To receive your Trend Micro Internet Security rebate, please submit within 30 days after purchase:

1. This completed rebate form. 2. Proof of product purchase from Internet Security. Hand-written or altered documents will not be accepted.

Please provide: (A) Valid* UPC code (cut out from bottom of package), AND (B) A copy of your confirmation email OR dated invoice/sales receipt with a purchase price of $25.00 or greater. * A valid UPC begins with the numbers 733199, all other UPC do not qualify.

Terms and Conditions: This Trend Micro Internet Security mail-in rebate offer is valid only on purchases made at Fry’s between December 17, 2008 and December 26, 2008. No purchase of Renewal Upgrade will qualify. Rebate request must be postmarked no later than 30 days after purchase date on your proof of purchase.

If not presented in time, the check will be void and the rebate unavailable. This card can be used everywhere Visa debit cards are accepted. The card will expire 12 months from date of issuance. End User must keep copies of all materials sent.
