Instructions for Form 2: Report on Final Examination (Defense) and Thesis Approval All graduate forms must be typed. This form is required to report the results of the master’s final defense and to approve the thesis. Student Information (Top Section) > Name, Student ID, Email: Type legal name, student ID number, and S&T email address > Degree Program: Select the degree program you are seeking > Thesis Title: Type thesis title > Date of Examination: Select date of examination Release Information/Final Research Grades (Middle Section) The academic advisor will complete these sections. Required Signatures/Approvals (Bottom Section) > Advisor and committee members Members must signify their pass/fail decision next to their signature A candidate will be considered to have passed the defense if all, or all but one, of the committee members vote that the candidate pass. > Department chair or designee > Vice Provost of Graduate Education or designee Additional Information > The advisor and committee should not sign and/or forward this form to the Office of Graduate Studies until they are in agreement that the thesis is in its final form. > After the form has been approved by the advisor/committee and the GLO rubrics are completed, collection of the rubrics must be indicated by stamping/initialing in the box at the bottom of the form. > The Form 2 will be signed by the Vice Provost of Graduate Education or designee only after required formatting corrections have been completed and the final copy of the thesis has been accepted by the Office of Graduate Studies. A candidate will be considered to have passed the defense if all, or all but one, of the advisory committee members vote that the candidate pass. Members of the advisory committee must signify their pass/fail decision next to their signature on this form.

By signing and submitting this form to the Office of Graduate Studies, the advisory committee is signifying that they have examined the thesis closely for both content and format, and determined that it meets the requirements for a master’s degree and is worthy of acceptance by the Missouri S&T graduate faculty. If the committee indicates that corrections must be made to the thesis, the candidate must make such corrections and then seek approval of the revised thesis from the committee members and obtain the necessary signatures.

Once this form has been signed and submitted to the Office of Graduate Studies, no other content changes can be made except for changes to the format outlined by the Thesis/Dissertation Specifications.
