A. For Licensure Personal appearances will be held in Conference Center Room C at 8:30 a.m
1. Nicole Seimetz RN Endo Failure to Disclose
Ms. Seimetz appeared for her personal appearance regarding her application for licensure. She currently
has an unencumbered nursing license in the state of Illinois. In 1999, she has in a fight. She did not realize
this was still on her record. She thought it had been expunged. She has not had any other issues. Ms.
Owens recommends that her license be issued free and clear. The following people were present during
the personal appearance: India Owens, Robin Riebsomer, Nicole Schuster, and Carri Burke.
2. Ronan Onita LPN Exam Graduated 2012/ Previous missed appearances *
Ronan Onita failed to appear for the 2nd time. Mr. Coto recommends the license application is denied.
3. Chinyere Oguayo RN Exam Previous Testing Attempts *
Chinvere Oguayo failed to appear for the 2nd time. Mr. Coto recommends the license application denied.
4. Heather Sue RN Endo Failure to Disclose * VACATED
5. Siyon Jones LPN Exam Grad Date 2013/Previous Testing Attempts *
Siyon Jones failed to appear for the 2nd time. Mr. Coto recommends the license application is denied.
6. Sharon Wilson RN Endo Failure to Disclose *
Sharon Wilson failed to appear for the 2nd time. Mr. Coto recommends the license application denied.
7. Kori Jones RN Exam Failure to Disclose
Kori Jones appeared for a personal appearance. Ms. Jones applied with for an RN license by examination
and checked “yes” to question 3 on her license application. In February 2010, she was arrested for
domestic battery. She located her fiancé at the time at the movies with another woman. She had been
going through his text messages in the lobby when he smacked the phone out of her hand and she
smacked him. The police were called and she was arrested. She was required to complete a Battery
Intervention program for 6 months and the charges were dismissed. Ms. Jones provided copies of court
documents showing that the charges have been dismissed. Mr. Coto recommends Kori Jones may sit the
exam. Her license may be issued free and clear upon passing the NCLEX.
8. Vladimir Najarro RN Exam Previous Testing Attempts
Vladimir Najarro failed to appear for the 2nd time. Matter will be rescheduled one time.
B. For Renewal- Personal appearances will be held in Conference Center Room C at 8:30 a.m.
1. Heather Taylor LPN Renewal (Suspension) Failure to provide statement
Heather Taylor failed to appear. Matter may be rescheduled one time.
2. Kimberly Repsold LPN Renewal Failure to provide statement – vacated
3. Shannon Simmons LPN Renewal Failure to provide statement
Shannon Simmons appeared for a personal appearance. Ms. Simmons applied for an LPN
renewal and answered “yes” to question 5. She had quit but they marked her as terminated. She
only worked there for 4 months. She’s been working as a nurse for 23 years. Three years ago she
was terminated from Cypress Grove. She was accused of leaving medication on the cart 2 years
ago. Mr. Coto recommends that she is renewed free and clear.
4. Bryan Schamber LPN Renewal Failure to provide statement
Bryan Schamber appeared for a personal appearance. Mr. Schamber applied for an LPN renewal
and answered “yes” to question 5. Schamber was terminated for not having his charting
completed in time. He had been working there for 3 years. He’s been a nurse for 14 years. When
asked if he’s been terminated before, he states that he was terminated in the past for being
accused of taking a lunch break without clocking out. He has not working as a Nurse since

October 2015. He is currently taking of his Grand Mother. Mr. Coto recommends that Bryan
Schamber’s license is renewed free and clear.
5. Regina Reinoehl LPN Renewal Failure to provide statement
Regina Reinoehl failed to appear. The matter may be rescheduled one time.
6. Janet Fekkes LPN Renewal Positive Response *
Janet Fekkes failed to appear for a 2nd time. Mr. Coto recommends the renewal application is denied.
7. Lauren Walker LPN Renewal Positive Response *
Lauren Walker appeared for a personal appearance. Ms. Walker applied for an LPN renewal and
answered “yes” to question 5 on her renewal application. Ms. Walker received a letter of
reprimand and failed to pay the Board Fines from an October 2015 Letter of Reprimand. She had
moved to Florida and was homeless because she could not find a Nursing job with a disciplined
license. She had appeared in December 2016 for a personal appearance but owed State Taxes and
the matter was reset until it was paid. Ms. Walker claimed that she spoke to someone about
lowering the Board Fines. That cannot be done during a personal appearance and requires an
administrative hearing. Mr. Coto recommends Lauren Walker’s license is issued free and clear.
However, a consumer complaint will be filed to the Office of Attorney General. Ms. Walker agrees
to the terms.
8. Mary Burkart LPN Renewal Positive Response *
Mary Burkart failed to appear a 2nd time. Mr. Coto recommends that the renewal application is
denied.
9. Suzanne Zimmerman LPN Renewal Positive Response *
Suzanne Zimmerman appeared for a personal appearance. Counsel Lorie Brown was present. Ms.
Zimmerman applied for an LPN renewal and answered “yes” to question 5. Zimmerman was
working at Rittenhouse of Portage. She had returned to work after completing cancer treatment
and was only cleared to work for 14 hours a week but her DON was scheduling her for more
hours. She had asked for less hours but they would not cooperate. She was accused of taking

Renewal and answered “yes” to question 5. In March 2016 McClendon was working at Elkhart
Rehab Center. She was terminated when 24 OxyContin Tablets were missing during a medication
count during a shift change. She immediately took a drug screening and it came back negative. A
complaint had been filed with the Office of Attorney General but it was closed. She had been
working there since 2002 and has been a Nurse since 1989. Mr. Coto recommends that her license

found guilty of Health Care Fraud in Federal Court. She was being investigating for claiming
hours she did not work in December 2015. She was working Home Health. July 2016, she pled
guilty and was charged. Barnett was sentenced 270 days in a halfway house. She is still living the
halfway house. She will be on federal probation for 2 years. She is on a payment plan to pay back

Emergency Room but didn’t feel fit to drive and pull into the driveway across the street from her
home. She had her 9 year old daughter with her. The police officer called an ambulance for her
but she was arrested. The charge was dismissed. The case was then reopened and the charges area
Operating a Vehicle While Intoxicated Endangering a Person and Operating a Vehicle with
Schedule I or II Controlled Substance (or its Metabolite) in Person's Body. Mr. Coto recommends
that Martha receives an ISNAP evaluation. If she is not a candidate she can be renewed free and

Catherine Cloutier appeared for a personal appearance. Ms. Cloutier applied for an LPN license
by examination. Ms. Cloudier answered “yes” to question 6 on her renewal application. Cloutier is
a recovering addict. She became addicted to prescription pain medication when she was having
complications with her menstruation cycle. She started attending Narcotics Anonymous after
reaching out to friends. She’s been sober for 3 years now. Mr. Coto recommends that Ms. Cloutier
may sit for the exam. Catherine Cloutier’s license may be issued free and clear upon passing the
NCLEX.
17. Donna Colwell RN Endo Failure to Disclose
Donna Colwell appeared for a personal appearance. Ms. Colwell applied for an RN license by
Endorsement. She holds an active and unencumbered license in New York. Colwell failed to
disclose a criminal matter on her license application. In February 2014, she was arrested for check
deception in Kentucky. She was not aware she was being served by the Sheriff. She had written
the check while she was having marital problems. She paid back restitutions and the matter was
resolved. Mr. Coto recommends Donna Colwell’s license is issued free and clear.
18. Megan Foster RN Endo Grad Date 2009/Positive Response
Megan Foster appeared for a personal appearance. Ms. Foster applied for a RN license by
Endorsement. She holds an active and unencumbered license in Ohio. She answered “yes” to
question 4 on her license application. In April 2006, she was working as a STNA at a long term
care facility. She had taken a picture of a resident that did something funny and showed it to other
staff members. She did not realize she had done anything wrong. She deleted the photo and was
terminated. Mr. Coto recommends Megan Foster’s license is issued free and clear.
19. Kimberly Climack RN Exam Previous Testing Attempts
Kimberly Climack appeared for a personal appearance. Ms. Climack applied for an RN license by
examination. She graduated from ITT Tech March 2016. She has failed the NCLEX 3 times.
Climack currently holds an active CNA license in Indiana. She completed the Kaplan review
course December 2016 with over a 95% probability of passing.. Mr. Coto recommends that Ms.
Climack may sit for her exam. Kimberly Climack’s license may be issued free and clear upon
passing the NCLEX.
20. Margaret Hominick RN Endo Failure to Disclose
Margaret Hominick failed to appear. The matter may be rescheduled one time.
21. Kathleen Maddox Rn Endo Failure to Disclose
Kathleen Maddox appeared for a personal appearance. Ms. Maddox applied for an RN license by
Endorsement. She holds an active and unencumbered license in Georgia. In March 2014 Maddox
was arrested for financial card fraud. Her and her husband was going through a divorce. Her
husband at the time received a prepaid card in the mail and she used it. He pressed charged but
he later dropped it. The case was dismissed July 2013. Mr. Coto recommends that Kathleen
Maddox’s license is issued free and clear.
22. Cherone Farley RN Exam Grad Date 2011
Cherone Farley appeared for a personal appearance. Ms. Farley applied for an RN license by
examination. Farley graduated from Ball State University in 2011 and never sat for her Board.
She works for as a coordinator at a health facility but does not work in patient care. Mr. Coto
recommends that she complete a review course and receive a 95% probability of passing letter
before she can be made eligible to sit for the exam. After she passed the exam she will be required
to complete a Refresher Course with a Clinical Component. Her license may be issued free and
clear upon completing the Refresher Course.
23. Mandy Wright APN App Failure to Disclose
Mandy Wright appeared for a personal appearance. Ms. Wright applied for APN license and
failed to disclose a criminal matter on her license application. In October 2003, she was charged
for public intoxicated and possession of a minor. She was venturing from different parties
downtown Indianapolis with an open alcoholic beverage. She was only 19 years old at the time.

The charges were dismissed. She never had any other issues since. Mr. Coto recommends that her
license is issued free and clear.
24. Cortine Woods RN Endo Failure to Disclose
Cortine Woods appeared for a personal appearance. Ms. Wood applied for an RN license by
Endorsement. She hold an active and unencumbered license in Florida. Woods failed to disclose a
criminal matter on her license application. In December 1999 she was charged with assault. She
had asked her boyfriend at the time to move his belongings out supervised after an altercation had
taken place at work. When she got home, he was moving his belonging unsupervised another
altercation escalated that resulted in the police being called by the neighbor. One of the officers
was the same officer that responded to the previous call at her work and they were both arrested.
The case was closed December 1999. Mr. Coto recommends that Cortine Wood’s license is issued
free and clear.
25. Moreen Kamau RN Exam Previous Testing Attempts *
Moreen Kamau appeared for a personal appearance. Ms. Kamau applied for an RN by
examination. She graduated from Breckenridge School of Nursing in 2015 and has failed the
NCLEX 3 times. Her last exam date was October of 2016. Mr. Coto recommends that she submit a
95% probability of passing letter before she can be made eligible to test. Moreen Kamau’s license
may be issued free and clear upon passing the NCLEX.

Ms. Medcalf failed to appear for her personal appearance. This matter to be reset for February

2. Denise Overmyer RN Renewal Positive Response
Ms. Overmyer appeared for her personal appearance regarding her application for renewal. Her
license expired 2015. She provided information for the board member. She applied in for renewal
in 2015 but could not make it to the personal appearance so she withdrew. She has a license in
Ohio. She had an OWI in March 2014. This matter was reported to the Ohio board. She had a
tragic personal issue and she had a hard time handling it. Prior to that event she didn’t drink
since she had been 19 years old. She also had gastric bypass surgery at that time also. She was
required to take a 3 day alcohol intervention program, 6 six group therapy course, she received a
letter from the Ohio board that she had some CEUs to complete and they would close the case. She
completed the courses. She has not been assessed for alcohol dependency. It was a onetime stupid
mistake. There has been no other issues. She has been with her employer for 3 years and prior to
that it was 18 years. Ms. Owens recommends that her license be renewed free and clear. The

3. Catherine Dewitt RN Renewal Expired Since 2001
Catherine Dewitt appeared for a personal appearance. Ms. Dewitt applied for an expired renewal
in 2011. Illinois is her home state and is active and unencumbered. She has not practiced since
2001. She was raising her children and her husband fell ill. She was an unpaid caregiver in Illinois
for her family. Her husband is an ER doctor and she frequently reads his medical journals. She
wants to return to nursing to be a Parish Nurse and is about to taking a Parish Nurse course in
February that covers the similar topics as the Refresher Course. She brought an advocate from
the program to help explain what the course entitles. She will send the syllabus of the course for
the Full Board to review. She needs an active license in order to take the course. Mr. Coto
recommends that Catherine Dewitt is renewed free and clear since she has been a caregiver and is
expected to take the Parish Course. Ms. Dewiit has been made aware Full Board may disagree
with his decision.
4. Debbie Holton RN Renewal Expired Since 2011
Ms. Holton failed to appear for her personal appearance. The renewal application is denied.
5. Shari McNeill LPN Renewal Positive Response
Ms. McNeill appeared for her personal appearance regarding her application for renewal. She
answered yes to question number 3. She had an altercation with her husband. She was arrested in
her home. This occurred in December 9, of 2015. She is currently on criminal probation. She goes
into see her probation officer tomorrow and that should be the end of her probation. She
submitted court docs to be reviewed. After the plea agreement the only charge she had was
disorderly conduct. They are still married. She is not currently working. She has not worked since
2009. She has been taking care of family members. She completed treatment for alcohol and
domestic abuse. She is currently involved in an aftercare program through the Y in South Bend.
She has a bench trial for February. She is not aware of this. The charges date from April 2016.

This was a result of a false charge against her. This was put in from her husband. She states her
husband will not testify in this matter. This cause number is different than the other one. She also
has a violation of her probation. The report her husband made was the probation violation. Ms.
Owens recommends that her license be renewed on probation with the following conditions:
ISNAP evaluation, completion of an RMA if meets criteria, completion of all criminal matters, 3
CEUs on ethics, 3 CEUs on professionalism, completion of a refresher course with a clinical
component before working, notify board of any relapses and/or arrest, quarterly reports, signed
board order, and she must practice in a supervised settings, no home health or hospice. Ms.
McNeill agrees to these terms. The following people were present during the personal appearance:

6. Jada Harrison LPN Renewal Positive Response
Ms. Harrison appeared for her personal appearance with Counsel Todd Ess. She answered yes to

patch on a patient. This happened in August 2015. She was caring for a hospice patient that day.
She went to remove the patch and didn’t see the other one. She put a patch on. There was no harm
with the patient. She was terminated in 2010 when she was a new nurse. She is currently working
with no issues. Ms. Owens recommends that her license be renewed free and clear. The following

Ms. VonWiller appeared for her personal appearance regarding her application for renewal. She
answered yes to question number 3 on her renewal application. She received an OWI in April of
2015. She does not remember what her BAC was but she knew it was high. When she was a
teenager she had a drinking driving charge. She has completed an IOP treatment. She provided
documentation. She is on criminal probation until June 8th 2017. She does not remember how
many drink she had that night. She knows it was a lot. She does have memory lapses from that
night. She is currently employed but has been off for 6 weeks due to foot surgery. Ms. Owens
recommends that her license be renewed on probation with the following conditions: ISNAP
evaluation, completion of RMA, if eligible, completion of criminal probation, 6 CEUs on impaired
nursing, 6 CEUs in professionalism, signed board order, quarterly reports, must report further
criminal matters and/or relapses. Ms. VonWiller agrees to the terms of the probation. The

9. Julia Walker LPN Renewal Failure to Provide Statement
Ms. Walker appeared for her personal appearance regarding her renewal application. She
answered yes to question number 5 on her renewal application. She was involuntary discharged
waiting to insubordination. She had doctor’s orders to have a resident discharged. Her executive
director felt that the order should be discontinued and she felt that was the wrong decision. It was
a verbal disagreement. She is currently working now with no issues. Ms. Owens recommends that
her license be renewed free and clear.
10. Mireille Washington LPN Renewal Failure to Provide Statement
Ms. Washington failed to appear for her personal appearance. The renewal is denied.
11. Harriet White LPN Renewal Failure to Provide Statement
Ms. White appeared for her personal appearance regarding her application for renewal. She
provided docs to the board. She answered yes to question number 3 on her renewal application. In
November 2015, she was arrested for disorderly conduct. She got into a fight with her husband.
She is on probation until the 19th of this month. She is on unsupervised probation. She is employed
with the same employer she was at when this happened. Alcohol was involved in this matter. She
doesn’t not drink now. She had 3 glasses of wine that night. Robin does not believe she will qualify
for an RMA. Due to the fact that the probation will be lifted on January 19 , 2017, Ms. Owens
recommends that the license be renewed free and clear.
12. Natalie Williams LPN Renewal Failure to Provide Statement
Ms. Williams failed to appear. Her renewal is denied.
13. Nancy Taylor LPN Renewal Failure to Provide Statement
Ms. Taylor failed to appear. Her renewal is denied.
14. Lisa Storey LPN Renewal Failure to Provide Statement
Lisa Storey appeared for a personal appearance. Counsel Lorie Brown was present. Ms. Storey
applied for an LPN renewal and answered ‘yes” to question 5. In August 2015, they discontinued
the alarm for a resident at risk for falling. She brought it up with the management that it was
dangerous. They claimed that she did not assess him after he got up off the wheelchair and fell.

Although she did access him. Lorie Brown was able to obtain video footage of the incident but she
believes it had been tampered with. Storey was terminated and she filed a grievance for wrongful
termination and it was successful. She had been working there for 11 years with no other issues.
Mr. Coto recommends that her license is renewed free and clear.
15. Catherine Mervyn LPN Renewal Failure to Provide Statement
Ms. Mervyn appeared for her personal appearance regarding her application for renewal. She
answered yes to question number 3. She had a DUI on 10/11/2014. This is not her first offense. In
2013, she had another OWI. She has taken classes and group therapy. She completed those at the
Samaritan center. She has not contacted ISNAP. The 2013 OWI was not reported on her previous
renewal. She is currently working. She has been a nurse for 32 years. She does not drink now. She
completed criminal probation. Ms. Owens recommends that her license be renewed on probation
with the following terms: ISNAP evaluation, 6 months active practice or length of RMA (if meets
criteria) whichever is longer, quarterly reports, signed board order, report any further criminal
matters and/or relapses to the board. 6 CEUs on addicted nursing, 6 on professionalism. Ms.
Mervyn agrees to these terms. The following people were present during the personal appearance:

Ms. Morris appeared for a personal appearance regarding her renewal application. She answered

injury to a resident. She has been a nurse for 23 years and there for a nurse for 22 years. This is
her first termination. She is currently working with no issues. She is currently working at
Kingston care. Ms. Owens recommends that her license be renewed free and clear. The following

Schuster, and Carri Burke.
17. Amy Nowling LPN Renewal Failure to Provide Statement
Amy Nowling appeared for a personal appearance. Ms. Nowling applied for an LPN Renewal and
answered “yes” to question 3. In July 2014, she was arrested for taking antibiotics that belonged
to her friend. She was taking doxycycline for herself because she was could not afford to see a
doctor at the time. She was arrested and charged with possession of legend drug without a
prescription. She served 3 months in jail and was placed on 1 year probation. Her probation
ended December 2015. In May 2016, she was arrested and charged with Neglect of a Dependent.
She is also being investigated by the Department of Child Services. The case is still open. Nowling
was unclear about the incident. Due to the 2 charges happening close to each other, Mr. Coto
recommends that Amy Nowling’s license is renewed on indefinite probation until the criminal
matter is resolved. As part of terms she will be required to submit a signed board order from her
employer and quarterly reports.
18. Angelia Parker LPN Renewal Failure to Provide Statement
Ms. Parker appeared for her personal appearance regarding her application for renewal. She
answered yes to question number 3 on her renewal application. The arrest was made in
occurrence with her son who has some issues. She is currently in the process of going to court to
get it dropped. Next court date is 2/17. She provided a letter from the Attorney stating the matter
will be dismissed. Her son and his girlfriend got into a fight. She got mad and made allegations on
the mom. Her son was involved in another issue and their house was invaded by the police and
they all were arrested. The charge was unlawful confinement resulting in bodily harm. The
girlfriend later recanted the story she gave. This matter is tabled until February full board so she
can have the court docs.
19. Tabitha Nora LPN Renewal Failure to Provide Statement
Ms. Nora appeared for her personal appearance regarding her application for renewal. She
answered yes to question number 5 on her renewal application. She was working at a facility for 6
years. A new DON came to the facility. She had issues with this same DON when she was a CNA
and trying to do clinical. As soon as she got that she started getting write ups and teachable
moments. She forgot to do a treatment but did sign that she had done it. She was terminated for
false documentation. She had never had any issues before the new DON arrived. This happened in
July 2016. She is currently working and has not had any issues. She has been at this facility since
February 2016. Ms. Owens recommends that her license be renewed free and clear. The

20. Fatemeh Esfandiarpour LPN Renewal Positive Response
Ms. Esfandiarpour appeared for her personal appearance regarding her application for renewal.
She answered yes to question number 5 on her application. She was working a full time job and a
PRN position. She was terminated from the PRN position for a positive drug test. Appeared- yes

to question number 5- terminated from her PRN position. The drug test came back positive for
cocaine. She has never done cocaine. She believes the tea her daughter brought her from South
America is the reason for the positive drug test. She looked on the computer and looked up the tea
and say that cocaine was in it. Her attorney had the tea tested and it confirmed a positive test for
cocaine. There has been a complaint filed with the AG’s office. She provided documents. She was
terminated from the PRN position but still remains at the full time position. Ms. Owens
recommends that her license is to be renewed free and clear. The following people were present

21. Britany Stowers RN Renewal Positive Response
Ms. Stower appeared for her personal appearance regarding her application for renewal. Laura
Iosue was here. She has a pending consumer complaint with the AG’s office. She had a PHSC in
front of the board. The matter has to deal with controlled substance. The agreement with the AG’s
office involves ISNAP but her license has been expired. She is currently on methadone and is in
the process of getting off. She was evaluated at Parkdale. She had possession of controlled
substance in September of 2015. She lost her job. She was charged. She had a vial of adavan. She
has a substance abuse history. She is on methadone for heroin use. When this happened she had
been using heroin for 2 years. She didn’t intentionally still the meds. She worked in the er and
forgot it was on her person. Her husband got on heroin and introduces that to her. She is not with
him anymore. There were times when she practiced under the influence. Ms. Owens recommends
the license be renewed on probation with the following conditions: completion of an ISNAP RMA,
completion of criminal probation, quarterly reports, signed board order, 6 CEUs on impaired
nursing 6 ethics, 6 on safe medication administration, report any relapses and/or future arrest,
limit practice to supervised settings only, no home health care or hospice. Ms. Stower agrees to

and was working on the Main Unit. The DON scheduled her to work another unit and she refused
to work both. Rather than resigning, she choose not to go in her next shift. She was terminated for
no call, no show in September 2016. She had worked there for a year. She currently works at the
Waters of Indiana since October and is the assistant of director of Nursing. She’s been working as
a Nurse since 2006. Mr. Coto recommends that Yvette Sissom license is renewed free and clear.

Teresa Smith appeared for a personal appearance. Ms. Smith applied for an LPN renewal and
answered “yes” to question 5. She was terminated in November 2015 from Pointe Village. She had
a Halloween party and her permission borrowed a resident’s shirt so they could dress like each
other for the event. Someone had called the State and filed a complaint. During their investigation
they claimed it was verbal abuse. She had never been in trouble before. She worked at Hampton
Oaks for 7 and a half years prior. Mr. Coto recommends that Teresa Smith’s license is renewed
free and clear.

Nancy Sons’ appeared for a personal appearance. Ms. Sons’ answered “yes” to question 5 on her
renewal application. July 7th 2016, she was suspended pending investigation at Terra Haute
Nursing and Rehabilitation. She had made a comment about an overweight resident. She said “He
can have what he wants, he’s obese anyways,” She had worked there for 11 years. She’s been a
Nurse for 21 years. She is currently not working as a Nurse and taking care of her grandchildren.
She worked at Meadows East prior. Mr. Coto recommends Nancy Sons’ license is renewed free
and clear.

Brittany McKinney appeared for a personal appearance. Ms. McKinney applied for an LPN
renewal and answered “yes’ to question 5. In December of 2015. She was at a pub with a friend.
Her friend had gotten into an altercation with another patron and the police were called. While
they were interviewing the witnesses, she was talking over the police officer. She was asked to be
quiet but she kept talking. She was arrested and charged with disorderly conduct. She was
required to complete 40 Hours of community service. Mr. Coto recommends Brittany McKinney’s
license is renewed free and clear.

Zella Green appeared for a personal appearance. Counsel Lorie Brown was present. Ms. Green
applied for an LPN Renewal and answered “yes” to question 5. She was working as an LPN at
Regional Hospital in Terre Haute. She was working 3 days a week while attending Ivy Tech to
obtain her RN degree. While on duty, a student nurse walked into a resident’s room unsupervised.
The student ordered Green to find a Tech to change the linen of the resident. This aggravated
Green because she was just a student nurse and was being disrecpectful. Green does not
remember what she said the student but she was terminated for being behavior/insubordinate.
Mr. Coto recommends that Zella Green’s license is renewed free and clear.
28. Amanda Bisher LPN Renewal Positive Response *
Amanda Bisher appeared for a personal appearance. Counsel Lorie Brown was present. Ms.
Bisher applied for an LPN renewal and answered “yes” to question 5. In July 2015 she was
terminated for not following company police and procedure. She had destroyed a narcotic without
a witness. She had many cards of non-controlled substances and 1 card of a controlled substance.
She was called away by another staff member. When she returned she flushed the medication
forgetting that one was a controlled substance. She self-reported to her supervisor and she was
terminated. She completed a drug screen at work that came up negative but Lorie Brown did not
have a copy. Bisher had drug evaluation done and submitted a copy of it into her file. Mr. Coto
recommends that her license is renewed free and clear.
