INTRODUCCIÓN
INgene q ParaTB Sondas es un ensayo diseñado para poner de manifiesto la presencia de ADN de Mycobacterium avium
subsp. Paratuberculosis (MAP) en muestras biológicas de un modo sencillo y con unos niveles de sensibilidad y
especificidad extremadamente altos. Bajo condiciones óptimas es capaz de detectar 1 copia de MAP por reacción.
El kit posee los reactivos y enzimas necesarios en dos mezclas que una vez unidas en un único envase (Maxter mix) se
encuentran en concentraciones idóneas, y está diseñado con el objeto de que únicamente sea necesaria la adicción del
ADN de la muestra problema, para detectar y cuantificar MAP, amplificándose una región del gen F57 de copia única
específica del patógeno. El ensayo utiliza una sonda Taqman marcada con FAM-BHQ1®. Por otro lado, cada Master Mix,
proporciona un control interno positivo (CIP) con cebadores y sonda Taqman marcada con JOE-BHQ1®, permitiendo
detectar falsos negativos debidos a una inhibición de la PCR. Además, contiene un fluorocromo pasivo de referencia,
ROX, que permite realizar una normalización pasiva de la señal evitando posibles errores en la cuantificación debidos a
diferencias de pipeteo, y que en el caso de utilizar modelos ABI PRISM de PCR a tiempo real de Applied Biosystems, es
necesario para eliminar interferencias entre los datos recogidos de varios canales de emisión.
Para la determinación del nº de copias de MAP y como control positivo de PCR, el kit incluye un control positivo estándar
con un nº de copias conocido de MAP. A partir de este control, puede ser generada la curva estándar que relaciona el nº
de copias del patógeno con el valor del ciclo umbral (Ct, cycle threshold).
El kit ha sido probado en todos los equipos de Applied Biosystem (StepOne, HT7300, HT7500) y LC 480 de Roche. Es
posible su uso en otros equipos que permitan la medida de dos fluoróforos aunque pueden cambiar los valores de Ct,
según el equipo utilizado.

CONSERVACIÓN DEL KIT
Una vez recibido el kit, añadir el contenido del reactivo “A” en el tubo del reactivo “B” y homogeneizar la mezcla (de
aquí en adelante AB). Mantener el kit a -20ºC hasta su utilización. En el caso de que la mezcla de los reactivos A y B no
pudiera realizarse en el momento de recepción del kit, mantener el kit a -20ºC hasta que los reactivos puedan ser
mezclados. Los componentes del kit se deben mantener a -20ºC y son estables durante 1 año desde la fecha de
fabricación (mirar fecha de caducidad en el envase).

NORMAS BÁSICAS PARA EVITAR CONTAMINACIONES
En general deben mantenerse las siguientes precauciones:
Ausencia de DNasas/RNasas en el material fungible a utilizar.
Uso de agua destilada autoclavada (25 min., 120ºC) libre de DNasas/RNasas.
Uso de puntas estériles con filtro.
Buena homogeneización de los componentes del kit una vez descongelados.
Mantener en todo momento las mezclas en hielo.
Repetidos ciclos de congelación y descongelación podrían reducir la sensibilidad de los reactivos y con ello la
sensibilidad del método, por lo que es conveniente dispensar los reactivos en alícuotas de un solo uso.

Para evitar contaminaciones que generen falsos positivos, es importante:
Separar físicamente el control positivo de PCR del resto de reactivos del kit.
La manipulación de las muestras a testar se debe realizar en un espacio físico diferente de donde se realizan los
análisis de productos amplificados.
El control positivo de PCR debe ser añadido en un espacio físico diferente de donde se añade la mezcla y de
donde se manipulan las muestras a testar.
RECOGIDA Y TRANSPORTE DE LAS MUESTRAS
INgene q ParaTB Sondas está indicado para la detección y cuantificación de MAP a partir de diferentes muestras
biológicas, preferiblemente de muestras de heces y cultivos puros.
El transporte al laboratorio se realizará en frío dentro de las primeras 24 horas. Si bien, es posible el almacenamiento de
las muestras a -20ºC hasta su extracción.

EXTRACCIÓN DEL MATERIAL GENÉTICO
Una vez aislado el ADN, se recomienda chequear la calidad y cantidad del ADN aislado realizando una medida de
absorbancia. Se utilizará cualquier procedimiento disponible en el laboratorio con el que se obtenga una buena calidad
de material genético a partir de las muestras. INGENASA puede suministrar un protocolo y buffer de extracción bajo
pedido.
Ingenasa ha realizado este ensayo con los métodos de extracción basados en:
- Método de Chomczynski and Sacchi de extracción fenólica.
- Métodos automáticos con uso de bolas magnéticas (Mag Max TM96 total NA isolation Ref AM1840).
- Métodos de purificación por columna: The QIAamp® Viral RNA Mini Kit (QIAGEN). High pure PCR template
Preparation kit (ROCHE)
Existen varias casas comerciales con productos para extracción que dan una calidad óptima de ácido nucleico, siguiendo
las indicaciones del fabricante. Entre ellas: Life-technologies, ROCHE, Qiagen, Marchery-Nalgene, otras.
PRECAUCIÓN: si la amplificación no es inmediata, conservar el DNA a -20ºC.
AMPLIFICACIÓN DEL MATERIAL GENÉTICO
MATERIALES NECESARIOS
- Hielo picado.
- ADN extraído de las muestras.
- Mezcla A – MANTENER EN HIELO PICADO SIEMPRE QUE ESTÉ FUERA DEL REFRIGERADOR.
- Mezcla B - MANTENER EN HIELO PICADO SIEMPRE QUE ESTÉ FUERA DEL REFRIGERADOR.
- O Mezcla AB- MANTENER EN HIELO PICADO SIEMPRE QUE ESTÉ FUERA DEL REFRIGERADOR.
- Control positivo de amplificación A1 (MAP)
- Agua libre de DNAsas y RNAsas.
Consideraciones previas a la preparación de la PCR
En cada experimento de PCR además de las muestras problema hay que incluir los siguientes controles:
Control negativo de extracción:
La muestra es sustituida por un buffer de elución o agua y es sometido al proceso de extracción de ácidos nucleicos.
Control negativo de PCR:
Incluye todos los reactivos de PCR excepto el ADN, en cuyo lugar se añade agua. Permite comprobar que los reactivos de
PCR no están contaminados.
Control positivo de PCR o Curva estándar en caso de cuantificación:
Las diluciones de la curva estándar, además de permitir cuantificar MAP en las muestras problemas, son un control positivo
de PCR de concentración conocida, cuyo resultado debe de ser positivo. Permite comprobar que los parámetros y
condiciones de PCR elegidos son adecuados y que no se ha producido ningún incidente en la reacción.

Réplicas (opcional):
Para que la cuantificación sea estadísticamente óptima se aconseja realizar 3 réplicas tanto de los puntos de la curva
estándar como de las muestras problema, y poder calcular así la desviación estándar con un nº mínimo de 3 datos.
Preparación de diluciones seriadas de la Curva Estándar para la cuantificación absoluta de MAP (Opcional)
Para la preparación de las diluciones necesarias en la generación de la curva estándard se recomienda el uso de microtubos
de centrifuga “Maximum Recovery” (no suministrados). Utilizar Tris-HCl 10 mM pH 8 como diluyente (suministrado).
Las diluciones seriadas se realizan a partir del Control Positivo Std siguiendo estas indicaciones:
1. Pipetear 90 μl de Tris-HCl 10 mM pH 8 en 7 tubos que se identificarán con nºs correlativos del 1 al 7.
2. Pipetear 10 μl del Control Positivo Std en el tubo identificado con el nº 1.
3. Vórtex.
4. Cambiar de punta y pipetear de nuevo 10 μl de tubo 1 en el tubo 2.
5. Vórtex.
6. Repetir los pasos 4 y 5 hasta completar las diluciones seriadas.
7. Utilizar los tubos 1, 4 y 6 como estándares en la PCR ver tabla:

PROCEDIMIENTO
1. Preparar y marcar convenientemente, tantos tubos para la amplificación como muestras vayan a procesarse,
incluyendo los controles y los puntos de la curva estándar según la tabla previa de curva estándar. Siempre
contar al menos con un control positivo de amplificación, y otro para el control negativo.
2. Sacar las mezclas A y B del congelador. O si ya han sido mezclados en el momento de la recepción del kit sacar la
mezcla AB. Mantener en hielo picado hasta su total descongelación. En caso de no haber sido mezcladas ambas
mezclas en el momento de recepción del kit, agitarlas ligeramente para su correcta homogenización y preparar
la mezcla de amplificación AB volcando el contenido de la mezcla A en B y marcarla como AB. El tubo que
contiene la mezcla, es conveniente que se mantenga en todo momento en hielo picado.
3. Determinar el nº de reacciones que se van a desarrollar.
4. Tomar en un tubo el volumen necesario de AB para el número de muestras que vayan a procesar. Se recomienda
preparar la mezcla en exceso (calcular un 10% adicional de todos los reactivos) a fin de compensar las posibles
pérdidas de volumen durante los pipeteos. Una vez preparada la mezcla homogeneizar correctamente.

5. Tomar en un tubo (n x 15) μl de la Mezcla AB. El volumen por cada una de las muestras será:

6. Una vez homogeneizada la mezcla anterior se dispensan 15 μl por muestra a analizar en n tubos de PCR.
7. Se añaden 10 μl* de ADN de cada muestra problema, 10 μl de los controles y 10 μl de cada una de las diluciones
seriadas de la Curva Estándar. (*Alternativamente se pueden añadir entre 2-10 μl de ADN y el resto de Tris-HCl
pH 8, 10Mm hasta completar un volumen final de muestra de 10 μl.)
8. Mezclar suavemente el contenido de cada tubo y asegurarse de que todo el líquido se encuentra depositado en
el fondo del tubo. Si no fuera así, centrifugar ligeramente hasta conseguirlo.
Siguiendo el manual de instrucciones de uso del termociclador a tiempo real, la reacción se coloca en el
termociclador y se programa con arreglo a las siguientes condiciones:

La lectura de la fluorescencia se realiza en el paso de elongación (señalado en la tabla)* y los canales por los que se

ANÁLISIS E INTERPRETACIÓN DE LOS RESULTADOS
De cada muestra problema analizada se obtienen datos relativos a la fluorescencia proveniente del canal FAM; elegir para
el análisis la selección automática del umbral. En el caso de que esta opción no sea viable, el análisis podría desarrollarse
de forma manual siguiendo las instrucciones de uso del termociclador.
Los posibles resultados se resumen en la tabla:
Canal FAM: Detección del patógeno
Un resultado positivo en la amplificación implica la obtención de una curva de fluorescencia típica con un valor del ciclo
umbral Ct <38.
Serán muestras dudosas aquellas con Ct´s que se encuentren entre 38-40, para estos casos recomendamos:
• Repetir el análisis.
• Secuenciar el producto amplificado.

Canal VIC: Detección del Control Interno (JOE)
Un resultado positivo en la amplificación implica la obtención de una curva de fluorescencia típica con un valor del ciclo
umbral Ct <45. Normalmente los valores que se registran son 30 < Ct < 45.

Caso 1. Si tanto la muestra problema como el control positivo de PCR muestran un resultado positivo, y por el contrario
los controles negativos de extracción y PCR muestran un resultado negativo.
Resultado positivo: La muestra contiene MAP.
Caso 2. Si el control positivo de PCR muestra un resultado positivo y, por el contrario, la muestra problema y los controles
negativos de extracción y PCR muestran un resultado negativo.
Resultado negativo: La muestra no contiene MAP.
Caso 3. Si el control negativo de extracción muestra un resultado positivo.
Resultado invalidado o nulo desde la etapa de extracción.
Caso 4. Si el control negativo de extracción muestra un resultado negativo, sin embargo, el control negativo de PCR
muestra un resultado positivo.
Resultado invalidado o nulo desde la etapa de amplificación.
Análisis cuantitativo (Opcional)
El número de copias de MAP de cada muestra problema puede ser cuantificado al inicio de la reacción con gran precisión
a través del ciclo umbral (Ct). De esta manera, utilizando las muestras patrones que contienen diluciones seriadas de
concentraciones conocidas de MAP, se construye una recta de calibración que relaciona los Ct con el logaritmo de la
cantidad inicial de molde, es decir, con el nº de copias de MAP que contiene la muestra. Extrapolando en esta recta los Ct
obtenidos para las muestras problema, se puede estimar el nº de copias inicial de MAP en las mismas.

La mayor parte de los equipos de PCR a tiempo real que existen actualmente en el mercado poseen software que realiza
automáticamente una cuantificación absoluta si todos los parámetros están correctamente configurados.
En función del termociclador, para realizar el análisis cuantitativo, una vez analizados los datos relativos a la fluorescencia
proveniente de dos canales, FAM (detección del patógeno) y VIC (control interno positivo, JOE) y validados los resultados,
puede ser necesario desactivar el canal que lee la fluorescencia del control interno positivo. Para más información
consultar el manual del equipo correspondiente.

LOCALIZACIÓN Y RESOLUCIÓN DE PROBLEMAS
1. Detección de señal de fluorescencia en el canal FAM en el control negativo de extracción.
Posible causa Solución
Contaminación ocurrida durante el
proceso de extracción
Repetir el proceso de extracción del ADN y el desarrollo de la
PCR usando nuevos reactivos.
Asegurarse de que el espacio de trabajo, equipos y los
diferentes instrumentos necesarios para la elaboración de la
PCR están descontaminados, libres de ácidos nucleicos.
2. Detección de señal de fluorescencia en el canal FAM (patógeno) en los controles negativos de PCR.
Posible causa Solución
Contaminación ocurrida durante la
preparación de la PCR.
Repetir la reacción de PCR con nuevos reactivos y si es posible
varias réplicas de cada muestra.
Si se utilizan tubos, cerrar cada uno de ellos directamente
después de la adición de la muestra.
Como norma estricta el control positivo se debe añadir el
último y en un espacio físico distinto de dónde se añaden las
muestras problema.
Asegurarse de que el espacio de trabajo, equipos y los
diferentes instrumentos necesarios para la elaboración de la
PCR están descontaminados, libres de ácidos nucleicos.
3. Ausencia de señal de fluorescencia en el canal FAM en los controles positivos de PCR.
Posible causa Solución

INTRODUCTION
INgene q ParaTB Probes is a real-time PCR kit suited for the detection and quantification of Mycobacterium avium subsp.
Paratuberculosis (MAP). Under optimal conditions it is able to detect 1 MAP copy per reaction.
The kit contains all necessary reagents and enzymes in two mixes. When mixed together, they are at the required
concentrations designed to detect and quantify MAP simply by adding the DNA of the problem sample. This results in
amplification of a single-copy F57 gene region which is specific to the pathogen. This kit makes use of a Taqman probe marked
with FAM-BHQ1®. On the other hand, each Master Mix provides a positive internal control (PIC) with primers and a Taqman
probe marked with JOE-BHQ1®, allowing detection of false negatives due to an inhibition of the PCR. In addition, it contains
a passive reference fluorochrome, ROX, allowing passive normalisation of the signal avoiding possible quantification errors
deriving from pipetting differences. When using Applied Biosystems' ABI PRISM PCR models in real time, this ROX is necessary
to prevent interferences between the data collected from various emission channels.
To determine the number of MAP copies and as a positive PCR control the kit includes a standard positive control with a
known number of MAP copies. This control allows generating the standard curve which links the number of pathogen copies
with the cycle threshold (Ct) value.
The kit has been tested for every system from Applied Biosystems (StepOne, HT7300, HT7500) and LC480 from Roche.
Although there is no reason not to work with thermocyclers from other commercial houses, as long as they have at least two
fluorescence channels, Ct values may experiments changes.

GUIDELINES FOR THE CORRECT CONSERVATION OF THE KIT COMPONENTS
On receiving the kit, add the contents of reagent A to the tube of reagent B and homogenise the mix (henceforth called AB).
Keep the kit at -20ºC until use. If the reagents A and B cannot be mixed immediately upon receipt of the kit, keep the kit at
-20ºC until reagents can be mixed. For longer storage periods, the components of the kit must be maintained at -20ºC and
are stable for 1 year from the manufacturing date (see expiry date on packaging).

PRECAUTIONS TO AVOID CONTAMINATION
The following points should be read with care:
Disposable items must be DNAse and RNAse-free.
Use DNAse and RNAse-free distilled, autoclaved water (25 min., 120ºC).
Use sterile filtered tips.
Ensure good homogenisation of the kit components once defrosted.
Maintain the A and B mixes stored in ice at all times. Exposing them to temperatures above 4ºC reduces the efficacy
of the PCR.
Repeated cycles of freezing and thawing may reduce the sensitivity of the reagents. Protect them from exposure to
light until use.
To avoid contaminations giving rise to false positives it is important to:
Physically separate the positive PCR control from the remaining reagents of the kit.
To carry out any handling of samples to be tested in a different location/room from the one where the amplified
products are being analyzed.
Add the positive PCR control in a different location/room from the one where the mix is added and where the
samples to be tested are being handled.
SAMPLES COLLECTION AND TRANSPORTATION
The assay could be performed using any biological sample considered of interest by the clinician. Our technique has been
standardised and contrasted with a considerable number of samples. INgene q ParaTB Probes is suitable for detection and
quantification of MAP using different biological samples, preferably samples from faeces and pure cultures.
The following are some recommendations and/or limitations relating to the method of obtaining and sending the samples
to the laboratory:
The samples must be cooled from the moment they are obtained until they are processed. Processing needs to occur within
24 hours after obtaining the samples. In case a longer time is envisaged, we recommend freezing the samples.

DNA EXTRACTION
Any extraction procedure should be used which yields good-quality of the extracted material.
In case you have no extraction procedures optimized, INGENASA could provide extraction protocol and buffer under
request.
Ingenasa has tested the following methods for the DNA extraction:
- Method of Chomczynski and Sacchi (Phenol extraction).
- Automatic magnetic methods (Mag Max TM96 total NA isolation kit, Ref AM1840.)
- Colum purification methods: QIAamp® Viral RNA Mini Kit (QIAGEN), High pure PCR template Preparation kit
(ROCHE).
There are several commercial houses that provide high quality extraction kits that could be used following the manufacture
instructions. Houses like: Life-technologies, ROCHE, Qiagen, Marchery-Nalgene, others.
CAUTION: If the amplification is not going to be done immediately, preserve the DNA at -20 ° C .
AMPLIFICATION OF THE GENETIC MATERIAL
REQUIRED MATERIALS
- Crushed ice.
- DNA extracted from the samples.
- Mixture A – KEEP IN CRUSHED ICE AT ALL TIMES.
- Mixture B - KEEP IN CRUSHED ICE AT ALL TIMES.
- Or Mixture AB- KEEP IN CRUSHED ICE AT ALL TIMES.
- Positive amplification control A1 (MAP).
- DNAse/RNAse-free water.
PROCEDURE
Considerations prior to PCR preparation
It is recommended that each PCR experiment includes the following controls in addition to the problem samples:
Negative extraction control:
The sample is replaced by an elution buffer or water and is included in the extraction of nucleic acids, in parallel to the rest of
the samples.
Negative PCR control:
Includes all PCR reagents except the DNA, instead of which water is added. Allows verifying that the PCR reagents are not
contaminated.
Positive PCR control or standard curve in the case of quantification:
The dilutions of the standard curve, in addition to allowing the user to quantify MAP in the problem samples, are a positive
PCR control of known concentration, whose result must be positive. This allows checking that the selected PCR parameters
and conditions are adequate and that the reaction has not been subjected to any incident.

Replicates (optional):
To achieve a statistically optimal quantification it is recommended to perform 3 replicates both of the standard curve points
and of the problem samples, thus allowing calculation of the standard deviation with a minimum amount of 3 values.
Preparation of serial dilutions of the Standard Curve for absolute MAP quantification (optional)
To prepare the dilutions required to generate the standard curve it is recommended to use "Maximum Recovery" centrifugal
microtubes (not supplied) and Tris-HCl 10 mM pH 8 as diluent (supplied). The serial dilutions are performed using the
Standard Positive Control following these indications:
1. Pipette 90 μl of Tris-HCl 10 mM pH 8 into 7 tubes labelled 1-7 accordingly.
2. Pipette 10 μl of the Standard Positive Control into the tube labelled 1.
3. Vortex.
4. Change tip and again pipette 10 μl from tube 1 into tube 2.
5. Vortex.
6. Repeat steps 4 and 5 until the serial dilutions have been completed.
7. Use tubes 1, 4 and 6 as standards in the PCR.

Preparation of the PCR
1. Take mixtures A and B out from the cooler keeping them in crushed ice. If the reagents A and B have not be mixed
upon receipt of the kit, make sure that they are correctly homogenised before using. Add the contents of reagent A
to the tube of reagent B and homogenise the mix (call AB). AB mix should be kept on ice at all times!
2. Determine the number of reactions to be performed, including controls and points on the standard curve according
to table
3. Prepare and identify as many tubes for the amplification as samples to be processed, adding an additional tube for
the positive amplification control, and another one for the negative control.
4. Place (n x 15) μl of AB mix in a tube.

5. Once the above mix has been homogenised, 15 μl per sample to be analyzed are dispensed into n PCR tubes.
6. 10 μl* of DNA from each problem sample, 10 μl from the controls and 10 μl from each of the serial dilutions of the
Standard curve are added. (*Alternatively, between 2-10 μl of DNA and the rest of Tris-HCl pH 8, 10Mm, can be
added until the final sample volume of 10 μl is reached).
7. Set the thermocycler to the following conditions:

ANALYSIS AND INTERPRETATION OF RESULTS
Analysis of results
Each problem sample analyzed yields data regarding the fluorescence originated from channel FAM. Automatic
threshold analysis (threshold and baseline) is recommended.
If this option is not viable, the analysis can be carried out manually following the thermocycler instructions.
Possible results are summarised in the table:
FAM channel: pathogen detection
A positive result during amplification implies a typical fluorescence curve with a Ct value <38.
Samples with Ct´s comprised between 38 and 40 should be regarded as doubtful. In that case we recommend:
• Repeating the analysis.
• Sequencing the amplified product.
VIC channel: Internal Control detection (JOE)
A positive result during amplification implies a typical fluorescence curve with a Ct value <45. Normally, values are
comprised within a range of 30 <Ct < 45

Quantitative analysis (optional)
The number of MAP copies of each problem sample can be quantified through the cycle threshold (Ct) at the start of the
reaction. Thus, using the standard samples containing serial dilutions of known MAP concentrations, a calibration curve
can be plotted which correlates the Cycle thresholds to the logarithm of the initial amount of template, i.e. the number of
MAP copies contained in the sample. Extrapolating the Cycle thresholds obtained for the problem samples on this curve
allows estimating the initial number of MAP copies in them.
Most currently available real-time PCR equipment include software to automatically perform an absolute quantification,
if all parameters are set correctly.

Depending on the thermocycler, after the analysis of the data relating to the fluorescence obtained via the FAM and VIC
channels (detection of pathogen and internal positive control JOE, respectively) and after validation of the results, a
quantitative analysis can be performed, for example, requiring deactivating the channel which reads the fluorescence of
the internal positive control. Please consult corresponding user manual for further details.

TROUBLESHOOTING
