INTERNATIONAL TRAVEL CONSENT FORM

Instructions

Due to increased security at airports many airlines are becoming increasingly cautious in allowing minor

children to travel without their legal guardians – especially internationally. Below is a sample of a

document that will need to get filled out if you are a minor traveling on an Expedition without, or with

just one, of your parents or guardians. If you are traveling alone BOTH parents/guardians will need to fill

it out. If you are with ONE of your parents/guardians then the NON-TRAVELING parent/guardian will

need to fill it out.

Please use the example below to fill out the form on the next page. Once completed, have it notarized

and make sure you give it to your trip leader before your trip.

I, {NON-TRAVELLING PARENTS/GUARDIANS FIRST LAST NAME}, make oath and state that I am/we are the {MOTHER/FATHER/GUARDIANS} and legal guardian of: Child: {CHILD’S FIRST LAST NAME} ({MALE/FEMALE}), born {CHILD’S BIRTH DATE} in the United States.

1) {CHILD’S FIRST LAST NAME} has my/our consent to travel with {TRAVELLING PARENT/TRIP LEADER FIRST LAST NAME}, ({MOTHER/FATHER/GUARDIAN} of child).

2) This consent is valid for travel starting on or about {START DATE OF TRIP} to {TRAVEL

DESTINATION} and returning on or about {END DATE OF TRIP}.

3) I/we authorize and recommend {TRAVELLING PARENT/TRIP LEADER FIRST LAST NAME}, to make emergency medical decisions on our behalf, in the event that my/our child requires emergency medical treatment at a time or place from which I/we may not be reached.

4) I/we authorize and recommend {TRAVELLING PARENT/TRIP LEADER FIRST LAST NAME}, to make

arrangements and determinations for all modes of travel, including itinerary changes as necessary during this trip.

5) In the absence of {TRAVELLING PARENT/TRIP LEADER FIRST LAST NAME}, should any medical,

travel or personal welfare incident arise, immediate contact shall be made to me/us, {NON-TRAVELLING PARENTS FIRST LAST NAME}. If immediate communication is not possible, these decisions are to be made by Young Life Staff or affiliates who will be traveling with the above mentioned child in {DESTINATION COUNTRY}.
