Instructions to the candidates:

1. Download the application form from the University website www.sduu.ac.in

2. Fill up the application with all relevant details.

3. Attach photocopies of all the relevant documents as detailed in the application

form.

4. Provide 5 copies of recent photographs. Write your name and Uni-GAUGE PGMD

2015 application number on reverse side of the photograph.

5. Ensure that the Demand Draft of Rs.500/- towards application fee is enclosed.

Write your name and Uni-GAUGE PGMD 2015 application number on reverse

side of the demand draft.

6. Please use A4 size envelope to send the duly filled application form to the

Registrar, SDUAHER, Kolar.

7. Super scribe on the Envelope “Application for Admission to Post Graduate

Medical Degree/ Diploma courses” 2015-16.

8. Please send the duly filled in application form along with the DD well in advance

by speed post or in person by 5.30 p.m. on Friday 20th March 2015.

9. The university shall not be responsible for the non-receipt/delay of the

application forms.
