Infor Access

To log into Infor, click on the Infor Landing Page icon on your computer desktop or you can access thelink by logging into MyHR at https://myhr.cuyahogacounty.us and scrolling down to the Infor Landing PageAccess section. For login assistance you can follow the appropriate login instructions on the MyHR homepageor contact the IT Support Desk at (216) 443-8007 or at [email protected]. Applicationaccess instructions differ depending on if you are logging in on the County network or logging in from theinternet.

Completing 2019 Benefits EnrollmentOpen Enrollment will be active from October 21st – November 4th, 2019.

1. Click the Open Enrollment link on the homepage.

2. Read the instructions, then click Next in the lower right corner.

3. Your current benefits will display. Click through each benefit type. The dependents that are enrolledin those plans (if applicable) will display under the Enrolled Dependents section. Review thisinformation for accuracy. Click Next in the lower right corner.

4. The View, Update or Add Dependents to your Benefit Plans screen displays. This screen listsdependents that you have identified to be covered by one or more of your plans. If you don't need tomake any updates to your dependents click Next.

Adding Dependents

5. If you have dependents that need to be covered under any of your plans, click Add to AddDependents

6. In the Add Dependent window, fill in all required fields marked with an asterisk *

7. Click OK to save the dependent

8. Continue this process to add all dependents

9. Click Next in the lower right corner

Adding Beneficiaries

10. The View, Update or Add Life Insurance Beneficiaries screen displays. This screen lists thebeneficairies that you have identified to be covered by your plan(s). If you don't need to make anyupdates to your dependents click Next.

11. If you have additional life insurance beneficiaries to add, click Add and complete the Add Beneficiaryform

Cuyahoga County provides a Basic Group Term Life insurance to each eligible full-timeemployee unless other benefits are provided through your union contract. The County

provides eligible employees with a $10,000 Basic Group Term Life policy.

Enrolling in Benefit Plans

12. The Enrollment page displays. The Enrollment Worksheet & Eligibility Survey are available on thispage. Click View Worksheet to see a listing of all of the benefit plans. The worksheet will open inanother window in your web browser. The benefit plans that you are currently enrolled in has acheckmark next to them. Close the enrollment worksheet to return to the Enrollment page.

13. Click Eligibility Survey. Answer the questions by clicking in the circle next to the appropriate Yes orNo answer under each question. Click OK when finished.

14. Click next on Enrollment tab to open the Medical Plan Options page.

15. Scroll down to view all Medical Plans. If you are enrolled in a Medical Plan for 2019 a checkmarkwill display next to that plan. Click on the plan that you are enrolled in and your covered dependentswill appear under the Enroll Dependents section. If you need to remove a dependent from the plan,uncheck the box under the select column to the left of the dependents name.

16. If you are changing plans or enrolling for the first time you will need to click on the box to the left ofthe plan you are choosing to select the appropriate plan. Select the appropriate Medical Plancoverage and option (Employee Only, Employee+Spouse, Employee+Children,Employee+Family[Spouse & Children]).

17. If applicable, ensure all appropriate dependents are selected for the plan.

18. Click Next in the lower right corner when you are ready to move on.

19. If you are enrolled in a Dental Plan for 2019 a checkmark will display next to that plan. Click on theplan that you are enrolled in and your covered dependents will appear under the Enroll Dependentssection. If you need to remove a dependent from the plan, uncheck the box under the select columnto the left of the dependents name.

20. If you are changing plans or enrolling for the first time you will need to click on the box to the left ofthe plan you are choosing to select the appropriate plan. Select the appropriate Dental Plancoverage and option (Employee Only, Employee+Spouse, Employee+Children,Employee+Family[Spouse & Children]).

21. If applicable, ensure all appropriate dependents are selected for the Dental plan

22. Click Next in the lower right corner

23. If you are enrolled in a Vision Plan for 2019 a checkmark will display next to that plan. Click on theplan that you are enrolled in and your covered dependents will appear under the Enroll Dependentssection. If you need to remove a dependent from the plan, uncheck the box under the select columnto the left of the dependents name.

24. If you are changing plans or enrolling for the first time you will need to click on the box to the left ofthe plan you are choosing to select the appropriate plan. Select the appropriate Vision Plancoverage and option (Employee Only, Employee+Spouse, Employee+Children,Employee+Family[Spouse & Children]).

25. If applicable, ensure all appropriate dependents are selected for the Vision Plan

26. Click Next in the lower right corner

Enrolling in Life Insurance

27. Basic Group Term Life Insurance will display.

This is employer paid – no employee contributions

This screen will not appear for employees in AFSCME 1748 and 2927

Cuyahoga County provides a Basic Group Term Life insurance to each eligible full-timeemployee unless other benefits are provided by the County through the union contract. The

County provides eligible employees with a $10,000 Basic Group Term Life policy.

28. In 2020, the Couny’s Life Insurance provider is changing from Guardian to MedMutual Life. You willsee your current $6,000 Basic Life Insurance coverage from Guardian which will end in 2019. Clickon the box to the left of MedMutual Basic Group Term Life Insurance to elect the new coverage fromMedMutual Life. This is employer paid – no employee contributions.

29. Select all appropriate beneficiaries for the plan by clicking on the box to the left of their name.

When you select a beneficiary, you will need to complete all required fields marked with ared asterisk * and click OK

If you need to add an additional beneficiary, click Add in the Designate Beneficiaries sectionand complete the Add Beneficiary form

30. Click Next in the lower right corner

31. If applicable, select the appropriate Supplemental Group Term Life Insurance benefit

Cuyahoga County offers employees the opportunity to purchase Supplemental Group Term LifeInsurance in $10,000 increments up to $500,000 through MedMutual Life Insurance. No

Evidence of Insurability (EOI) is required for Supplemental Life if you are less than 65 years ofage. If you are age 65-69 you can enroll in $20,000 without EOI and if you are 70+ all newly

elected amounts are subject to EOI. EOI is an application process in which you provideinformation on the condition of your health.

A benefit reduction schedule applies if you are 70 or older. The Guarantee Issue amount, fortimely enrollment, is $500,000 for employees less than age 65, and $20,000 for employee ages

65-69. If age 70+, Evidence of Insurability is needed for all amounts.

32. If you elect Supplemental Group Term Life Insurance, you will need to type in the amount ofcoverage you’re electing in increments of $10,000, up to $500,000

If you waive the Supplemental Group Term Life Insurance, you will not see this step

33. Select all appropriate beneficiaries for the plan

When you select a beneficiary, you will need to complete all required fields marked with ared asterisk * and click OK

If you need to add an additional beneficiary, click Add in the Designate Beneficiaries sectionand complete the Add Beneficiary form

35. If you have a spouse and/or dependent children (under age 26 and unamrried), select DependentLife Insurance Spouse/Child(ren).

This is employer paid – no employee premiums

This screen will not appear for employees in AFSCME 1748 and 2927

36. Select all appropriate dependents for the plan

If you need to add an additional dependent, click Add in the Enroll Dependents section andcomplete the Add Dependent form

Enrolling in Flexible Spending Accounts

38. Select to enroll in or waive a Medical Flexible Spending Account (FSA)

The Medical FSA is used to pay for qualified health care expenses, not covered by otherinsurance, including annual deductibles, co-insurance, copays, eyeglasses, contacts, etc.

39. If you elect a Medical Flexible Spending Account, you will need to type in the amount of moneyyou would like to put in the account

Unused balances will be forfeited. Your last day to incur claims is March 15, 2021 and yourlast day to submit the incurred claims for reimbursement is May 31, 2021.

40. Click Next in the lower right corner

41. Select to enroll in or waive a Dependent Care Flexible Spending Account

The Dependent Care FSA is for child care expenses for children under thirteen (13) years oldand they must be your dependent under federal tax rules. You may also use this plan for the

care of a spouse or dependent who is incapable of self-care and regularly spends at leasteight (8) hours per day in your home.

42. If you elect a Dependent Care Fexible Spending Account, you will need to type in the amount ofmoney you would like to put in the account

For your 2020 plan year, you have until March 31, 2021 to file claims for expenses incurredby December 31, 2020. Unused balances will be forfeited.

43. Click Next in the lower right corner

Submit Benefit Elections

44. Review your elections on the Review Summary of Benefits and Submit page for accuracy

You can verify the specific plan(s) you enrolled in by clicking through the menu to your leftby the green checkboxes and also on the next screen after you hit submit

45. Click Submit in the middle of the screen

46. Complete the electronic signature

47. Click OK

48. Click the PDF image to save/print a copy of your open enrollment elections

You will see the appropriate benefit elections on the PDF confirmation page
