Information for filling in the "Application for a Final Thesis" and the

"Examination File"

This document consists of two parts: Application for a Final Thesis and Examination File

You can fill in the document online (JavaScript needs to be activated) or download the “.pdf file” to your computer. Forms filled by hand cannot be processed! We suggest to open this form with the free Adobe Reader software. You can only fill in required or optional fields; all other fields will be filled in automatically or by the Head of the Board of Examiners (HoBE) or the Faculty Service Team. Use the Tab key to navigate from one field to another. The mouse-over will reveal some quick tips for filling in the respective field. Printing is only possible when you have filled in all required fields. You need to fill in only the first page. Please fill in your current contact information (telephone, address, email address - do not use the HAW email address) to ensure that the supervisors, Head of the Board of Examiners (HoBE) or the Faculty Service Office (FSO) can contact you in case of further enquiries. Please print the whole document (14 pages not including this page). Please sign the Application for a Final Thesis (page 2). Please present the Application for a Final Thesis to your first and second supervisor and the HoBE (Head of the Board of Examiners) for approval and signature. After the Application has been signed and a deadline for submission has been added by HoBE, hand in this form with the following documents to the Faculty Service Office (FSO):

• A single copy of the academic degree of the 2nd supervisor • Accompanying sheet

Note: Only if the documents are submitted your application will be processed. If you require an extension, please ask the first and second supervisors first for their approval. The HoBE will inform the FSO and they will send you the new deadline via email. When you submit your thesis (two printed copies and one CD, labeled!) to the Faculty Service Office, please do not forget to hand in the following:

• The Examination File • The statutory declaration, that you are the author of the final thesis • The declaration concerning a publication signed on page 3 • The abstract of your Thesis (max. one page) • All remaining pages

If you want you may give the copies to the advisors peronally. In this case sign it on the Examination File. The Faculty Service Office will send the advisor forms to the advisors with detailed insturctions for academic appraisals. You will then receive a routing slip that needs to be signed by:

• The responsibles of the laboratories • The facility managers (key) • And a staff member of the library

Only if you have submitted the routing slip will your examination certificate and Masters certificate be awarded. For Bachelor students: If you have completed the requirements, you must hand in the form for issuing a Bachelor degree certificate. http://www.haw-hamburg.de/fakultaeten-und-departments/ls/services/fsb-ls/download-fuer-studierende.html For Master students: If you have completed the requirements, please hand in a list of graded modules to the Head of the Board of Examiners. Important: Please note that your Helios account will be deactivated 70 days after exmatriculation. After this date, you will not be able to print your transcript of records. Your HAW email account will be deactivated 1 year after exmatriculation. Until then, you will be able to use it. Once your examination certificate and Bachelor/Masters certificate have been edited by the Faculty Service Office, you will be notified by email of the relevant information.
