It is very important that you confirm the
paternity affidavit was accepted by DVS (marked Y
in the PA field on the results screen).

IPACT: Out-of-State Births
NOTE: There may be an out-of-state county
and state listed, or the fields may be blank.
This means that a child was born out-of-
state to WV parents and the other state has
notified WV DVS of the birth.
If the birth certificate for such a child is
needed, it cannot be obtained from WV
DVS, the other state will need to be
contacted.

IPACT: Father’s SSN
If a SSN is not listed for the father, it
does NOT mean that there is no father
on the birth certificate.
It could be due to:
The DVS’s large backlog of paternity
orders which still need to be entered, or
The child may have been born prior to
1991.

2. Enter the quantity of BCs and/or
PAs needed. Up to 5 each may be
ordered at a cost of $12 each.

After clicking on Order Now, an order
confirmation screen will appear.
Print a copy for your records.
Your office is billed only for
certificates/affidavits issued.

To check the progress of your order,
click on Order History at the top.

DVS processed and
mailed the requested
documents.
