To provide an information, support and advice service to people with disabilities, long term conditions and their carers in ways that are sensitive to their needs and wishes.

To undertake delegated responsibilities for specific areas of work as directed by the Community Service Station Manager.

Main Duties:1 To provide accurate up to date information in suitable formats to meet the needs of the client groups attending the project.

2 To provide support and advice on the range of equipment available to enable clients to maximise their independence.

3 To provide support, advice and assistance to clients to enable them to access statutory, private and 3rd sector services to meet their individual needs.

4 To signpost clients to appropriate agencies.

5 To consult with service users and carers via questionnaires and focus groups to inform the development of the project.

6 To manage a caseload of clients and maintain computerised and manual case records in accordance with relevant legislation and the organisations policies and procedures.

7To work with other professionals to create referral pathways and processes.

8To have knowledge and understanding of Direct Payments and Personal Budgets policy and guidance.

9To contribute to the organisations monitoring and evaluation by:

a) maintaining monthly spreadsheets for client contacts and assessments for reporting purposes

b) submitting monthly progress reports to the Manager and other written reports to the Community Service Station Partnership Board and funders, as required.

10To demonstrate technical and professional expertise by maintaining knowledge of policies, procedures, guidance, research findings and best practice with a commitment to continuous professional development.

11To promote and encourage multi-agency working partnerships with voluntary, statutory and non statutory agencies working to meet the needs of the service users and their carers.

12Communicate information effectively by adopting the appropriate style when writing to individuals or groups or when using information technology.

13To contribute to the development and implementation of the organisations quality assurance systems.

14Demonstrate a flexible approach to change by adapting positively to new circumstances, seeing beneficial opportunities in change and encouraging others to do likewise.

STATEMENT OF MAIN TERMS OF EMPLOYMENTPLACE OF WORKYou will currently be required to work currently at/from Southport Community Service Station, 23-35 Scarisbrick Avenue, Southport PR8 1NW. You may be required to work from any other location as directed by the company. You will not be required to work outside the United Kingdom.

HOURS OF WORKYour normal hours of work are 35 per week, with an unpaid break will be allocated each day according to the length of your shift. However flexibility is needed and you may need to work different hours from time to time which could include evenings and weekends. You may be required to work additional hours when authorised and as necessitated by the needs of the business.

REMUNERATIONYour salary is currently 19,935 per annum payable monthly by credit transfer as detailed on your pay statement. Any work required to be undertaken in addition to normal working hours will be compensated by time off in lieu (TOIL). Further details of which are documented in the handbook.

COLLECTIVE AGREEMENTSNo collective agreements directly affect your terms and conditions of employment.

ANNUAL HOLIDAYSYour holiday year begins on 1st April and ends on 31st March each year. You will receive a paid holiday entitlement of 25 working days during a complete holiday year. On completion of five years service at 1st April your entitlement will increase in line with the following table:-

SICKNESS PAY AND CONDITIONSWe have a contractual sick/injury pay scheme (inclusive of SSP) which, on completion of 12 months service, provides payment during periods of certificated sickness as follows:-

Where a sickness period runs into another year (i.e. during e period of absence the length of service reaches a new qualification period) payments will be based on the sickness pay entitlement at the commencement of the sickness period. The employee must have returned to work for at least three months continuous service before being entitled to a further period of full and half pay as long as they have not used their full entitlement with the twelve month period preceding rule.

Where employees have reached the full entitlement of sickness pay in normal circumstances SSP rates only will apply.
