1. Basic information– Your Ref, Appl/Reg No., Name in register

2. Change details– Checkbox to mark change request– New details (name, address, agent’s address)

3. Address for Service– Always have to fill in (changed or not)

4. Text file attachment– Facilitates input of a long list of numbers (more details followed)

Selection of Country/Territory/Area

• A country/territory/area must be selected (Change of Address)• Some countries/territories/areas do not fall under the list (ISO-3166-1, WIPO’s code)

• Workaround: – Countries/territories/areas not selectable in the form => Paper form– Examples: Channel Islands, Isle of Man

• The list will be posted in FUG

– Further enhancement will be available by April 2006• If such a country is required, leave the country field empty• The request will be passed to slip-road

Rectification & Correction of Error

• Rectification: T6 (Trademarks)• Correction of Error: D2 (Designs), P7 (Patents)• IPD will perform sample-check on the interactively a

pproved D12, P18 and T5 requests

Partial Approval

• Purpose:– An erroneous record will not block the other proper records from being

processed• Example:

– One T5 form– 100 trademarks

• One of them: Removed• Others: In force

– The 99 trademarks will be processed as a whole:• Approved, or• Slip-road
